DURING LINE CHECK, MAINTENANCE FOUND THE SLIDE LANYARD FOR THE 1R DOOR EMERGENCY ESCAPE SLIDE TO BE FRAYED. MAINTENANCE REPLACED THE LANYARD AND RETURNED THE AIRCRAFT TO SERVICE.

DURING OVERNIGHT CHECK FOUND FWD CARGO COMPARTMENT, LT SIDEWALL SUPPORT HAS SECTION CUT OUT FOR ACCESS TO SHEET METAL REPAIR. REPAIRED SPLICED IN ORIGINAL SIDEWALL STIFFENER SECTION IAW SRM 53-01.
WHILE CRUISING AT FL320, THE RED "CABIN PRESS" WARNING LIGHT ILLUMINATED WITH THE CABIN ALTITUDE CLIMBING. CAPTAIN DECLARED AN EMERGENCY, DIRECTED AN IMMEDIATE DESCENT TO 10,000 AND EXECUTED THE RED BORDERED CHECK LIST. THE MASKS WERE DEPLOYED IAW QRH. FLIGHT CONTINUED TO DESTINATION AT 10,000 FEET ALTITUDE AND LANDED WITHOUT FURTHER INCIDENT. MX FOUND AN ACM TURBINE OUTLET DUCT SEPARATED AND REPLACED IT AND THE APPROPRIATE CLAMPS.
DURING M-CHECK FOUND HAT CHANNELS ON FLOOR PANEL NR 17 CHAFED ON ENDS. REPAIRED PER EA 98-216802.
DURING LINE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY SIDEWALL LIGHTING IN THE VENTRAL STAIRWAY WAS INOPERATIVE. MAINTENANCE REPLACED THE BATTERIES IN THE POWER SUPPLY AND THE LIGHTS RETURNED TO AN OPERATIONAL CONDITION.
DURING LINE CHECK, MAINTENANCE FOUND THAT TWO OF THE THREE EMERGENCY PATHWAY LIGHTS FOR THE CATWALK IN THE TAIL COMPARTMENT WAS INOPERATIVE. MAINTENANCE REPAIRED BROKEN WIRING, REPLACED THE BATTERIES IN THE POWER SUPPLY, AND FUNCTIONALLY CHECKED THE SYSTEM.
DURING LINE CHECK, MAINTENANCE FOUND THAT THE EMERGENCY PATHWAY LIGHTING IN ROWS 5/8 OF THE CABIN WERE INOPERATIVE. MAINTENANCE REPLACED THE POWER SUPPLY AND BATTERY PACKS AT THE FWD RT OVERWING EXIT SIGN AND THE LIGHTS RETURNED TO AN OPERATIONAL CONDITION.
DURING LINE CHECK, MAINTENANCE FOUND THAT THE EXTERIOR EMERGENCY FLOOD LIGHT WERE INOPERATIVE. MAINTENANCE REPLACED BATTERY PACK AND LIGHTS. RETURNED TO AN OPERATIONAL CONDITION.
THE RIGHT MAIN LANDING GEAR TIRE AND WHEEL SEPARATED FROM THE AXLE ON TAKEOFF OUT OF DTW. SOME DEBRIS DID IMPACT UPON THE FUSELAGE AND ENGINE COWLING CAUSING MINOR DAMAGE. THE FIRST STAGE COMPRESSOR DISK WAS REPLACED DUE TO DAMAGE ALSO. THE AIRCRAFT CONTINUED TO, AND LANDED SAFELY WITHOUT FURTHER INCIDENT, AT BDL. THE RIGHT MAIN LANDING GEAR WAS INSPECTED, NEW TIRES INSTALLED IN THE NR 3 AND NR 4 POSITIONS, ALL SECONDARY DAMAGE WAS REPAIRED, AND THE AIRCRAFT REMAINS OUT OF SERVICE AS OF 21MAR06.
DURING LINE CHECK FOUND CRACK IN AFT PRESSURE DOOR FRAME STEEL SPLICE STRAP J060110-93. REPAIRED PER EA 98-200540.
FIRST OFFICER ATTEMPTED TO LOWER REAR STAIRWAY AND DROPPED THE TAIL CONE INSTEAD. TAIL CONE EVACUATION SLIDE DID NOT DEPLOY OR INFLATE. REPLACED THE SLIDE AND THE TAIL CONE. AIRCRAFT RETURNED TO SERVICE.
DURING M-CHECK FOUND CRACK IN STRINGER 27R AT FORWARD SECTION OF AUX FUEL TANK. REPAIRED PER EA 98-177371, REFERENCE LOG PAGE 1512239.
DURING M-CHECK, FOUND CORROSION ON RIGHT SIDE OF LOWER FORWARD AUX FUEL TANK SUPPORT BRACKET. REPAIRED PER EA 98-147731 REFERENCE LOG PAGE 1512238.
DURING CLIMBOUT AFTER TAKEOFF, THE CABIN FILLED WITH SMOKE. CREW DECLARED AN EMERGENCY AND RETURNED TO FNT WITHOUT INCIDENT. MAINTENANCE DISCOVERED THE AIR CYCLE MACHINE WAS PUMPING SMOKE INTO THE CABIN. REPLACED THE AIR CYCLE MACHINE, NUMEROUS TEMP SENSORS, CONTROLLERS, AND VALVES. AIRCRAFT RETURNED TO SERVICE.
