RETURNED TO LAX DUE TO RIGHT HYDRAULIC SYSTEM FAILURE. LOSS OF FLUID DUE TO FAILED O-RING ON CASE DRAIN MODULE DIFFERENTIAL PRESSURE INDICATOR BOSS. REPLACED O-RING AND RESERVICED HYDRAULIC SYSTEM.
CREW REPORTS TWO UNCOMMANDED YAW EVENTS EN ROUTE. LT AND RT YAW DAMPENER MODULES WERE CHECKED FOR RECORDED FLT FAULTS AND NONE WERE FOUND. MCDP CHECKS 1, 9, 40, AND 69, RELATING TO THE AUTOPILOT DIRECTIONAL SERVOS, WERE PERFORMED AND ALL CHECKED OK. CENTER FCC WAS REPLACED AS A PRECAUTION. AUTOPILOT DOES NOT INTERFACE WITH RUDDER SERVOS DURING CRUISE. FLIGHT RECORDER DATA DID NOT INDICATE ANY ENGINE ANOMALIES TO ACCOUNT FOR THE EVENT. THERE IS NO CLEAR EXPLANATION FOR THE UNCOMMANDED YAW.
RETURNED TO ORD DUE TO LEADING EDGE SLAT INDICATION. REPLACED THE S282, AND S297 SWITCHES. SYSTEM OPS CHECKS GOOD.
NR 2 ENG SILENT SURGE FLAGGED BY AIRCRAFT. THIS EVENT WAS A SILENT SURGE THAT WAS REPORTED BY THE ENGINEER WHO MONITORS THE ECM. THE EPR WAS BELOW 1.25. THE AIRCRAFT AND FLIGHT CREW DID NOT REPORT THIS EVENT. SURGE CHECKS WERE ACCOMPLISHED ON THE ENGINE WITH NO DISCREPANCIES NOTED. AIRCRAFT RETURNED TO SERVICE.
LEADING EDGE SLAT ASYMMETRY EICAS DISPLAYED. ACCOMPLISHED APPROPRIATE FAULT ISOLATION MANUAL, AND AIRCRAFT MAINTENANCE MANUAL PROCEDURES. BITE TESTED THE FSEU, AND PSEU, INSPECTED ALL TARGETS. OPERATED THE FLAPS ELECTRICALLY AND HYDRAULICALLY. NO FAULTS FOUND UNABLE TO DUPLICATE INTERMITTENT EICAS MESSAGE.
LEFT WING INBD FLAP CENTER EXTERNAL HINGE FITTING CRACKED ON INBD SIDE FLANGE. MAJOR REPAIR ACCOMPLISHED. REPAIRED HINGE FITTING PER SRM 57-53-02, FIG 201 AND INSTALLED REPAIR PER SRM 51-40-2.
ABORTED TAKEOFF AT 80 KTS TWICE DUE TO FLAPS CONFIGURATION WARNING. CAUSE OF CONFIGURATION WARNING IS UNKNOWN. THE TAKEOFF CONFIGURATION MODULE WAS REPLACED. SHOP TESTING SHOWED NO FAULT FOUND.
LT BLEED FAILED ON TAKEOFF ROLL. CREW WAS DOING A PACKS OFF TAKEOFF WITH ENG BLEED SWITCHES ON AND ALL ISOLATION VALVES OPEN. NORMAL CONDITION TO GET BLEED OFF ON ONE SIDE OR THE OTHER DUE TO DUAL BLEED PROTECTION.
EICAS MESSAGE "LE SLAT ASSY" DISPLAYED AFTER TAKEOFF. DIVERTED INTO HNL. SUPPLEMENTAL DATA UPDATE REPLACED #9 OUTBOARD SLAT PROXIMITY SWITCH S297. SYSTEM OPERATIONAL CHECK OK, NO ADDITIONAL OCCURENCES.
