CRACKS DISCOVERED IN TAIL CONE STATION 6616 DURING 1600 HOUR INSPECTION. AMC EO 32-5302-EO-2290 PREVIOUSLY COMPLIED WITH.
WHILE ON A 11 MILE FLIGHT FROM BASE TO HOSPITAL WE HAD THE TAIL ROTOR CHIP LIGHT COME ON. I BURNED THE CHIP AND THE LIGHT WENT OUT, BUT MADE THE DECISION TO ABORT FLIGHT. WHILE ON A LONG SHALLOW FINAL TO OUR BASE THE TAIL ROTOR CHIP LIGHT CAME BACK ON AND DID NOT GO OUT AFTER SECOND BURN. THE LIGHT REMAINED ON ONCE SAFELY LANDED ON OUR PAD AT BASE.
IN FLIGHT FUEL LEAK ON ENGINE #1 DUE TO 2 HARD FUEL LINES RUBBING TOGETHER DURING FLIGHT. REMOVED AND REPLACED DAMAGED PIPE IAW SAFRAN EMM 06 73-19-00-900. LEAK TESTS AND RUNUP SATIFACTORY.
UPON ARRIVING AT A SCENE JUST AS WE BEGAN THE OVERHEAD RECON THE TAIL ROTOR CHIP LIGHT ILLUMINATED. EXECUTED THE EP IAW THE AIRCRAFT EMERGENCY PROCEDURE, THE LIGHT CAME ON AGAIN AFTER THE SECOND ATTEMPT TO BURN OFF. THE FLIGHT WAS ABORTED, AIRCOM AND GROUND CONTROL WERE NOTIFIED AND WE RETURNED TO BASE. MECHANIC WAS CALLED IN AND PERFORMED INSPECTION AND CLEANING OF TAIL ROTOR CHIP DETECTORS. FOUND NO VISUAL CHIPS FOUND. PERFORMED 20 MINUTE MAINTENANCE RUN WITH NO FURTHER LIGHT. AIRCRAFT WAS RETURNED TO SERVICE.
WHILE PERFORMING AN INSP OF THE FUSELAGE, FOUND A CRACK IN THE SKIN BEHIND THE LT SLIDING DOOR TRACK. CONTACTED MFG IAW THE SRM FOR A REPAIR SCHEME.
(QMLD) DURING INSP OF THE ACFT, FOUND A CHAFE OF THE RT HORIZONTAL STABILIZER OPENING CAUSED BY THE BONDING BRAID. THE MM STATES TO CONTACT ECD OR REPRESENTATIVE FOR REPAIR. AEC HAS BEEN CONTACTED AND WE ARE AWAITING A REPAIR SCHEME.
