FLIGHT ATTENDANT REPORTS ELECTRICAL/OZONE SMELL WAS IN THE GALLEY FORWARD TO THE COCKPIT DOOR. LT OMEGA BEAM AT FR25 CORRODED AND CRACKED. REPAIRED LT OMEGA BEAM AT FR25 IAW AE EO EM--53MR-1842.
OPERATOR EXPERIENCED AN ON-WING CARBON RUPTURE EVENT FOLLOWING LANDING. THE EVENT OCCURRED ON THE NR 3 POSITION BRAKE ( RT INBD) PN 21707, SN 2398, AND RESULTED IN A COMPLETE RUPTURE OF THE PRESSURE PLATE AND NR1 ROTOR. PASSENGERS WERE REQUIRED TO PERFORM AN EMERGENCY DEPARTURE FOLLOWING REPORTS THA THE BRAKES WERE SMOKING, BUT NO INJURIES WERE REPORTED. INITIAL FEEDBACK SUGGENSTS THAT THE BRAKE HAD SUSTAINED 3787 CYCLES PRIOR TO RUPTURE. THE CAUSE IS STILL UNKNOWN PENDING INVESTIGATION.
ON FLIGHT 4176 THE TOWER ADVISED THAT AFTER TOUCHDOWN SAW A PUFF OF WHITE SMOKE FROM AIRCRAFT. DURING TAXI THE CREW NOTICED THE LEFT BRAKE PEDAL SOFT. THEN LOST NR 2 SYSTEM QUANTITY & PRESSURE AS THE CREW STOPPED THE AIRCRAFT. ON INSPECTION FOUND PIECES OF BRAKE AND HYDRAULIC FLUID ON THE GROUND. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 2, 3 AND 4 TIRE ASSEMBLY IAW AE MTX WC 32-700-11. REMOVED AND REPLACED THE NR 2 AND 3 BRAKE IAW AE MTX WC 32-600-14. THE AIRCRAFT WAS RETURNED TO SERVICE.
THE CAPTAIN DECLARED AN EMERGENCY ON FLIGHT 2704 DURING THE CLIMB THE CREW REPORTED THE NR 1 AND NR 2 COMMUNICATION RADIO MANAGEMENT UNIT FAILED AS UNABLE TO CHANGE THE FREQUENCY. THE CAPTAIN ELECTED TO RETURN TO DFW. THE AIRCRAFT LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NR 1 AND NR 2 RADIO MANAGEMENT UNITS. THE OPERATIONAL CHECKS WERE GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 2710 THE CREW REPORTED RECEIVED A BLEED 2 LEAK. THE CREW COMPLIED WITH AOM MESSAGE AND CLEARED. THE CAPTAIN DECLARED AN EMERGENCY AND LANDED SAFELY AT DESTINATION. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED O-RINGS ON NR 6 CONNECTOR IAW EMB 145 AMM 36-11-10 AND REMOVED AND REPLACED NR 8 CONNECTOR IAW EMB 145 AMM 36-11-10. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3294 DURING TAXI THE FLIGHT ATTENDANT AND PASSENGERS NOTED AND SMELLED SMOKE. THE CAPTAIN WALKED BACK AND NOTED THE SAME ODOR. THE AIRCRAFT WAS REMOVED FROM SERVICE. INSPECTED AREA AT 10BC AND REPLACED BURNT FLUORESCENT LIGHT HARNESS ASSEMBLY AT 9BC AND 10BC AND REPLACED FLUORESCENT TUBES AT 9BC AND 10BC IAW AMM 33-21-06. NO FURTHER ODORS NOTICED. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3906 DURING CLIMB THE CREW RECEIVED AN EICAS WARNING FOR NR 2 BLEED LEAK. THE CREW ELECTED TO RETURN TO ORD AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE AIR BLEED LINE O-RING AT JOINT NR 16 (UNDER CARGO FLOOR PANEL 272BF) IAW EMB 145 AMM 36-11-10. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 9808 AFTER TAKEOFF THE CREW REPORTED THE LANDING GEAR WOULD NOT RETRACT. THE CREW RECEIVED A LANDING AIR/GROUND FAIL MESSAGE ON EICAS. THE CREW ELECTED TO RETURN TO CMH AND LANDED WITHOUT INCIDENT. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE NOSE LANDING GEAR PROXIMITY SWITCH. THE OPERATIONAL CHECK WAS GOOD, REFERENCE EMB AMM 32-63-02. ALSO REMOVED AND REPLACED THE NOSE LANDING GEAR HARNESS, REFERENCE AMM 32-00-03. EXTENSION AND RETRACTION OF LOG SYSTEM REFERENCE EMB AMM 32-30-00. THE HARNESS PART AND SERIAL NUMBERS: P/N 14521495403, S/N NA. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING MAINTENANCE BASE VISIT FOUND THE REAR PRESSURE BULKHEAD DENTED/CREASED BEYOND LIMITS. THE AIRCRAFT WAS REMOVED FROM SERVICE. REPAIRED REAR PRESSURE BULKHEAD IAW EO EM-53MR-1438. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING PS-1 CHECK FOUND THE CABIN CEILING PANEL 263ALC EMERGENCY LIGHT ASSEMBLY HAD MELTED TO BULB AND CRACKED. THE AIRCRAFT WAS REMOVED FROM SERVICE. REMOVED AND REPLACED THE EMERGENCY LIGHT ASSEMBLY ON CABIN CEILING PANEL 263ALC IAW EMB145 AMM 33-50-08. THE OPERATIONAL CHECK WAS GOOD. THE AIRCRAFT WAS RETURNED TO SERVICE.
DURING THE MAINTENANCE BASE VISIT INSPECTION FOUND THE AFT PRESSURE BULKHEAD HAS A DENT, (AT 4 OâCLOCK POSITION VIEWED FROM INSIDE AFT AVIONICS COMPARTMENT LOOKING AFT). THE AIRCRAFT WAS REMOVED FROM SERVICE. COMPLETED DOUBLER REPAIR TO BULKHEAD IAW AEAI EO-53MR-1438. THE AIRCRAFT WAS RETURNED TO SERVICE.
ON FLIGHT 3962 DURING CRUISE A FLAP FAIL MESSAGE APPEARED AND REMAINED ON EICAS. THE FLAPS FAILED AT 0 DEGREES. THE CREW DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. AIRCRAFT REMOVED FROM SERVICE. THE AIRCRAFT WAS INSPECTED TO THE EXTENT NECESSARY FOR A FERRY FLIGHT AND FOUND TO BE IN SAFE CONDITION FOR A FERRY FLIGHT. REMOVED AND REPLACED SERVABLE FLAP MOTOR DRIVE IAW EMB 145 AMM 27-53-04. OPERATIONAL CHECK GOOD. AIRCRAFT RETURNED TO SERVICE.