FLIGHT ATTENDANT REPORTED FORWARD CABIN EMERGENCY PATH LIGHTS INOPERATIVE. REPLACED BATTERY PACK AND BATTERIES. OPERATIONAL CHECK NORMAL.
LOSS OF RIGHT HYDRAULIC SYSTEM QUANTITY OCCURRED DURING CRUISE FLIGHT. CREW REQUESTED EMERGENCY EQUIPMENT ON STANDBY AT DESTINATION STATION AND FLIGHT WAS CONTINUED. AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED THE RIGHT HYDRAULIC SYSTEM QUANTITY INDICATOR. OPERATIONAL CHECK NORMAL. AIRCRAFT RETURNED TO SERVICE.
DURING CRUISE FLIGHT EN ROUTE BWI-DTW, SPARKS BEGAN EMITTING FROM CENTER COCKPIT PEDESTAL UNDER ADF, ARCING LASTED APPROAIMATELY FIVE SECONDS. FLIGHT WAS DIVERTED TO CLE AND LANDED WITHOUT INCIDENT. MAINTENANCE INSPECTED FORWARD PEDESTAL AREA AND FOUND WIRES BURNED AT P1-244 TO SPOILER ACTUATOR. COTTER PIN IN SPOILER ACTUATOR ROD WAS FOUND CHAFED TO WIRE BUNDLE. REPAIRED WIRES AND REROUTED WIRE BUNDLE AWAY FROM SPOILER ACTUATOR ROD, OPERATIONAL CHECK NORMAL.
DURING PERIODIC CHECK, FOUND 4.5 INCH CRACK ON LOWER LEFT WING LEADING AT WS 195 FORWARD OF ACCESS DOOR 1132A. INSTALLED INTERNAL AND EXTERNAL DOUBLERS PER EA 98-163654.
DURING LINE MAINTENANCE INSPECTION, FOUND LEFT FORWARD OVERWING EMERGENCY EXIT SIGN LIGHT INOPERATIVE. REPLACED BATTERIES, OPERATIONAL CHECK NORMAL.

ON DEPARTURE, NOSE GEAR WOULD NOT RETRACT. NOSE STEERING WAS UNLOCKED AND AIRCRAFT WOULD NOT PRESSURIZE. FLIGHT DIVERTED TO MIA AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED SCHRADER VALVE AND 'O' RING AND SERVICED STRUT, GEAR OPERATIONAL CHECK OK.
ON TAKEOFF FROM DTW, NOSE GEAR WOULD NOT RETRACT WHEN SELECTED UP. NO PRESSURIZATION AND GEAR UNSAFE LIGHT ILLUMINATED. RECYCLED GEAR WITH NO CHANGE. FLIGHT RETURNED TO DTW AND LANDED WITHOUT INCIDENT. RESEALED AND SERVICED NOSE STRUT ASSEMBLY. GEAR OPERATIONAL CHECK OK.
DURING LINE CHECK, FOUND CEILING EMERGENCY LIGHT AT SEAT ROW 2 INOPERATIVE. REPLACED BATTERIES. OPERATIONAL CHECK OK.
DURING CRUISE TO IND, THE NR 2 VOR BECAME INOPERATIVE. FLIGHT RETURNED TO DTW AND LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED NAV RECEIVER. OPERATIONAL CHECK GOOD.
DURING LINE CHECK, FOUND OVERHEAD CABIN EMERGENCY LIGHTS AT MID CABIN AREA INOPERATIVE. REPLACED BATTERIES, OPERATIONAL CHECK OK.
DURING CLIMBOUT, THE RIGHT ICE PROTECTION TEMPERATURE HIGH LIGHT ILLUMINATED. FOLLOWED COM PROCEDURES AND LIGHT WENT OUT. FLIGHT RETURNED TO MSP AND LANDED WITHOUT INCIDENT. REPLACED THE RIGHT AUGMENTATION VALVE AND ICE PROTECTION REGULATOR VALVE, OPERATIONAL CHECK OK.
DURING LINE MAINTENANCE INSPECTION, FOUND ROWS 12 THROUGH 15 OVERHEAD EMERGENCY LIGHTS INOPERATIVE. REPLACED BATTERIES, OPERATIONAL CHECK NORMAL.
ON CLIMBOUT, CAPTAIN'S VSI INDICATOR FAILED. DUE TO WEATHER CONDITIONS AT DESTINATION (AZO) DECISION WAS MADE TO DIVERT TO DTW FOR REPAIR. AIRCRAFT LANDED IN DTW WITHOUT INCIDENT. REPLACED CAPTIAN'S VSI INDICATOR, OPERATIONAL CHECK OK.
NOSE LANDING GEAR RED UNSAFE LIGHT REMAINED ON AFTER GEAR WAS RETRACTED. FLIGHT RETURNED TO DTW AND LANDED WITHOUT INCIDENT. REPLACED NOSE GEAR PROXIMITY CONTROL UNIT AND DOWNLOCK SENSOR. CYCLED GEAR SEVERAL TIMES. OPERATIONAL CHECK OK. AIRCRAFT RETURNED TO SERVICE.
