(NR 1 ENG HOC): OIL LEAKING FROM FUEL PUMP DRAIN 4 DPM WITH ENG STATIC.
APU OIL CONSUMPTION IS EXCESSIVE. [SUP 4-20-07] APU WAS REMOVED FROM AIRCRAFT FOR HIGH OIL CONSUMPTION. OIL CONSUMPTION WAS INVESTIGATED AND CONFIRMED IN THE TEST CELL. THE LRU OIL PUMP WAS REPLACED AND ALL HOC WAS GONE. WITH PROPER TROUBLESHOOTING, THIS APU COULD HAVE BEEN REPAIRED WITH THE APU STILL IN THE AIRCRAFT. THE REMOVAL OF THIS APU WAS UNNECESSARY. THE OIL PUMP IS A LINE REPLACEABLE UNIT.
ALL AIRCRAFT EMERGENCY LIGHTS COME ON WHEN POWER REMOVED FROM AIRCRAFT. FOUND EMERGENCY LIGHT SWITCH S1 DEFECTIVE. REPLACED SWITCH. OP CHECKS NORMALLY.
FIRE-OVERHEAT TEST DID NOT INITIATE WHEN -ENG APU CARGO- TEST SWITCH PUSHED. RETESTED ON GROUND PER AMM 26-15-00, P 501. UNABLE TO DUPLICATE. NO PROBLEM FOUND. CHECKED OK
FOUND FWD GALLEY CROSS AISLE EEL LIGHT INOP. REPLACED EEL STRIP, OPS CK OK.
RETURNED TO FIELD DUE TO LEFT BLEED OVERHEAT INDICATION. THE WING DUCT LEAK DETECTORS TS211 & TS212 WERE REPLACED. DURING TROUBLESHOOTING THE SENSOR THAT WAS CAUSING THE FAULT COULD NOT BE DETERMINED SO BOTH WERE REPLACED. THE REMOVED PARTS ARE NON-STOCK, NO FURTHER ANALYSIS. AS OF 09FEB06 THIS WAS THE ONLY ABNORMALITY REPORTED IN AMIS FOR CHAPTER 26. AN 8-IN CRACK WAS FOUND IN THE DUCT CAUSING BLEED AIR LEAKAGE AND "BODY DUCT LEAK" EICAS MESSAGE TO APPEAR. THE DUCT WAS REPLACED.
FUSELAGE DENTED AND GOUGED FWD OF AFT CARGO PIT CONTROL PANEL DOOR. FUSELAGE SKIN REPAIRED PER SRM. SUPPLEMENTAL INSPECTIONS REQUIRED BY 37,500 CYCLES ARE MONITORED BY AMIS CF9010.
LT O/W COMPT INTEGRATOR TORQUE CHECK FAILED AND LATCHES MUST BE REPLACED. REMOVE AND REPLACE LATCHES IAW UAL B767 MM-25-65-98.
CABIN FLOORBEAM WEB CRACKED STATION 1439, LBL 25. MAJOR REPAIR ACCOMPLISHED. INSTALLED FILLER AND ANGLE DOUBLER.
FLAME-OUT OF NUMBER 2 ENGINE. INADEQUATE FUEL SUPPLY PROVIDED TO NR 2 ENGINE. CONDUCTED EXTENSIVE ENGINE AND FUEL SYSTEM DIAGNOSTIC TESTING. DISCOVERED NO INDICATION OF MECHANICAL OR ELECTRONIC SYSTEM FAULTS. CAREFULLY TRACKED AIRPLANE OPERATION AFTER INCIDENT WITH NO FURTHER ANOMALIES INDICATED.
R EICAS CMPTR EICAS MSG DISPLAYED. REPLACED EICAS COMPUTER AND LOADED CORRECT OPS AND OPC SOFTWARE. OPS OK. THERE WAS NO FAULT FOUND IN THE REMOVED EICAS COMPUTER.
AT APPROX 300 AGL NR 2 ENGINE HAD A COMPRESSOR SURGE, MAX EGT 638F. ENGINE EXPERIENCED A GROUP 3 SURGE. SURGE IS CAUSED BY EXCESSIVE BLADE TIP CLEARANCES IN THE HPC REAR STAGES. PERFORMED TEST-21 AND ENGINE FAILED. ENGINE WAS REMOVED FOR SHOP REWORK. AD 2001-25-11 ALREADY IN EFFECT FOR FLEET DETERIORATION MANAGEMENT. FINAL FIX PENDING.
APU WOULD NOT START IN FLIGHT AFTER 3 ATTEMPTS AT FL 300, FL320, AND FL360, RTF.
LEFT GEN TRIP EN ROUTE TO HNL WOULD NOT RESET. APU WOULD NOT START, RTF.
APU FAILED TO START AT FL340. AFTER TURNBACK, IT DID START AT FL200. NR 1 GEN ALSO INOP, RTF.
ON GEAR RETRACTION DURING T/O FROM OGG, AIR DEMAND PUMP LOW PRESS LIGHT CAME ON. DIVERTED TO HNL. *S/D* REPLACED AIR DRIVEN PUMP.
LEFT AND RIGHT ENGINE FLAMEOUT REPORTED AT FL240. RESTARTS ACCOMPISHED. DIVERTED KOA. EMERGENCY DECLARED. LANDED APPROXIMATELY 24000 LBS OVERWEIGHT.
AFTER T/O DURING FLAP RETRACTION A LOUD METAL ON METAL SOUND WAS HEARD THROUGHOUT THE ACFT. THIS NOISE CONFIRMED BY FLT CREW WHILE FLAPS IN MOTION AND CEASED IMMEDIATELY WHEN FLAPS STOPPED. *S/D* INTERNAL FAILURE OF CENTER HYDRAULIC SYSTEM AIR DRIVEN PUMP (ADP) CAUSED GRINDING SOUND IN PUMP. THIS PUMP IS ON DEMAND FOR T/O WITH BOTH LANDING GEAR AND TRAILING EDGE FLAP SYSTEMS OPERATIVE. REPLACED SUBJECT ADP WITH SERVICEABLE UNIT.
