RIGHT MAIN LANDING GEAR COLLAPSED ON LANDING ROLLOUT.
MAJOR REPAIR ENGINEERING ORDER TO REPAIR DAMAGE FOUND DURING INSPECTION IN THE WEB OF STRINGER CAUSED BY SCREW THREADS, WHICH IMPINGED ON THE STRINGER WEB DUE TO AN IMPROPERLY LOCATED NUTPLATE
RT FUEL XFER AMBER LIGHT CAME ON IN FLIGHT LONG WITH LOW PRESSURE LIGHT. MAINTENANCE REPLACED RIGHT ENGINE LOW FUEL PRESSURE SWITCH.
RIGHT BLEED AIR FAIL ILLUMINATED IN FLIGHT, WENT OUT DURING DESCENT. FOUND EVA TUBING CHAFED ON THE METAL TAPE AROUND BLEED LINE, OTBD OF RT INBD WING. NO EVIDENCE OF BLEED AIR LEAK. REPAIRED WITH SPLICE TUBE.
UPPER LEFT ZONE OF AFT PRESSURE BULKHEAD IS GOUGED. REPAIRED IAW EO 15-28-10.
MID RIGHT ZONE OF THE AFT PRESSURE BULKHEAD IS GOUGED. REPAIRED IAW EO 15-28-10.
UPPER RIGHT ZONE OF THE AFT PRESSURE BULKHEAD IS GOUGED. REPAIRED IAW EO 15-28-10.
LEFT BLEED AIR FAIL & MASTER CAUTION ILLUMINATED. WARNING LIGHT EXTINGUISHED AFTER COMPLETING IMMEDIATE ACTION ITEMS. FOUND EVA TUBING IN NEED OF REPAIR. REPAIRED TUBING. NO FURTHER ACTION REQUIRED.
RIGHT BLEED AIR FAIL & MC ILLUMINATED. A HOLE WAS DISCOVERED IN THE EVA SENSE LINE IN RT NACELLE LE. LINE WAS REPLACED WITH NO DEFECTS NOTED.
CABIN DOOR ANNUNCIATOR ILLUMINATED DURING FLIGHT. ADJUSTED CABIN DOOR LOCK SWITCH.
WITH RT BLEED AIR PNEUMATIC ENV OFF, RT BLEED FAIL LIGHT INTERMITTENT ON & OFF. FOUND & REPLACED RUPTURED EVA TUBE.
CRACKS SUSPECTED IN FUSELAGE SKIN VERIFIED BY NDT. THE DAMAGED AREA IS LOCATED INBD OF STRINGER 14L & NEAR FRAME 177.5. THE TWO FORE AFT CRACKS IN THE BELLY SKIN ARE APPROX 3.5 INCHES LONG & RUN FROM APPROX 1.5 INCH AFT TO 1.8 INCH FWD OF FRAME 177.5 & ARE ROUGHLY 1 INCH INBD OF STRINGER 14LH. THE SIDE TO SIDE CRACK IN THE SKIN UNDER FRAME 177.5 RUNS FROM ROUGHLY 1.5 INCH INBD TO 3.5 INCH INBD OF STRINGER 14LH. THE CRACK IN THE FS 177.5 FRAME IS IN THE BEND RADIUS BETWEEN THE WEB & CHORD & RUNS APPROX 4 INCH INBD FROM THE STRINGER 14LH. REPAIRED USING EO.
ON APPROACH, LANDING GEAR EXTENDED. LANDING GEAR APPEARED TO EXTEND NORMALLY, HOWEVER RIGHT MAIN GEAR INDICATED UNSAFE (GREEN DOWN AND LOCKED LIGHT NOT ILLUMINATED). ATC ADVISED OF INTENTION TO HOLD WEST OF AIRPORT IN ORDER TO UTILIZE "LANDING GEAR UNSAFE INDICATION" ABNORMAL CHECKLIST. EXECUTED LANDING GEAR MANUAL EXTENSION IAW CHECKLIST. FLIGHT CREW DECLARED AN EMERGENCY AND REQUESTED CRASH/FIRE/RESCUE EQUIPMENT. REQUESTED AND RECEIVED CLEARANCE FOR FLY-BY. TOWER ADVISED THAT RIGHT MAIN LANDING GEAR APPEARED TO BE DOWN AND IN PLACE. AN UNEVENTFUL LANDING FOLLOWED. MAINTENANCE FOUND RIGHT MAIN GEAR DOWN GREEN LIGHT BULB BAD. INSTALLED NEW BULB. JACKED AIRCRAFT AND PERFORMED MLG RETRACTION. OPS CHECKED GOOD.
DURING FLARE FOR LANDING, THE AIRCRAFT ABRUPTLY AND WITHOUT WARNING YAWED HARD RIGHT. TOUCHDOWN OCCURRED DURING THE GO-AROUND. THE PILOT NOT FLYING REPORTED THAT THE RIGHT PROP PITCH LIGHT ILLUMINATED IMMEDIATELY PRIOR TO THE EXCURSION. AFTER THE GO-AROUND, THE PROPELLERS WERE RESET TO 1500 RPM AND A POWER-ON APPROACH TO LANDING WAS ACCOMPLISHED. DURING THAT TOUCHDOWN, THE RIGHT PROP PITCH LIGHT ILLUMINATED AGAIN.
RT PROP PITCH LIGHT ILLUMINATED IN FLIGHT. AIRCRAFT YAWED RIGHT. POWER INCREASED TO MAINTAIN CONTROL. WHEN RETURNED TO IDLE FOR LANDING IT WAS OK. TROUBLESHOT AND NO FAULT FOUND. DURING FLARE FOR LANDING, RT PROP APPEARED TO GO INTO GROUND RANGE, NO PROP PITCH LIGHT NOTED. AFTER LANDING, CONDUCTED GND IDLE LOW PITCH CHECK PER CFM, NO ANOMALY NOTED. ADJUSTED BETA RODS AND SET FLIGHT IDLE TORQUE. ALL CHECKS NORMAL.
LANDING GEAR FAILED TO EXTEND WHEN 'GEAR DOWN' SELECTED.. RESULTED IN EMERGENCY EXTENSION OF LANDING GEAR. THE MICRO-SWITCH IN THE LANDING GEAR CONTROL SWITCH ASSEMBLY WAS FOUND TO BE STICKING. THE LANDING GEAR CONTROL SWITCH WAS REPLACED AND SYSTEM OPERATED NORMALLY. (M)
PRECAUTIONARY LANDING WAS MADE AFTER NOSE AND RT MLG LIGHTS FAILED TO LIGHT DURING MANUAL EXTENSION. LIGHT BULBS IN GEAR LIGHTS WERE FOUND BURNED OUT. LIGHT BULBS WERE REPLACED AND GEAR WAS OPERATIONALLY CHECKED THROUGH 6 CYCLES, CHECKED OKAY. COMPONENT PN 3086070843004. (X)
