AFTER PASSING THROUGH FL 360, APU QUIT WITH FAULT LIGHT AND WOULD NOT RESTART SINGLE GENERATOR. OPS ON RETURN TO ATL DUE TO LEFT ENG GENERATOR INOP ON MCO AND PLACARD. SERVICED APU OIL TO FULL. ECU BITE SHOWED OVERSPEED FOR AUTO SHUTDOWN. T/S PER FIM 49-11-00 FIG 111. REMOVED AND REPLACED FCU PER M/M. APU CHECKS GOOD.
THE BS 365 FLOORBEAM WAS CORRODED ON THE UPPER CHORD AT RBL 33. REPAIR ANGLES WERE INSTALLED.
DURING HMV AT YVR, THE STA 365 PASSENGER FLOORBEAM WAS FOUND CORRODED AT RBL 33.
DURING HMV AT YVR, A PASSENGER FLOOR DECK PLATE AT STA 1684, RBL 60 WAS FOUND GOUGED.
THE UPPER SURFACE OF THE DECK PLATE IS GOUGED AT BS 1684, RBL 60. THE GOUGE WAS BLENDED SMOOTH AND A REPAIR DOUBLER INSTALLED.
DURING HMV AT YVR, A CRACK WAS FOUND IN THE NR 2 PYLON O/B SKIN AT STA 133.5.
DURING HMV AT YVR, AN EXISTING REPAIR WITH INSUFFICIENT FASTENER EDGE DISTANCE AND SPACING WAS FOUND AND REPLACED WITH A PERMANENT REPAIR.
THE NR 2 ENGINE PYLON BULKHEAD FITTING AT NAC STA 116.05 WAS FOUND WITH EXISTING REPAIRS THAT REQUIRED REPLACEMENT. EXISTING REPAIRS REMOVED AND A LARGER REPAIR FITTING WAS INSTALLED.
AT FLT LVL 270 FROM ATL TO BWI FLT ATTN REPORTED WHITE/GRAYISH SMOKE COMING FROM THE AFT GALLEY ABOVE THE CEILING PANEL. REMOVED GALLEY POWER, SMOKE STILL EVIDENT. DECLARED EMERGENCY INTO CLT. DISCOVERED THE AFT GALLEY LEFT COFFEE MAKER HAS BURNED HEATING ELEMENT AND WAS SOURCE OF SMOKE. REPLACED COFFEE MAKER.
AT GATE IN SJC OPERATIONS BLEW L4 DOOR SLIDE DURING NORMAL A/C SERVICE ON GND. THE SLIDE OPERATED NORMAL DUE TO IT BEING ARMED WHEN OPERATIONS OPENED DOOR..THIS OCCURED ON THE ORIGINATING LEG OF FLT 1432 10FEB IN SJC.. C/A MTC. REPL SLIDE PACK AND DOOR ASSIST BOTTLE IN SJC..
FUSELAGE / SKIN / FS 520 / STRG. 28L : GROUND DAMAGE CUTOUT REPAIR THE FUSELAGE SKIN WAS FOUND DENTED AT TWO LOCATIONS BETWEEN STRINGERS 27L & 28L AT FS 518 AND FS 532. THE DAMAGED AREAS WERE REMOVED WITH 2" CUTOUTS IN THE SKIN. THE TWO CUTOUTS WERE REPAIRED WITH A SINGLE EXTERNAL ALUMINUM DOUBLER PER ER/A 382221-14, REV A.
SHORTLY AFTER TURNING ON WING A/I DUCT LEAK ILLUMINATED ALONG WITH RT ENG ANTI ICE. A/C DIVERTED INTO PHX. FOUND CLAMP DPN 366201022 HAD FAILED IN THE RT ENG/PYLON FWD SECTION, 16 SMALL RIVITS HAD FAILED ON THE CLAMP ALLOWING THE DUCT TO SEPARATE.
ON WALKAROUND IN LAX FOUND NR 1 ENGINE BLADES NICKED. NR 1 ENGINE BLENDED 3 BLADES MARKED PER MM 72-31-02-06 FIGURE 605.
R4 DOOR SKIN WAS FOUND GOUGED AT THE LWR HINGE CUTOUT. REPAIRED PER ERA 331237-14.
FOUND LEFT TRAILING EDGE OUTBOARD FLAP DAMAGE DUE TO BIRD STRIKE. REPLACED FLAP.
JUST BEFORE A/C REACHED CRUIS NO. 2 ENG LOST OIL PRESS. THE ENG PLACED AT IDLE . THE ENG SHUT ITSELF OFF BEFORE THE PILOT COULD TURN OFF THE FUEL CONTROL. FOUND METAL IN OIL SCREEN. FOUND THE ENG LOCKED UP.
NR 1 ENG TURBINE SECTION DAMAGED.
EXTERIOR SKIN HAD TWO SMALL HOLES CAUSED BY HAIL IN DFW. BOTH HOLES WHERE LESS THAN 2 INCHES IN DIAMETER. DAMAGE WAS REPAIRED IN ACCORDANCED WITH SRM 57-70-01 WITH REF TO 51-70-17. FOUND DURING SHOP VISIT FOR NR 8 SPOILER PANEL.
TWO AREAS OF DELAMINATION; ONE AREA IS 1 ACROSS AND 44 FROM THE OUTBOARD END, THE OTHER IS A .5 DELAMINATION 15 FROM THE INBD END. ACCOMPLISHED REPAIRS PER SRM 57-60-01. FOUND DURING SHOP VISIT FOR LEFT WING AILERON.
AT LETTER CHECK, FOUND SPOILER PANEL WITH PUNCTURE AT 53 INCHES FROM LEFT EDGE AND 13 INCHES FROM AFT EDGE OF PANEL AND TWO CRACKS IN LOWER SURFACE SKIN AT 14 INCHES FROM IB END. REPAIRED ALL DAMAGE PER SRM 57-70-01 WITH REF TO SRM 51-70-17.
DURING LETTER CHECK, COMPLETED SCHEDULED PERMANENT REPAIR FOR CRACKS IN NR 1 AFT FLAP. REPAIRED PER SRM 57-53-01 AND SRM 51-70-17.
DURING LETTER CHECK FROM SCHEDULED MTC, INSTALLED PERMANENT REPAIR FOR DENT ON NR 9 L/E SLAT, PER SRM 57-43-02.
PUNCTURE IN THE UPPER SKIN OF SPOILER, APPROX 25.5 FROM O/B EDGE, 4 INCHES FROM TRAILING EDGE. REPAIR CONSISTED OF BONDING 3 REPAIR PLIES USING GRAPHITE CLOTH REPAIR MATERIAL AND 9390 ADHESIVE RESKIN SYSTEM, CURED AT 200 DEGREES FOR A TOTAL OF FOUR HOURS AS CALLED OUT IN THE 57-70-01 REF 51-70-17.
LT I/B AFT FLAP SUFFERED HAIL DAMAGE ON UPPER SURFACE. FLAP WAS REPAIRED PER SRM 57-53-01.
