DURING TAKEOFF AND CLIMB, CREW NOTICED A FUEL SMELL AS WELL AS FUEL ON THE CLAMSHELL DOOR WINDOWS AND TAILBOOM SECTION. AIRCRAFT RETURNED TO THE AIRPORT FOR FURTHER INSPECTION. SUPPLEMENTAL INFORMATION RESEATED FUEL CAP, UNABLE TO DUPLICATE THE ISSUE.
DURING TAKEOFF AND CLIMB, CREW NOTICED A FUEL SMELL AS WELL AS FUEL ON THE CLAMSHELL DOOR WINDOWS AND TAIL BOOM SECTION. AIRCRAFT RETURNED TO THE AIRPORT FOR FURTHER INSPECTION.
DURING CRUISE FLIGHT, THE NR 2 ENGINE CHIP LIGHT ILLUMINATED ALONG WITH MASTER CAUTION LIGHT. ATTEMPTED TO BURN CHIP IAW RFM AND CHIP DID NOT BURN. ABORTED FLIGHT, DECLARED AN EMERGENCY. PLACED THE NR 2 ENGINE TO GROUND POSITION. NO OTHER ABNORMAL INDICATIONS FROM THE NR 2 ENGINE. COMPLETED A SHALLOW APPROACH TO A RUNNING LANDING. LANDED SAFELY WITHOUT INCIDENT, IMMEDIATELY SHUT DOWN THE NR 2 ENGINE AND COMPLETED A NORMAL SHUTDOWN OF THE NR 1 ENGINE.
DURING RUN UP OF AIRCRAFT FOR A FLIGHT, TAIL ROTOR CHIP LIGHT ILLUMINATED. PILOT ABORTED TAKEOFF AND CALLED MECHANIC. MECHANICS INSPECTION OF TAIL ROTOR CHIP DETECTOR REVEALED A 3MM METAL CHIP IDENTIFIED AS CATEGORY E INDICATING A SIGNIFICANT FAILURE.
AFTER TAKEOFF, NURSE IN THE BACK THOUGHT SHE SMELLED FUEL FUMES. LANDED AT NEAREST AVAILABLE SITE, AIRPORT, WITHOUT FURTHER INCIDENT..
WHILE COMPLETING TASK 26A0337-35, 12 MONTH TAIL CONE, WORN SKIN REPAIR, FOUND A CRACK ON THE FWD ECU MOUNT'S FWD SUPPORT. THE CRACK STARTS AT THE TOP OF THE SUPPORT & TERMINATES AT THE LOWER RIVET.
DURING A SCHEDULED INSPECTION OF THE COMPRESSOR BLADES, AN INLET HOUSING STRUT ON THE INTAKE WAS FOUND TO HAVE A SMALL CHUNK OF METAL MISSING FROM THE TOP & BOTTOM OF THE STRUT. THIS CHUNK WAS FOUND MISSING IN THE RADIUS OF THE STRUT. THIS ENG WAS REMOVED & SENT OUT FOR REPAIR, A SERVICEABLE ENG WAS RECEIVED & INSTALLED. THIS ACFT HAS SINCE RETURNED TO SERVICE AT THE TIME OF THIS SUBMISSION.
