D/L CODES FROM PSEU AND A HARD FAIL WAS DISCOVERED ON LT MLG CODES WAS LEFT GEAR UPLOCK FAR. DID NOT HAVE A CHANCE TO DO GEAR SWINGS BUT BELIEVED GEAR WAS SLOW TO COME UP. FOUND NR 3 ACCUMULATOR LOW, SERVICED NR 3 ACCUMULATOR IAW CRJ AMM 12-12-29 D/L LOADED CODES FROM PSEU AND RESET PSEU. GEAR SWINGS OP CHECK GOOD. MLG SELECTOR VALVE, NO LEAK NOTED.
CREW REPORT ON 01/11 STATED THAT IMMEDIATELY AFTER T/O THE STAB TRIM AND MACH TRIM CAUTION MSGS POSTED AT THE SAME TIME. RETRIEVED FAULT CODES FROM MDC. PERFORMED HSTCU OPS TEST PER 27-40-00. OPS CHECK GOOD.
FLAPS FAIL AT ZERO. RETRIEVED FAULT CODES 1 JAM 2. PDU DIFF 3 UNSPECIFIED, CLEARED FAULTS OPS CHECKS GOOD AT THIS TIME REF 27-51-00.
LAV SMOKE INDICATION AT 200FT. NO SMOKE OR FUMES BUT ODOR. SUSPECT GLYCOL IN AIR CONDITIONING. MX PERFORMED HIGH POWER RUNS. NO DEFECT NOTED.
EVENT SUMMARY STATED REJECTED TAKEOFF AT 40KT DUE TO LT COWL ANTI-ICE CAUTION MSG. TROUBLESHOOTING CONFIRMED THE PROBLEM, AND RTG. REMOVED AND REPLACED LT COWL ANTI ICE VALVE REF AMM 30-22-01. PERFORMED OPS CHECK OF ANTI ICE. OPS CHECK GOOD.
DURING INSPECTION THE FOWARD FLOOR EMERGENCY FLOOD LIGHT WAS FOUND INOPERATIVE. THE BULB WAS REPLACED AND A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED.
DURING FLAP EXTENSION, THE FLAPS WOULD NOT EXTEND BEYOND 20 DEGREES. THE MALFUNCTION DID NOT INTERFERE WITH THE SAFE OPERATION OF THE FLIGHT. THE FLIGHT WAS COMPLETED NORMALLY AND PASSENGERS DEPLANED NORMALLY. THE FLAP CONTROL LEVER WAS REPLACED. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RETURNED TO SERVICE.
DURING APPROACH OF FLIGHT 7522 CREW REPORTED A FLAP SYSTEM FAILURE WITH FLAPS A 8 DEGREES. FLIGHT LANDED UNEVENTFULLY. A SYSTEM OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT WAS RETURNED TO SERVICE.
DURING INSPECTION THE LEFT FORWARD EMERGENCY LIGHT WAS FOUND INOPERATIVE. THE BULB WAS FOUND FAULTY AND REPLACED. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT RETURNED TO SERVICE.
DURING CLIMB OF FLIGHT 7267 CREW REPORTED STABILIZER TRIM SYSTEM 1 AND 2 FAILURE MESSAGES. CREW DECLARED EMERGENCY AND RETURNED TO ORD. FLIGHT LANDED WITHOUT INCIDENT AND PASSENGERS DEPLANED NORMALLY. THE DISCONNECT SWITCHES WERE FOUND ENGAGED DUE TO FOD AND WERE CLEANED. A SATISFACTORY OPERATIONAL CHECK WAS PERFORMED AND AIRCRAFT RETURNED TO SERVICE.
DURING CLIMB OF THE FLIGHT FROM IAD TO GSP, THE CREW REPORTED THAT UPON LANDING GEAR RETRACTION HAD NO INDICATION OF GEAR RETRACTION ON THE FLIGHT DECK. THE THREE GREEN GEAR DOWN/LOCKED INDICATORS REMAINED ILLUMINATED. THE CREW REPORTED, RECEIVED ANTI-SKID OUTBOARD AND PARKING BRAKE SOV FAIL MESSAGES. MAINTENANCE PERSONNEL REMOVED AND REPLACED THE LANDING GEAR SELECTOR. LANDING GEAR EXTENSION/RETRACTION OPERATIONAL CHECKS WERE SATISFACTORY AND THE AIRCRAFT WAS RETURNED TO SERVICE.
