CREW REPORT STATED THAT WITH ONE PACK DEFERRED, RECEIVED PACK OVER PRESSURE WARNING ON T/O ROLL. ABORTED T/O. RESET AND SUCCESSFULLY TOOK OFF. NO RETURN TO GATE.
AFTER TAKEOFF CREW RECEIVED GEAR DISAGREE WITH GEAR UP. WOW INPUT FAIL AND NOSE STEERING MESSAGE. CREW DECLARED AN EMERGENCY. ADJUSTED NLG WOW SENSORS AND PERFORMED GEAR SWINGS. OPS CHECK GOOD.
DURING INSPECTION THE LEFT HAND OVERWING EMERGENCY EXIT LIGHT WAS FOUND IOPERATIVE AND REPLACED. A SATISFACTRY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RETURNED TO SERVICE.
DURING INSPECTION THE LEFT HAND FOWARD EMERGENCY LIGHT WAS FOUND INOPERATIVE. THE BULB WAS REPLACED AND A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED.
DURING MAINTENANCE INSPECTION THE 3RD EMERGENCY EXIT SIGN WAS FOUND INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE EMERGENCY EXIT SIGN BULB. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RELEASED FOR CONTINUED SERVICE
DURING MAINTENANCE INSPECTION, THE OVERWING EMERGENCY EXIT LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE LAMP IN THE EMERGENCY EXIT LIGHT. AN OPERATIONAL CHECK WAS SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
During preflight inspection, the crew reported that the forward emergency exit light was inoperative. Maintenance personnel removed and replaced the lamp in the forward emergency exit light. An operational check was satisfactory and the aircraft was returned to service.
FLT 6771 - IAD-GSP - DURING CLIMB, AIRCRAFT MADE AN UNSCHEDULED LANDING DUE TO LOUD NOISE COMING FROM COCKPIT RIGHT SIDE. AIRCRAFT RETURNED TO GATE WITH NO FURTHER PROBLEMS. MAINTENANCE INSPECTED AND SECURED OXYGEN SERVICE DOOR. OPS CHECK OK. AIRCRAFT WAS RELEASED AND APPROVED FOR RETURN TO SERVICE. (M)
IAD/BNA - FLT 6700 - DURING CRUISE AT 0745 HOURS ON 4-15-98, AIRCRAFT MADE UNSCHEDULED LANDING DUE TO TAT AND RT PITOT HEAT FAILURE. AIRCRAFT LANDED AT IND AND WENT TO THE GATE WITH NO FURTHER PROBLEMS. MAINTENANCE INSPECTED AND REMOVED AND REPLACED AIR DATA SENSOR CONTROLLER, OPS CHECK OK. AIRCRAFT WAS RELEASED AND APPROVED FOR RETURN TO SERVICE. (X)
IAD/IND - FLT 6724 - DURING APPROACH AT 1415 HRS ON 4-3-98, AIRCRAFT MADE ABORTED APPROACH INTO IND DUE TO LANDING GEAR FAILED TO EXTEND NORMALLY AND CREW USED EMERGENCY EXTENSION. AIRCRAFT LANDED AT IND AND WENT TO GATE WITH NO FURTHER PROBLEMS. MAINTENANCE INSPECTED AND PERFORMED GEAR SWINGS AND COULD NOT DUPLICATE PROBLEM AND RECEIVED NO FAULT CODES. OPS CHECK OK. AIRCRAFT WAS RELEASED AND APPROVED FOR RETURN TO SERVICE. (X)
FLT 6718 - ATL-IAD - DURING CRUISE, AIRCRAFT MADE AN SCHEDULED LANDING WITH ONE DISCREPANCY WHICH AIRCRAFT WAS DAMAGED BY HAIL AND A/C HAD A ENGINE SHUTDOWN DUE TO FLAMEOUT. ENGINE WAS RESTARTED WITH NO PROBLEMS INDICATING. AIRCRAFT LANDED AT IAD AND WENT TO GATE WITH NO FURTHER PROBLEMS. MAINTENANCE INSPECTED AND MAINTENANCE REMOVED AND REPLACED RADOME, OPS CHECK OK. AIRCRAFT WAS RELEASED AND APPROVED FOR RETURN TO SERVICE. (M)
