ONE CRACK FOUND AT B.L. 510, STATION 3627 W.L. 1349 MEASURING 30MM IN LENGTH. / TWO CRACKS FOUND AT STATION 4010, BUTT LINE 6, WATER LINE 1020 MEASURING 30MM & 50MM. / TWO CRACKS FOUND AT STATION 4010, BUTT LINE 0 & 1, WATER LINE 1020 MEASURING 8MM & 10MM. / ONE CRACK FOUND AT B.L. 250, STATION 4460 W.L. 1020 MEASURING 10MM IN LENGTH. / MULTIPLE CRACKS FOUND AT STATION 4010, BUTT LINE 1, WATER LINE 1020. THESE CRACKS 30MM, 15MM, 10MM, 15MM AND 20MM. THESE 5 CRACKS ARE ALL ATTACHED AND SPIDER OUT FROM ONE ANOTHER. / ONE CRACK FOUND AT STATION 4010, BUTT LINE 10, WATER LINE 1010 MEASURING 30MM.
1.5 INCH CRACK FOUND ON TRAILING EDGE OF BLADE FAIRING. CRACK STARTS AT 60MM FROM TOP OF BLADE FAIRING AND RUNS DOWN AND UNDERNEATH BLADE.
AFTER PILOT WASHED AIRCRAFT, BS 8081, WL 1300 ON PILOTS SIDE OF TAIL BOOM THERE WAS A 2.5 INCH CRACK SEEN BETWEEN THE MOUNTED STRINGER RIVET LINES. A SECOND CRACK WAS LOCATED AT BS 8085, WL 1400 ON PILOTS SIDE OF TAILBOOM THERE WAS A 2.25 INCH CRACK NOTED AFTER A DYE PEN INSPECTION ON THE UPPER STRINGER RIVET LINE. AFTER FURTHER INSPECTION, A THIRD CRACK WAS LOCATED AT BS 7826, 3 INCHES LEFT OF BL ON ATTACHMENT RING FOR FLUX VALVE AND VHF ANTENNA COVER SUPPORT. CRACK ON ATTACHMENT RING IS APPROXIMATELY 2.25 INCHES LONG THROUGH RING CHANNEL.
DURING DAILY AIRWORTHINESS CHECK, AN APPROXIMATELY 1/2" CRACK WAS FOUND ON THE RIGHT HAND TAILROTOR GEARBOX MOUNT BULKHEAD DOUBLER PN 109-0373-21-104. CRACK IS LOCATED IN THE RADIUS BEND OF DOUBLER TOWARDS UPPER RIGHT HAND EDGE BEHIND NUTPLATE.
AIRCRAFT WAS PREPARING TO TAKEOFF AFTER START UP, WHEN THE MAIN TRANSMISSION CHIP LIGHT ILLUMINATED. PILOT SHUTDOWN THE ACFT. REMOVED MAIN TRANSMISSION CHIP DETECTOR & FOUND IT SATURATED WITH METAL PARTICLES. IAW THE MM, DRAINED & FLUSHED THE MAIN TRANSMISSION OIL SYS, CHANGED THE OIL FILTER, & CONDUCTED A GND RUN. IT FAILED THE GND RUN UP. MAIN TRANSMISSION ASSEMBLY R & R. THIS TRANSMISSION ASSEMBLY HAD BEEN INSTALLED AFTER BEING REPAIRED ON 02 16 2016. THERE HAD BEEN TWO OTHER CHIP INDICATIONS BEFORE THE ONE THAT OCCURRED ON 05 16 2016.
