IT IS SURMISED THAT AN UNSTEADY N2 SIGNAL TO THE ENGINE FADEC MAY ACCOUNT FOR THE INABILITY OF THE ENGINE TO ACCELERATE.

NR 2 ENGINE FLAMEOUT. FLAME OUT WAS CAUSED BY FUEL LEAK AT THE AIR/OIL COOLER MUSCLE LINE. THE FUEL MUSCLE LINE WAS REPLACED AND THE ENGINE WAS REMOVED WITH IN 25 CYCLES DUE TO A SURGE ABOVE 1.25 EPR CAUSED BY THE FLAME OUT. THE MUSCLE LINE FAILED DUE TO VIBRATION CAUSED BY THE ANGLE GEARBOX. THE ANGLE GEARBOX HAD THREE BEARING RETAINING SCREWS LOOSE AND ONE HAD FALLEN OUT. ROOT CAUSE OF THE FAILURE APPEARS TO BE INSUFFICIENT RETENTION OF THE AB4 BEARING RETAINING SCREWS. THE ADDED ADDITION OF LOCKING LIQUID WILL BE USED ON FUTURE BUILDS IN THE ANGLE GEARBOX.

THIS CAUSED THE OVER-RETRACTION. LINK WAS POSSIBLY LEFT OFF DURING BRAKE ROD INSPECTION.

EMERGENCY EXIT LIGHTING (EEL) STRIP WAS INOPERABLE FROM SEAT 11G THROUGH RIGHT SIDE OF THE MID GALLEY. REPAIRED ASSEMBLY PER MM 2-0-15-5. OPERATIONS CHECK NORMAL.
THE RIGHT PACK WAS NOISY AND CAUSING A VIBRATION IN THE FORWARD CABIN DURING THE FLIGHT. THE AIRCRAFT DIVERTED TO JFK. *S/D* THE NOISE AND VIBRATION WAS CAUSED BY A LOOSE FLAPPER IN THE SUPPLY FAN CHECK VALVE. THE CHECK VALVE WAS REPLACED PER MM 21-58-02, OPERATIONS CHECK NORMAL.
AIRCRAFT RETURNED TO THE FIELD AT SFO UNDER AMBER ALERT DUE TO A FIRE BELL AURAL WARNING. *S/D* THE ENGINE FIRE DETECTION SYSTEM CAUSED THE AURAL WARNING. A BITE TEST WAS PERFORMED AND INDICATED NO FAULTS. THE M1000 BELL CHIME MODULE WAS REPLACED AS A PRECAUTION. THIS MODULE WAS DETERMINED TO BE SERVICEABLE AND WAS RETURNED TO SERVICE. NO OTHER FAULTS HAVE BEEN INDICATED SINCE THIS INCIDENT.
