DURING HMV3PSV19 ROUTINE OPN 6002 WAS BEING PERFORMED TO TEST THE RAM AIR TURBINE DEPLOYMENT SYSTEM. THE RAT FAILED TO DEPLOY TO FULL EXTEND AND LOCK POSITION. MTC CLEANED AND LUBED LINKAGE, DOOR HINGES AND ADJUSTED COMPARTMENT DOOR LINKAGE PER MM. RAT WAS RETESTED PER MM 29-21-00-735-143, OPERATION WAS NORMAL.
ON TAXI OUT BHM, NOSE GEAR STEERING GOT VERY WOBBLY, CALLED MTC, THAY FOUND THE RIGHT NOSE GEAR TIRE ASSEMBLY TO HAVE COME APART. SUSPECT OUTER BEARING FROZEN. REMOVED AND REPLACED NOSE WHEEL.
A/C NOSE LANDING GEAR RT AXLE SHEARED ON TAXIWAY, HAD TO REPLACE NOSE LANDING GEAR PISTON.
EQUIPMENT OVERHEAT LIGHT ON AND ALSO HAD A LIGHT ODOR IN COCKPIT. REPLACED NR 1 FWD EQUIPMENT COOLING FAN AND CLEANED FILTER.OPS OK.
DURING THE CURRENT HMV, ADDITIONAL DAMAGE WAS FOUND LOCATED TO AN EXISTING SKIN REPAIR. THE SUBJECT SKIN REPAIR WILL BE REMOVED AND A LARGER REPAIR ENCOMPASSING BOTH DAMAGES INSTALLED. REPAIRED PER ERA 362260-14, REV C.
FUSELAGE SKIN FS 1690-1710, STRINGER18L, DENT REPAIR. THE FUSELAGE SKIN WAS DENTED JUST BELOW STRINGER 18L BETWEEN FS 1690 AND FS 1710. THE DAMAGE WAS REMOVED WITH TWO SKIN CUTOUTS. THE CUTOUTS WERE REPAIRED WITH AN EXTERNAL ALUMINUM DOUBLER PER ER/A 367505-14, REV C.
FUSELAGE SKIN FS 1276, STRINGER 4R, LIGHTNING STRIKE REPAIR. THE FUSELAGE SKIN WAS STRUCK BY LIGHTNING AT FS 1276 AND STRINGER 4R. THE DAMAGE WAS REMOVED FROM THE UPPER SKIN WITH AN APPROX 2 CUTOUT. THE CUTOUT WAS REPAIRED WITH AN EXTERNAL ALUMINUM DOUBLER PER ER/A 367569-14, REV A.
DENT FOUND IN FUSELAGE SKIN AT BS 500 BELOW S26R. DAMAGE CUTOUT IS APPROX 4 BY 6. REPAIRED PER ERA 367513-14.
TWO DENTS WERE FOUND AT BS 1690 AND BS 1710, EACH BELOW S18L. REPAIRED PER ERA 367505-14.
WHILE ACCOMPLISHING PERM REPAIR PER ORIGINAL ISSUE OF ERA 362260-14, ADDITION DAMAGE (TOOLING DAMAGE) WAS FOUND, NECESSITATING A LARGER SKIN CUTOUT. REPAIRED PER ERA 362260-14, REV B.
SMOKE IN CABIN AT LT MID GALLEY FROM AFT COFFEEMAKER. C/B PULLED, SMOKE STOPPED. REPLACED COFFEEMAKER.
MID GALLEY AT 2L DOOR AFT SEC COFFEEMAKER BEGAN SMOKING, MODERATE AMOUNT ACRID SMOKE-POWER SECURED. ALL C/B ASSOCIATED WITH GALLEY PULLED, SMOKE STOPPED IT TOOK 10 MIN FOR HEAT TO STOP COMING FROM TOP CORNER AREA, COOLED SLOWLY. NOTE AT ONE TIME BULKHEAD AFT OF GALLEY WAS WARM TO TOUCH. WATER STARTED LEAKING BAD. SECURED WATER VLV. FOUND WATER MAKER BURNED UP. WATER DISCONNECT LEAKING. ELECT AND WATER DISCONNECTED ON NEW COFFEEMAKER. NEED WATER HOSE ASSY.
AFT EMERGENCY LIGHTING INOP. RESECURED CONNECTOR.
NR 2 ENGINE STALLED WITH LOSS OF OIL PRESSURE AND ENGINE ROLL BACK. REPLACED ENGINE PER 6T103.
P/R - EMERGENCY SLIDE WILL NOT ARM 2R. REATTACHED ENGAGEMENT LUG TO GIRT BAR, CKS GOOD.
LEFT ENGINE FWD MOTOR MOUNT, I/B SIDE THRUST LINK ROD FWD ATTACHMENT TO FAN, HAS 8 BOLTS DAMAGED OR MISSING. ALL FOUR OUTER BOLTS SHEARED, 1 INNER BOLT SHEARED WITH REMAINING 3 DAMAGED. REPLACED CLEVIS DL (P/N 726701473). REMOVED 099 INNER BOLT DL (P/N 726201036), OUTER BOLT DL (P/N 726201037), 4 OF BOTH BOLTS MM 71-21-01-4 IPC 72-00-33.
ON LAYOVER, L-1 AND R-1 DOOR EMERGENCY EXIT LIGHS FAIL TO TEST. REPLACED POWER SUPPLY, OPERATION NORMAL.
