CATASTROPHIC FAILURE OF STARTER/GENERATOR IN FLIGHT. GENERATOR FAILURE LIGHT CAME ON IN FLIGHT AND EXTREME NOISE WAS HEARD COMING FROM ENGINE COMPARTMENT ALONG WITH ELECTRICAL BURNING SMELL IN COCKPIT AND CABIN. MECH: POST INSPECTION REVEALED A SIGNIFICANTLY DAMAGED STARTER GENERATOR. POSSIBLE FAN OR COMMUTATOR FAILURE RESULTING IN RAPID DISASSEMBLY. PART OF THE BRUSH/COMMUTATOR SHROUD WAS FOUND DENTED FROM THE INSIDE. PART OF THE BRUSH CAGE WAS FOUND RESTING ON TOP OF THE FUEL CONTROL AND A SIGNIFICANT AMOUNT OF DEBRIS WAS FOUND RESTING ON THE LOWER ENGINE COWL AFT OF THE ENGINE ACCESSORY GEARBOX. NO EVIDENCE OF FOD WAS FOUND IN THE ENGINE INTAKE AND VISUAL INSPECTION OF THE COMPRESSOR IMPELLER BLADES SHOWED NO SIGNS OF DAMAGE. REMOVAL OF THE STARTER-GEN REVEALED THE SHEAR SHAFT HAD SEVERED WHICH LIKELY EXPLAINS THE PILOT'S REPORT THAT VIBRATIONS CEASED APPROXIMATELY 4 MINUTES OUT FROM LANDING. NO SECONDARY DAMAGE HAS BEEN FOUND AT THE TIME OF THIS REPORT.
CARGO DOOR WARNING LIGHT ILLUMINATED ON DESCENT IN KMSY PASSING THROUGH 11,000 FT. THE CREW WAS INSTRUCTED TO FASTEN SEATBELTS. THE QRH PROCEDURE WAS FOLLOWED. THE DOOR WAS FLUSH, SECURE AND PRESSURIZATION WAS NOT LOST. AN INFLIGHT EMERGENCY WAS DECLARED AS A PRECAUTION. THE WARNING LIGHT WENT OUT AS WE DESCENDED THOUGH 6000 FEET. THE AIRCRAFT WAS LANDED WITHOUT INCIDENT. POSTFLIGHT INSPECTION OF THE CARGO DOOR FOUND IT TO BE SECURE. - MECHANIC - PERFORMED THE DOOR WARNING ADJUSTMENT / TEST PER PILATUS PC-12 AMM 12-A-52-70-00-00A-903A-A, FOUND SWITCH SPRING ROLLER AND LOCKING LEVER WERE WITHIN ADJUSTMENT AND MAKING CONTACT. GAPS WERE IN THE PROPER RANGE WHEN LEVER WAS ACTUATED TO THE POINT OF THE LIGHT COMING ON ON THE CAWS, DID FIND A GAP BETWEEN THE DOOR STRUCTURE AND THE LOCKING LEVER ADJUSTMENT SCREW (OUT OF ADJUSTMENT). ADJUSTED THE LOCKING LEVER ADJUSTMENT SCREW TO THE PROPER POSITION TO THE DOOR STRUCTURE AND RETESTED. NO DEFECTS FOUND.
DISCREPANCY: SMOKE IN CABIN. ARRIVED AT AIRCRAFT, MEDICAL STAFF INFORMED ME THE SMOKE HAD COME OUT OF PORTABLE POWER STRIP/ SURGE PROTECTOR 8 SOCKET 115VAC OUTLET EXTENDER. DURING FLIGHT PILOT TURNED OFF POWER TO LIFEPORT BED AND SMOKE STOPPED COMING OUT OF UNIT. I REMOVED THE UNIT FROM THE AIRCRAFT AND NOTICED IT WAS VERY WET AS IF SOMETHING WAS SPILLED INTO THE UNIT AND IT SMELLED OF ELECTRICAL BURN. I WENT TO WALMART AND BOUGHT ANOTHER OUTLET STRIP AND GAVE TO MEDICAL STAFF. NO FURTHER ACTION TAKEN.
