WE HAD A REPEAT OCCOURANCE OF THE BRAKE SYSTEM NOT WORKING PROPERLY. THE CREW LANDED AND THE BRAKES WORKED AS INTENDED BUT AFTER STOPPING THE BRAKES STOPPED WORKING (HYDRAULIC PUMP TURNED OFF THAT SUPPLIES PRESSURE TO THE BRAKES). THE CREW USED THE BACKUP BRAKE SYSTEM AND BROUGHT THE AIRCRAFT TO A STOP AND SIGNALED GROUND CREW TO CHOCK THE AIRCRAFT AND HOOK UP A TUG TO GET IT TO THE PARKING AREA. WE SENT A TEAM FROM SBP TO PSP WHERE THE AIRCRAFT WAS LOCATED AND IDENTIFIED A FAULTY RELAY IN THE BRAKE SYSTEM THAT TURNED THE HYDRAULIC MOTOR ON/OFF. WE BELIEVE THIS WILL BE THE FINAL FIX FOR THIS INTERMITTENT ISSUE.
ON TAXI AT KUOX OUR CREW EXPERIENCED A LOSS OF BRAKING DURING THE INITIAL BRAKE TEST. THE CREW USED THE EMERGENCY BACKUP BRAKE SYSTEM AND BROUGHT THE ARCRAFT TO AN UNEVENTFUL STOP. GROUND CREW WAS CONTACTED AND THE AIRCRAFT WAS REPOSITIONED BACK TO THE RAMP. TROUBLESHOOTING IS UNDERWAY AND WE HAVE A TECHNICIAN THAT IS VERY FAMILIAR WITH THE XLS SENT FROM OUR FACILITY TO THE AIRCRAFT TO ENSURE THIS ISSUE IS CORRECTED.
N876UC OUR CITATION 560 XLS WAS TAXIING AT KASE (ASPEN) ON 3/2/2025. WHILE PERFORMING THE BRAKE CHECKS AFTER JUST SETTING OFF FROM THEIR PARKED POSITION THE CREW RECOGNIZED THAT THE BRAKES WERE NOT WORKING PROPERLY. THIS CAUSED THE AMBER MASTER CAUTION LIGHT TO ILLUMINATE AND THE BRAKE LO PRESS/ANTI SKID INOP LIGHTS ILLUMINATED AS INTENDED. THIS WAS UNEVENTFUL AND THE AIRCRAFT HAD PLENTY OF OPEN SPACE IN FRONT OF IT. THE CREW FOLLOWED PROCEDURE AND RAN THEIR CHECKLIST. THE CREW USED THE BACKUP EMERGENCY BRAKE SYSTEM THAT ALLOWS AN ACCUMULATOR TO POWER THE BRAKES, BROUGHT THE AIRCRAFT TO A SAFE HALT AND RADIOED THE TOWER REQUESTING TO BE TOWED BACK TO THE RAMP. THE AIRCRAFT WAS REPOSITIONED TO THE RAMP AND WE REMOVED IT FROM SERVICE AND WE CURRENTLY HAVE A MAINTENANCE VENDOR TROUBLESHOOTING THE AIRCRAFT. WE INTEND TO CHANGE THE HYDRAULIC PUMP MOTOR AS WE THINK THAT WAS THE ROOT CAUSE OF THE ISSUE. PLEASE LET ME KNOW IF YOU HAVE ANY QUESTIONS REGARDING THIS EVENT AND IâLL BE HAPPY TO GET THEM ANSWERED.
DESCENDING THROUGH 10,000 MSL, THE THROTTLES WERE RETARDED TO IDLE. THE RT ENGINE IMMEDIATELY ROLLED BACK AND SHUTDOWN. LEFT THROTTLE WAS ADVANCED TO MAINTAIN AIRSPEED AND ALTITUDE. IGNITIONS WERE TURNED ON IAW THE CHECKLIST AND THE RT ENGINE STARTED BACK UP. ONCE ENGINES STABILIZED, DESCENT CONTINUED AND A NORMAL LANDING AT DESTINATION FOLLOWED. INVESTIGATION FOR THE ROOT CAUSE IS STILL ONGOING. SUSPECTING THE FUEL CONTROL UNIT AS THE FAILED COMPONENT.
THE RT FORWARD SIDE WINDOW CRACKED ON THE INNER PANE DURING A FLIGHT AT 41,000 FT CAUSING THE CREW TO MAKE AN UNSCHEDULED LANDING. SPOKE WITH ACFT MFG AND THEY CONFIRMED THAT THAT THE INNER CRACKING IS UNCOMMON. UPON INSPECTION THERE WAS NO OVER VOLTAGE DAMAGE OR OVER TORQUING OF THE FRAME SCREWS.
