DISCREPANCY: DURING C-CHECK AT BQK, STAMBAUGH AVIATION FOUND CORROSION ON THE LH KEEL BEAM IN THE AFT LOWER CARGO PIT BETWEEN FUSELAGE STATIONS 950-1010.CORRECTIVE ACTION: STAMBAUGH AVIATION IN BQK, REMOVED AND REPLACED THE LH KEEL BEAM BETWEEN STATIONS 950-1010 IAW B727 SRM 51-30-2. THEY PERFORMED AN HFEC INSPECTION OF THE NEW INSTALLED LH KEEL BEAM IAW B727 NDT MANUAL PARAGRAPH 6 51-00-00 PROCEDURE 23. NO DEFECTS NOTED AT THIS TIME
DURING ROUTINE MAINTENANCE AT KYIP CORROSION WAS FOUND ON R/H HORIZONTAL STAB TRAILING EDGE BEAM AT STATIONS 88.9, 166.5, AND 182.1. REPAIR STATION FABRICATED AND INSTALLED 3 EACH REPAIR STRAPS IAW FVCI, REPAIR DWG. 55-19103-1, -2, AND -3. AND 8110-3âS 19103-1, -2, AND -3,.
CREW REPORTED â EXPERIENCED SEVERAL ABNORMAL AIRCRAFT VIBRATIONS. DISCUSSED WITH CREW. DECLARED AN EMERGENCY. LANDED AT NEAREST SUITABLE AIRPORT USING NORMAL PROCEDURES. USA JET MAINTENANCE WAS SENT TO KAFW TO TROUBLESHOOT PROBLEM. UPON INSPECTION OF THE #2 ENGINE METAL SHAVINGS WERE FOUND IN THE INLET. THE FRONT ACCESSORY COVER WAS REMOVED, AND IT WAS FOUND THAT THE #1 BEARING HAD FAILED. THIS WAS THE CAUSE OF THE VIBRATION IN FLIGHT. THE #2 ENGINE IS BEING REPLACED AND THE ENGINE SHOP WILL SEEK TO DETERMINE THE CAUSE OF THE #1 BEARING FAILURE. NOTE: C1 FAN DISK WAS REPLACED AT SHOP VISIT WHICH WOULD HAVE NECESSITATED THE INSPECTION OF THE #1 BEARING FOR SERVICEABILITY. A SUPPLEMENTAL REPORT WILL BE FILED IF THE ENGINE SHOP CAN DETERMINE A CAUSE FOR THE #1 BEARING FAILURE WITH SOME DEGREE OF CERTAINTY. THIS ENGINE IS ON LEASE TO USA JET AIRLINES AND THE #1 BEARING MANUFACTURER, PART NUMBER AND SERIAL NUMBER ARE NOT IMMEDIATELY AVAILABLE. REPORT IS BEING FILED WITHOUT THIS INFORMATION TO MEET THE FAR 121 REPORTING REQUIREMENTS FOR AN INCIDENT SUCH AS THIS. A SUPPLEMENTAL REPORT SHALL BE FILED ONCE THIS INFORMATION IS MADE AVAILABLE BY THE ENGINE LESSOR.
PIT - THE AFT EMERGENCY POWER SUPPLY UNIT FAILED TEST. MAINTENANCE REPLACED THE AFT EMERGENCY POWER SUPPLY UNIT. OPERATIONALLY CHECKS OK.
RDU - DURING THE EMERGENCY LIGHT TEST, RECEIVED 'EPSU COMM EMER R' MESSAGE. MAINTENANCE REPLACED A DECODER/ENCODER UNIT. OPERATIONALLY CHECKED OK. (M)
DCA - RECEIVED AN EPSU EXT LOAD EMER R AND AN EPSU INT LOAD EMER R FAULT MESSAGE DURING THE EMERGENCY LIGHTS TEST. MAINTENANCE REPLACED THE CENTER RIGHT EMERGENCY POWER SUPPLY UNIT. SYSTEM OK FOR SERVICE. (M)
DCA - THE AFT LEFT DOOR EMERGENCY ESCAPE SLIDE FAILED THE EPSU TEST. MAINTENANCE RESEATED A PIN IN THE CANNON PLUG FOR THE AFT LEFT DOOR EMERGENCY ESCAPE SLIDE. SYSTEM OK FOR SERVICE. (M)
