ELECTRICAL SMELL NOTED DURING GROUND OPERATION FROM FLIGHT INSPECTION SYSTEM. #2 FLIGHT INSPECTION VOR/ILS RECEIVER FAILED AT SAME TIME ODOR WAS DETECTED.
AIRCRAFT FAILED TO PRESSURIZE DURING CLIMBOUT
A CRACK WAS FOUND IN THE RADIUS OF A LEFT WING ROOT RIB ATTACH CHANNEL. REPAIRED IAW EO 18-28-40, MAJOR REPAIR.
BRAKES WEAK. REMOVED AND REPLACED ALL FOUR BRAKE ASSEMBLIES.
LEFT PROPELLER PITCH LIGHT ILLUMINATED INTERMITTENTLY DURING FLIGHT,
DURING APPROACH TO FULL-STOP LANDING, ATTEMPTED TO ADVANCE PROP LEVERS FROM 1500 RPM SETTING TO 1700 RMP. PROP LEVERS WOULD NOT ADVANCE FURTHER THAN 1540 RPM. BOTH PROP LEVERS WERE EXERCISED AND BINDING RELEASED.
SEVERE CORROSION ON LOWER FUSELAGE SKIN & ANTENNA DOUBLER AT APPROXIMATELY STATION 265.75.
DURING D-CHECK DISCOVERED RUDDER CRACKED AT LOWER HINGE, LT ATTACH BOLT HOLE.
PILOT NOT FLYING, NOTICED ENLARGED AREA OF DELAMINATION IN LOWER LEFT CORNER OF RIGHT WINDSHIELD. FLIGHT WAS ABOUT 50 NM NORTHWEST OF AIRPORT AT FL 230 WHEN THE PROBLEM WAS NOTICED. FLIGHT DIVERTED BACK TO AIRPORT UNEVENTFULLY.
APPROXIMATELY 2 TO 2 1/2 MINUTES AFTER TAKEOFF, NOTICED THE LEFT TORQUE GAUGE GOES TO APPROX 10 PERCENT OR LESS, FOLLOWED BY THE LEFT NO FUEL TRANSFER LIGHT ILLUMINATING, FOLLOWED BY THE AIRCRAFT YAWING TO THE LEFT AND THEN THE OTHER ENGINE GAUGES REFLECTING THE LEFT ENGINE FAILED. NO FIRE LIGHT WAS ILLUMINATED. UNEVENTFUL SINGLE ENGINE LANDING ACCOMPLISHED. REPLACED THE LEFT ENGINE PY LINE GOING TO THE PROP GOVERNOR WITH SERVICEABLE ASSEMBLY P/N 3032125. GROUND RUN OPS CHECKED GOOD.
WHILE CONDUCTING CLEARANCE BELOW PATH RUN, A PAIR OF PIGEONS DIVED AND STRUCK LEFT ENGINE AREA. HEARD A THUMP AND ALL CREW MEMBERS STARTED NOTICING A BURNING SMELL. NO ABNORMAL ENGINE INDICATIONS WERE OBSERVED. CREW REQUESTED IMMEDIATE LANDING AND WAS GIVEN A LANDING CLEARANCE. AIRCRAFT LANDED WITHOUT INCIDENT, TAXIED CLEAR OF RUNWAY AND SHUTDOWN. FIRE DEPARTMENT CHECKED AC AND RELEASED IT TO BE TOWED. PIGEON REMAINS WERE FOUND ON THE LEFT ENGINE INTAKE, INSIDE THE ENGINE IN THE SCREEN, ON THE LEFT WING LEADING EDGE, AND ON THE RT SIDE OF THE RUDDER.
ON 12/31/2002, REPLACED LANDING GEAR CONTROL SWITCH. SWITCH S/N 1669A, WAS INSTALLED AND SYSTEM CYCLED THREE TIMES WITH NO APPARENT PROBLEM. ON 1/09/2003, AIRCRAFT LEFT FOR MAINTENANCE TEST FLIGHT BECAUSE RIGHT ELEVATOR HAD BEEN REMOVED AND REINSTALLED AFTER BALANCE CHECK AFTER HARDWARE REPLACEMENT. ON TAKEOFF RELEASE LEVER FAILED TO RETRACT SO GEAR HANDLE COULD BE RAISED NORMALLY. WHEN CHECKED ON JACKS WITH SQUAT SWITCH DISCONNECTED RELEASE STILL DID NOT OPERATED. REMOVED FROM AIRCRAFT AND TRIED AGAIN ANS WITH JUMPER WIRES UNIT STILL FAILED TO RELEASE SO LEVER COULD BE RAISED. THIS CONTROL SWITCH FAILED ON THE FOURTH TIME IT WAS OPERATED SINCE LAST REPAIR. SWITCH WAS REPLACED AND PROBLEM CORRECTED. (M)
