ACRID SMOKE IN CABIN AFTER START UP. AVIONIC ON & ENVIRONMENTAL ON. MX CONFIRMED #2 TRANSPONDER WAS SOURCE OF FUMES. REMOVED AND REPLACED #2 MODE S TRANSPONDER.
TOP OF LEFT OUTBD FLAP HAS 1 3/4" CRACK ON SURFACE. REPLACED LEFT OUTBOARD FLAP WITH REPAIRED FLAT ASSY.
LANDING GEAR HANDLE WAS SELECTED UP. LANDING GEAR DID NOT RETRACT AND INDICATION REMAINED GEAR DOWN.INSPECTED HYD. RESERVOIR AND ACCUMULATOR FOR PROPER SERVICING. INSPECTED ALL CPS AND WIRING FOR SECURITY CHAFING AND PROPER ROUTING ON MLG SQUAT SWITCHES, POWER PACK SWITCHES, POWER PACK SELECTOR VALVE, LANDING GEAR HANDLE SWITCHES, POWER PACK CP SELECTOR VALVE, LANDING GEAR HANDLE. CYCLED GEAR 4 TIMES, GEAR FUNCTIONS NORMAL. REMOVE, INSPECTED AND CLEANED LANDING GEAR UP PRESSURE SWITCH. REINSTALLED AND SAFETIED. CYCLE GEAR 3 MORE TIMES, GEAR FUNCTIONS NORMAL.
LEFT GENERATOR TIE OPEN IN FLIGHT. INSPECTED THE AIRCRAFT AFTER LANDING, OPERATED ALL ELECTRICAL SYSTEMS WITH ENGINES ON LINE. FOUND NO DEFECTS. OPERATED ELECTRICAL DC SYSTEMS DURING ENGINE RUNS & GROUND POWER, BATTERY FOUND GOOD.
SMELL OF SMOKE ON ACFT. FOUND THE ACFT TO HAVE HAD AN ENG COMP WASH PRIOR TO FLIGHT. PERFORMED 10 YEAR LANDING GEAR SWING WITH NO DEFECTS NOTED.
LEFT GEN TIE OPENED ON TAKEOFF ROLL, RESET NORMAL OPEN. R & R BUS TIE CONTROL CARD. RAN & OPERATIONS CHECKED DC GEN SYS & BUS TIE SYS OPERATIONS CHECK WAS GOOD.
DURING C-CHECK INSPECTION DISCOVERED LEFT LOWER SKIN PANEL (P/N 101-400010-113) CORRODED.
DURING C- CHECK INSPECTION DISCOVERED CRACK IN CABIN BOTTOM SKIN (P/N 101-430205-5).
AFTER AFIS SYSTEM HAD GIVEN ERRONEOUS RESULTS, LANDED TO POWER DOWN AND RESET SYSTEM. AFTER POWER UP, ELECTRICAL FUMES WERE PRESENT IN FLIGHT DECK. SHUT DOWN ELECTRICS/ENGINES AND EVACUATED AIRCRAFT.
SIC IN RIGHT SEAT OBSERVED THAT NR 2 INBOARD FORWARD PORTION OF ENGINE COWL WAS LOOSE. IMMEDIATELY NOTIFIED ATC, DISCONNECTED AUTOPILOT AND THEN EXECUTED A PRECAUTIONARY LANDING.
IN AN ATTEMPT TO REDUCE POWER FOR DESCENT, NR 1 ENGINE POWER LEVER LOCKED UP AT CRUISE POWER. DESCENDING THROUGH 8000 FEET, ENGINE WAS SECURED AND AIRCRAFT LANDED SAFELY. REPLACED POWER CONTROL CABLE ASSEMBLY. (M)
RIGHT ENGINE FAILED SHORTLY AFTER TAKE-OFF (1940Z) CLIMBING THROUGH 2,500 FT. EMERGENCY PROCEDURES ACCOMPLISHED, AND LANDED WITHOUT FURTHER INCIDENT. ENGINE WAS REMOVED AND SENT TO O/H FACILITY. ENGINE TEARDOWN REPORT INDICATES THE INFLIGHT SHUTDOWN WAS MOST LIKELY CAUSED BY THE UNCOUPLING OF THE ACCESSORY GEAR BOX TO THE COMPRESSOR. FAILURE OF THE SPADE LOCK SYSTEM ALLOWED THE INPUT SHAFT TO MOVE BACK INTO ITS BEARING, SCORING ITS JOURNAL. THIS REDUCED THE INTER-PLAY OF THE SPLINE AND OVER TORSIONED THE SPLINE CONTACT CAUSING THE UNCOUPLING.
RIGHT ENGINE FAILED SHORTLY AFTER TAKEOFF, CLIMBING THROUGH 2,500 FT EMEGENCY PROCEDURES ACCOMPLISHED, AND LANDED WITHOUT FURTHER INCIDENT. (M)
CREW NOTICED OFFENSIVE ODOR IN COCKPIT AND CABIN. NO SMOKE WAS PRESENT. CREW ABORTED FLIGHT. MECHANIC WAS SENT TO LOCATION, ENGINES WERE RAN AND ALL POWER ON AND COULD NOT DUPLICATE PROBLEM. AIRCRAFT RETURNED TO HOME STATION AND AVIONICS FOUND THAT THE IRU BLOWER MOTOR WAS EXTREMELY HOT. WHEN MOTOR WAS REMOVED AND SHAFT TURNED ROUGH AND COULD SEIZE. MOTOR WAS REPLACED AND NO FURTHER PROBLEMS WERE ENCOUNTERED. (M)
LANDING GEAR CIRCUIT BREAKER TRIPPED. WOULD NOT RESET, GEAR PUMPED DOWN, 3 GREEN INDICATED. PERFORMED INSPECTION OF ALL ACCESSIBLE LANDING GEAR COMPONENTS AND COMPLETED SYSTEMS OPERATIONAL CHECKS. NO DISCREPANCIES WERE FOUND DURING INSPECTION AND LANDING GEAR PERFORMS SATISFACTORILY IAW T14128.2. BEECHCRAFT TECHNICAL REPRESENTATIVE CONSULTED. INVESTIGATION OF THIS CONDITION CONTINUED AT THIS TIME. UPDATE: 3-29-95 - REPAIRED SHORTED WIRE ON MICROSWITCH FOR LANDING GEAR SERVICE VALVE. VERIFIED PROPER OPERATION OF LANDING GEAR. FINAL INSPECTION OF RII COMPLETED BY INSPECTOR. (X)
