WHILE EN-ROUTE TO A SCENE CALL, A MEDICAL CREW MEMBER IN THE CABIN STARTED NOTICING THE EXTERIOR SEAL THAT HOLDS THE COPILOTS GREENHOUSE WINDOW COMING LOOSE. MADE AN UNEVENTFUL LANDING. ONCE ON THE GROUND, A MECHANIC REPLACED THE SEAL FOR THE WINDOW, AND RETURNED THE AIRCRAFT TO SERVICE.
THE PILOT & MEDICAL CREW WERE ENROUTE TO AN ACCIDENT SCENE WHEN THE PILOT FELT SOME FEEDBACK IN THE TAIL ROTOR PEDALS. ABORTED THE FLIGHT & PROCEEDED TO THE NEAREST AIRPORT FOR A PRECAUTIONARY LANDING. MADE AN APPROACH TO A RUNWAY TO ENSURE CONTROLLABILITY, THEN EXECUTED A GO-AROUND, & MADE ANOTHER APPROACH TO RNWY 14. THE TAIL ROTOR PEDALS CONTINUED TO HAVE SOME FEEDBACK, BUT THE ACFT WAS CONTROLLABLE. A RUN ON LANDING WAS MADE WITH MINIMUM RUN ON. THE ACFT WAS SHUT DOWN ON THE RNWY. DURING THE FLIGHT, THE PILOT WAS IN CONTACT WITH APPROACH CONTROL, AIRCOM & DID ADVISE THEM THAT HE WAS MAKING A PRECAUTIONARY LANDING. INSPECTED TAIL ROTOR FLIGHT CONTROL SYS, BELL CRANKS, CONTROL RODS, CONTROL CABLE, NO DEFECTS NOTED. INSPECTED TAIL ROTOR SERVO CONTROL, NO DEFECTS NOTED. NOTED SMALL AMOUNT OF MOISTURE INSIDE THE HYD RESERVOIR TANK CAP & FINGER SCREEN. CLEANED CAP & FINGER SCREEN THEN DRAINED HYD FLUID & SERVICED WITH NEW HYD FLUID. CHECKED TAIL ROTOR ACCUMULATOR PRESS, FOUND AT 185 PSI, SERVICED ACCUMULATOR WITH NITROGEN TO PROPER LIMITS IAW CURRENT AMM. APPROX 210 PSI. COMPLIED WITH GND RUN & VISUALLY CHECKED ALL THE FLIGHT CONTROLS WITH PEDAL MOVEMENTS, COULD NOT FIND ANY BINDING OR FEEDBACK. COMPLETED OPERATIONAL FLIGHT CHECK WITH NO FEEDBACK NOTED. RETURNED ACFT TO SERVICE.
AFTER STARTING THE ENG FOR A SCENE FLIGHT, THE NURSE & MEDIC DETECTED AN UNIDENTIFIED ODOR. ATTEMPTS TO LOCALIZE & IDENTIFY THE ODOR WERE UNSUCCESSFUL & WE ELECTED TO SHUT DOWN WITH NO FURTHER INCIDENT. AFTER ADDITIONAL GND RUN, THE ODOR WAS DETERMINED TO BE DUST & POLLEN IN THE HEATER DUCT. THE ACFT WAS RETURNED TO SERVICE.
PILOT ATTEMTED START PROCEDURE WITH NO SUCCESSFUL LIGHT OFF. FUEL WAS FOUND ON THE ENGINE DECK, DOWN THE DECK DRAIN TUBES, AND ON PORTIONS OF THE LT SIDE OF THE ENGINE. FOUND THAT THE LT INJECTOR TUBE WAS SHEARD FLUSH WITH THE END OF THE FLARE SUPPORT, .1250 IN PAST THE FACE OF THE B-NUT ON THE UPPER SIDE OF ASSY ON THE TUBE. ON INTIAL INSPECTION OF THE BREAK IN THE UPPER SECTION OF THE TUBE, REMAINING PART OF THE LINE WAS APPROX 5 MM OFF CENTER FROM WHERE THE SHEAR POINT IS, INDICATING THE LINE WAS UNDER SLIGHT TENSION. AFTER THE TUBE WAS REMOVED, INSPECTED THE LOWER SECTION OF THE TUBE AND FOUND SMALL DETENTS AND RUB MARKS AROUND THE OUTSIDE OF THE LINE MATCHING UP PERFECTLY WITH THE INNER SIDE OF THE FLARE SUPPORT. MEASURMENT AROUND THESE MARKS WHERE AT 0.2325", AT THE POINT OF WEAR MARKS RECORDED 0.2310
