THE DIRECT CAUSE OF THEN ANOMALY IS UNKNOWN; HOWEVER, IT HAS NOT RECURRED SINCE REPLACEMENT OF COMPONENTS.
FLIGHT RETURNED TO FIELD WITH CABIN PRESSURE FAULTS. THE EQUIPMENT COOLING OVERBOARD EXHAUST VALVE FAILED. THE VALVE WAS NOT DEACTIVATED PER THE MEL CAUSING A LOSS OF CABIN PRESSURIZATION ON THE SUBSEQUENT FLIGHT. THE VALVE WAS RE-DEFERRED PER THE MEL AND REPLACED AT THE NEXT MAINTENANCE OPPORTUNITY WITH NO FURTHER PROBLEMS. LINE QA IS CONDUCTING A INVESTIGATION INTO THE CIRCUMSTANCES.

NR 1 ENGINE SHUTDOWN DUE TO LOSS OF OIL PRESSURE AND HIGH TEMP. OIL PUMP DRIVE GEAR FAILURE. OIL PUMP DRIVE BEARING AND GEAR ASSY WERE FOUND LOOSE AND COCKED IN THE GEARBOX BEARING BORE. BEARING RETAINING SCREWS (ITEM 10 ) WERE BACKED OUT OF THE GEARBOX HOUSING AND LAYING IN THE BOTTOM OF THE GEARBOX. THIS RESULTED IN FRACTURE OF THE OIL PUMP DRIVE MODULE GEAR SHAFT IN THE WEB OF THE GEAR. THE FINAL ROOT CAUSE IS STILL BE INVESTIGATED. ENGINE WILL REQUIRE OVERHAUL OF ALL BEARINGS DUE TO LOSS OF OIL PRESSURE. MAIN GEARBOX WILL BE OVERHAULED.

RIGHT HAND MID-GALLEY OVEN FIRE. EXTINGUISHED FIRE WITH HALON EXTINGUISHER. ISOLATED ELECTRICAL SUPPLY BY TURNING-OFF LEFT UTILITY BUS. *S/D* AIRCRAFT CONNECTORS AND WIRING WERE INSPECTED AND FOUND TO BE SERVICEABLE. A NEW OVEN WAS INSTALLED AND OP CHECKED GOOD. THE REMOVED OVEN WAS INSPECTED BY THE HOME SHOP IN SFO AND FOUND TO HAVE PLASTIC DEBRIS MELTED ON AND ABOVE THE BAFFLE AND ON THE HEATING ELEMENT IN THE UPPER AREA OF THE OVEN. IT IS SUSPECTED THAT A PLASTIC BAG LEFT ON THE OVEN INSERT WAS DRAWN INTO THE OVEN HEATING AREA. OVEN INSERT PACKAGING, TRANSPORT AND LOADING PROCEDURES HAVE BEEN REVIEWED WITH UA CATERERS PER A SAFETY ALERT METER DTD 12/28/98.
INITIAL LANDING GEAR EXTENSION RENDERED AMBER TAIL SKID AND GEAR LIGHTS AND EICAS GEAR DISAGREE MESSAGE. DID GO-AROUND, RECYCLED GEAR. SUBSEQUENT GEAR EXTENSION NORMAL.
TRAILING EDGE LIGHT ON DURING APPROACH AT POSITION 20. EXECUTED MISSED APPROACH. NEXT APPROACH FLAPS WORKED NORMALLY. *S/D* AN INTERMITTENT INTERNAL FAILURE IN THE TRAILING EDGE FLAP POWER DRIVE UNIT HYDRAULIC CONTROL VALVE MODULE ACTIVATED THE T/E FLAP FAILURE PROTECTION SYSTEM WHICH INHIBITS FLAP MOVEMENT. REPOSITIONING THE FLAP CONTROL LEVERS TO THE FULL RETRACT POSITION UNLATCHED THE FAILURE PROTECTION SYSTEM AND RESTORED FULL FLAP OPERATION. THE SUBJECT T/E FLAP PDU CONTROL VALVE WAS REMOVED, REPLACED, AND OPERATIONALLY CHECKED PER MM 27-51-32. ALL SYSTEMS CHECKED NORMAL.
SECTION OF EMERGENCY TRACK LIGHTS FROM 6AB TO 13AB REMAIN ON WITH SWITCH ARMED AND OFF POSITIONS. REPLACED POWER SUPPLY. OPS CHECK OKAY.
ABORTED TAKEOFF AT LHR AT 90 KNOTS DUE TO RT ENGINE BLEED OFF LIGHT ILLUMINATED AND EICAS MESSAGE RIGHT ENGINE BLEED OFF. *S/D* REPLACED RH PRSOV 02 JULY. SHOP FOUND INLET FILTER COMPLETELY CLOGGED, PREVENTING VALVE FROM OPENING.
TAKEOFF ABORTED APPROXIMATELY 90 KNOTS DUE TO NO CAPTAIN'S AIR SPEED INDICATION. *S/D* FOUND LOOSE PITOT ON LEFT ADC. SECURED PITOT LINE, AIRSPEED OPS NORMAL.
RIGHT ENGINE BLEED OFF' EICAS AND BLEED OFF WARNING LIGHT DISPLAYED DURING CLIMBOUT. *S/D* MAINTENANCE FOUND WIRE CHAFED THROUGH AT PRESSURE REGULATING VALVE, CONTACTING ADJACENT METAL TUBING. REPAIRED WIRING AND SECURED.
DIVERTED TO MIA UNDER AMBER ALERT DUE TO GENERATOR INDICATION PROBLEM. *S/D* THE LEFT ENGINE INTEGRATED DRIVE GENERATOR WAS BELIEVED TO BE AT FAULT. THE LEFT ENGINE IDG WAS REPLACED, OPERATIONS CHECK NORMAL. THE SHOP FOUND NOTHING WRONG WITH THE REMOVED IDG.
DURING CLIMB-OUT, THE RIGHT ENGINE EXPERIENCED HIGH EGT AND TWO COMPRESSOR STALLS. THE CREW INITIATED AN IN-FLIGHT SHUT DOWN. AIRCRAFT RETURNED TO MIAMI WITH AN UNEVENTFUL LANDING. THE HIGH PRESSURE COMPRESSOR FIFTH STAGE BLADE FAILED. THE ENGINE WAS REPLACED. PART TOTAL CYCLES, 2,473.
ABORTED TAKEOFF AT LESS THAN 60 KNOTS DUE TO NR 1 ENGINE VIBRATION. THE HIGH PRESSURE COMPRESSOR EXPERIENCED A FIFTH STAGE BLADE FAILURE. THE ENGINE WAS REPLACED. PART TOTAL CYCLES, 2,471.
LEFT EXTERIOR EMERGENCY LIGHTS WON'T TEST. REPLACED LAMPS.
