AS A RESULT OF SHOP FINDINGS, THE MOST LIKELY CAUSE OF THIS EVENT IS A LOOSE CONNECTOR AT THE SWITCH (FIXED DURING SWITCH REPLACEMENT).

DURING ORD-IAD LEG, FLIGHT CREW REPORTED FAILURE OF BOTH AUTO CABIN PRESSURE CONTROLLERS. REPLACED NR 2 AC MOTOR. 3/29/01: DF534 WAS CREATED FOR AUTO PRESS SYSTEM NR 2. BITE ON BOTH NR 1 & 2 CPC SHOWED NR 2 AC MOTOR FAULT. 4/5/01: REPLACED NR 2 AC MOTOR (MR21324-142 OFF, -80 ON). ENGINEERING CALLOUT (EG3456) ACCOMPLISHED ON 4/18/01, PERFORMED WIRE CHECKS BETWEEN AC MOTORS AND CABIN PRESS CONTROLLERS. NO DISCREPANCIES WERE FOUND.
FLAP INDICATION EICAS. DIVERTED TO MIA. *S/D* AN IMPROPERLY INSTALLED NOSE LANDING GEAR CENTERING CAM DID NOT ALLOW PROPER NOSE GEAR COMPRESSION, CAUSING CONFIGURATION WARNING LIGHTS TO APPEAR ON EICAS. REPLACED THE NOSE LANDING GEAR CENTERING CAM WITH NEW CAM. NOTE THAT THERE WAS NO PART DEFECT. CAM WAS IMPROPERLY INSTALLED.
AFTER TAKEOFF FLIGHT EXPERIENCED A CONFIGURATION SYSTEM WARNING HORN. WOULD NOT RESET. AIRCRAFT RETURNED TO JFK. ON FIRST APPROACH WITH FLAPS AT 30 THE FLAPS MOVED TOWARD UP, UNCOMMANDED. *S/D* NOSE GEAR STRUT WAS SERVICED WITH NEW SEALS IN JFK. CENTERING CAM WAS INADVERTENTLY INSTALLED 180 OUT WHICH PREVENTED NOSE GEAR AIR/GROUND SIGNAL TO READ AIR MODE ON SUBSEQUENT FLIGHT. CENTERING CAM WAS REPOSITIONED IN MIAMI.
NR 2 ENGINE REPLACEMENT REQUIRED DUE TO OIL SMELL IN THE CABIN. LAV SMOKE DETECTOR ACTIVATED. RTF, OVERWEIGHT LANDING. ACCOMPLISHED OIL SYSTEM VACUUM AND PRESSURE CHECKS AND EXTENSIVE TEST CELL RUNS WITH NOTHING FOUND. ENGINE WAS RETURNED TO SERVICE AND HAS OPERATED OVER 2500 HOURS ON ANOTHER AIRCRAFT WITH NO REPORTS OF OIL ODOR OR SMOKE IN THE CABIN.
AIR-GND DISAGREE, NOSE AIR GND DISAGREE AND LDG GEAR MONITOR EICAS MSG''S DISPLAYED AFTER TAKEOFF. ALSO HAD AUTO THROTTLE DISCONNECT, TAIL SKID MSG, PROBE HEAT. ALL EICAS MSG''S CLEARED EXCEPT AIR-GND DISAGREE, NOSE AIR GND DISAGREE AND LDG GEAR MONITOR. DIVERTED TO ORD. *S/D* REPLACED PROXIMITY SWITCH ELECTRONIC UNIT.
RTF WITH MAIN BATTERY EICAS MESSAGE. *S/D* REPLACED MAIN BATTERY, CHECK NORMAL.
TAKEOFF ABORTED AT 110 KNOTS. *S/D* E AND E COMPARTMENT DOOR WARNING ILLUMINATED. REPLACED AND ADJUSTED E AND E DOOR CLOSED SENSOR S201 PER MM, GROUND OPS CHECKS OK.
EMERGENCY EXIT FLOOR LIGHTING IN RIGHT SIDE OF CABIN ROWS 21 TO 29 COMES ON INTERMITTENTLY. *S/D* THE CAUSE OF THE INTERMITTENT EEL WAS FOUND TO BE A BAD BATTERY ASSEMBLY. UAL CORRECTED THE PROBLEM BY REPLACING THE BATTERY ASSEMBLY WITH AN OVERHAULED, RECHARGED UNIT. NO PROBLEMS HAVE OCCURED SINCE THE REPLACEMENT, SYSTEM OPERATES NORMALLY.
RIGHT ENGINE NR 1 FIRE LOOP STATUS MESSAGE DISPLAYS CONTINUOUSLY. *S/D* PROBLEM CAUSED BY A FAULTY AUTOMATIC FAULT AND LOGIC TEST CARD (AFOLTS). REPLACED CARD, SYSTEM OPS CHECKS GOOD.
ABORTED TAKEOFF DUE TO LEFT ENGINE OVERTEMP. *S/D* THE 2.95 BLEED SOLENOID VALVES REMAINED OPEN AT HIGH POWER DUE TO INTERNAL CORROSION IN THE 2.95 BLEED VALVES. THIS LED TO THE LEFT ENGINE OVER TEMP CONDITION. THESE VALVES ARE CURRENTLY UNDERGOING AN UPGRADE TO NON-CORROSIVE MATERIAL.