PILOT REPORTED A "GEAR UNLOCK" ANNUNCIATOR MOMENTARILY ILLUMINATED DURING DESCENT. A HYD PRESSURE ANNUNCIATION ALSO ILLUMINATED AND EXTINGUISHED WHEN THE "GEAR UNLOCK" INDICATOR EXTINGUISHED. THE TECH TROUBLESHOT THE ISSUE TO A LT MLG UPLOCK SWITCH. SWITCH R & R WITH A NEW SWITCH FROM MFG. PERFORMED 30 LANDING GEAR SWINGS WITH NO ABNORMAL INDICATIONS. PERFORMED LANDING GEAR FUNCTIONAL TEST IAW MM REV 15, CHAPTER 32-01-00, FUNCTIONAL TEST WAS SATISFACTORY.
ON TAKEOFF OUT OF LUK TO TSY AT 4000 FT RIGHT ENGINE STARTED SURGING FROM FULL POWER TO IDLE, PILOT REDUCED POWER TO 85 PERCENT THEN 10 SECONDS ENGINE STARTED TO SURGE FROM 85 PERCENT TO IDLE, PILOT THEN SHUT ENGINE DOWN AND RETURNED TO LUK NO PROBLEM. CAUSE FOR THIS PROBLEM NOT KNOWN AT THIS TIME 7-27-2007. ENGINE REPAIRED BY REPLACING BLEED AIR VALVES AND ENGINE WIRE HARNESS. AIDS RECORD SUBMITTED ON 8/1/2007
CABIN DOOR LIGHT CAME ON AFTER TAKEOFF. RERIGGED CABIN DOOR PINS SO CABIN DOOR LIGHT STAYS EXTINGUISHED DURING FLIGHT. NEXT FLIGHT HAD NO PROBLEMS.
PAX DOOR INTERCOSTAL TOP RADIUS CRACKED AT 1" IN LENGTH AT DATUM FS 338.
PAX DOOR FRAME TOP RADIUS CRACKED AT 1" IN LENGTH FORWARD OF DATUM 1 AT FS 338.90.
RT ENGINE SUPPORT BEAM IS CRACKED IN LOWER WEB AT .75".
RT FS 621 FLOOR SUPPORT IS CRACKED AT 1" IN ANGLE.
LBL 9 FLOOR SUPPORT HAS CORROSION AT FS 325 TO 379.
LBL 27 FLOOR SUPPORT HAS CORROSION AT FS 325.5 AND 333.5.
LT FS 621 FLOOR SUPPORT IS CRACKED AT 1" IN ANGLE.
RBL 9 FLOORBEAM HAS CORROSION AT FS 325 TO 392.
RBL 27 FLOORBEAM HAS CORROSION AT FS 337.
FS 280 BETWEEN LBL 19.0 AND RBL 12.0 HAS LEVEL 2 CORROSION.
ENGINE FLAMED OUT FOLLOWED BY AMBER ENGINE LIGHT.
ENGINE FLAME OUT FOLLOWED BY AMBER ENGINE LIGHT. ENGINE RELIGHT NORMAL. ENGINE CONTINUED OPERATION NORMAL.
EVENT SUMMARY STATED LT PACK OVERHEAT IN AUTO, ADG AUTO DEPLOY. REMOVED AND REPLACED DEPLOYMENT SQUIB. RETRACTED ADG. SYSTEM OPS CHECK GOOD.
CA'S WINDOW CRACKED. RTF. MX REMOVED AND REPLACED LT FRONT WINDSHIELD. DID PRESSURIZATION CHECK.
CREW REPORT STATED THAT WITH ONE PACK DEFERRED, RECEIVED PACK OVER PRESSURE WARNING ON T/O ROLL. ABORTED T/O. RESET AND SUCCESSFULLY TOOK OFF. NO RETURN TO GATE.
AFTER TAKEOFF CREW RECEIVED GEAR DISAGREE WITH GEAR UP. WOW INPUT FAIL AND NOSE STEERING MESSAGE. CREW DECLARED AN EMERGENCY. ADJUSTED NLG WOW SENSORS AND PERFORMED GEAR SWINGS. OPS CHECK GOOD.
DURING INSPECTION THE LEFT HAND OVERWING EMERGENCY EXIT LIGHT WAS FOUND IOPERATIVE AND REPLACED. A SATISFACTRY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RETURNED TO SERVICE.
DURING INSPECTION THE LEFT HAND FOWARD EMERGENCY LIGHT WAS FOUND INOPERATIVE. THE BULB WAS REPLACED AND A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED.
DURING MAINTENANCE INSPECTION THE 3RD EMERGENCY EXIT SIGN WAS FOUND INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE EMERGENCY EXIT SIGN BULB. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RELEASED FOR CONTINUED SERVICE
DURING MAINTENANCE INSPECTION, THE OVERWING EMERGENCY EXIT LIGHT WAS FOUND TO BE INOPERATIVE. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE LAMP IN THE EMERGENCY EXIT LIGHT. AN OPERATIONAL CHECK WAS SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
During preflight inspection, the crew reported that the forward emergency exit light was inoperative. Maintenance personnel removed and replaced the lamp in the forward emergency exit light. An operational check was satisfactory and the aircraft was returned to service.
