LEFT-HAND FUSELAGE SKIN UNDER FORWARD WING TO BODY FAIRING CORRODED. MAJOR REPAIR ENGINEERING ORDER 17-28-43 "LOWER FUSELAGE MULTIPLE BAY SKIN REPAIR" ISSUED TO REPAIR THE CORROSION DAMAGE TO LOWER FUSELAGE SKIN BEHIND THE WING-TO-BODY FAIRING. THE DAMAGE EXTENDS OVER TWO BAYS FROM APPROXIMATELY FS188 TO FS202.
AN INFLIGHT OUT OF TRIM SITUATION WHICH WAS DESCRIBED AS, AN EXCESSIVE NOSE UP PITCH, ON TAKEOFF REQUIRING AN IMMEDIATE ACTION. FOUND THE BRASS DRIVE GEAR LOOSE ON THE ELEVATOR TRIM WHEEL. THIS RESULTED IN THE TRIM POSITION INDICATING DIAL SLIPPING DURING TRIM WHEEL OPERATION & GIVING A FALSE INDICATION OF THE ELEVATOR TRIM TAB POSITION. MX SHOULD BE AWARE OF THE POSSIBILITY OF SLIPPAGE, BETWEEN THE ELEVATOR TRIM WHEEL & THE DIAL ASSEMBLY. A MX ADVISORY CONTAINING INSTRUCTIONS ON HOW TO CHECK FOR & CORRECT THIS HAS BEEN ISSUED.
DURING CLIMB OUT AFTER ILS APPROACH, CREW NOTED RED LIGHTS IN THE GEAR HANDLE & A BLINKING LDG TAXI LIGHT CAUTION ADVISORY ANNUNCIATOR. THE LG RELAY CB WAS POPPED. VISUALLY CONFIRMED THE RT MLG WAS PARTIALLY UP. BEECH 300 POH ABNORMAL PROCEDURES FOLLOWED. UNSAFE INDICATION CORRECTED, ATC NOTIFIED. SAFE LANDING ACCOMPLISHED WITHOUT FURTHER INCIDENT. CHECKED ACFT ON GND & WAS UNABLE TO DUPLICATE PROBLEM.
WHILE CONDUCTING AN ILS-1, STRONG ELECTRICAL FUMES (NO SMOKE) FILLED THE FUSELAGE. DETERMINED FUMES WERE EMANATING FROM FLIGHT INSPECTION PANEL. RAN SMOKE AND FUME ELIMINATION CHECKLIST AND EXECUTED IMMEDIATE LANDING. MAINTENANCE VHF COMM NR 3 WAS BURNED OUT.
WHILE AT CRUISE, TECHNICIAN NOTICED A BURNING SMELL, WITHIN A FEW SECONDS SMOKE BEGAN TO FORM IN THE BACK OF THE AIRCRAFT. PIC DIRECTED EVERYONE TO DON OXYGEN AND RUN APPROPRIATE EMERGENCY CHECKLIST. CREW DECLARED EMERGENCY WITH ATC AND BEGAN VFR DESCENT TO CLOSEST SUITABLE AIRFIELD. LANDED UNEVENTFULLY WITHIN APPROX 5 MINUTES OF THE EMERGENCY. CLEARED THE RUNWAY AND EVACUATED AIRCRAFT. SOURCE OF SMOKE UNDETERMINED AT THIS TIME.
WHILE CLIMBING TO EN ROUTE CRUISING ALTITUDE OF FL190, CABIN DOOR ANNUNCIATOR ILLUMINATED AT APPROXIMATELY 18,700 FT. EXECUTED EMERGENCY DESCENT AND "CABIN DOOR UNLOCK" EMERGENCY PROCEDURES. AT 9000 MSL CABIN DOOR ANNUNCIATOR EXTINGUISHED. AIRCRAFT LANDED UNEVENTFULLY. INSPECTION OF DOOR AFTER LANDING INDICATED IT WAS SECURELY LATCHED. MAINTENANCE FOUND UPPER AFT DOOR SWITCH OUT OF ADJUSTMENT. ADJUSTED AFT DOOR SWITCH. PRESSURIZED AIRCRAFT TO 6 PSI. DOOR WARNING SYSTEM CHECKED SAT.
AFTER 45 MINUTES FROM DEPARTING JEFFERSON CITY, MO EN ROUTE TO OKC, OK AT FL260, THERE WAS A LOUD SOUND (LIKE A GUN SHOT) AND THE PILOT SIDE WINDSHIELD WAS SHATTERED WITH SHROUDS OF GLASS FALLING OUT. INFORMED KANSAS CITY CENTER AND REQUESTED IMMEDIATE DESCENT TO CLOSEST VFR AIRPORT WITH 6000 FT RUNWAY. ABNORMAL SHATTERED WINDSHIELD CHECKLIST PERFORMED AND PNF LANDED THE AIRCRAFT.
CLIMBING THRU 7300`, AT CLIMB POWER, HEARD A PRONOUNCED POP, FELT THE ACFT YAW LT AND FELT A DECELERATION. THE PILOT FLYING CONFIRMED THAT THE LT ENGINE HAD SHUTDOWN AND THE PROP HAD FEATHERED. ATC WAS NOTIFIED AND EMERGENCY WAS DECLARED. THE ENGINE FAILURE IN FLIGHT EMERGENCY CHECKLIST WAS COMPLETED AS WELL AS THE SINGLE ENGINE LANDING CHECKLIST. AFTER AN UNEVENTFUL LANDING AND TAXI OFF OF THE RUNWAY, THE SHUTDOWN CHECKLIST WAS ACCOMPLISHED AND THE ACFT WAS TOWED TO THE HANGER. (K)

ACRID FOUL SMELLING SMOKE PERMEATED CABIN, ENVIROMENTAL TEMP CIRCUIT BREAKER POPPED. DECLARED EMERGENCY AND DIVERTED. SMOKE DISSIPATED. CABIN TEMP INCREASED BECAUSE FORWARD AND AFT BLOWER MOTORS WERE INOPERATIVE. DID NOT REQUEST CRASH EQUIPMENT. LANDED UNEVENTFULLY. MECHANIC COULD NOT DUPLICATE AFTER 30 MINUTE RUN UP WITH CREW PRESENT. ALL SYSTEMS OPERATING. AIRCRAFT RETURNED TO AIRWORTHY CONDITION.
