TEN MINUTES INTO THE FLIGHT AT 14000' THE LEFT ENGINE BLEED AIR FAIL LIGHT ILLUMINATED. FOUND A HOLE IN EVA TUBING. TUBING REPLACED.
LEFT BLEED AIR FAIL LIGHT ILLUMINATED IN FLIGHT, 25 MIN INTO FLIGHT. MAINTENANCE REPLACED LEFT BLEED AIR VALVE.
L BL AIR FAIL LIGHT 1 HR INTO FLIGHT. LIGHT RESET WITH PNEU/ENV OFF. ILLUMINATED 30" LATER. RESET WITH PNEU/ENV OFF. NEVER ILLUMINATED AGAIN FOR .8 FLIGHT TIME. DISCREPANCY CLEARED WITH THE REPLACEMENT OF LEFT HAND BLEED AIR PRESSURE SWITCH
WITH LANDING GEAR CONTROL HANDLE IN UP POSITION GEAR REMAINS DOWN AND 3 GREEN GEAR DOWN LIGHTS REMAINED ILLUMINATED. ENGAGED DOWN LOCK REL TO RAISE GEAR.
LT MAIN GEAR LT DID NOT ILLUMINATE WITH GEAR HANDLE DOWN. UNSAFE LANDING GEAR CHECK LIST AND TROUBLESHOOTING BY MAINTENANCE REVEALED THE LT MAIN LANDING GEAR DOWN AND LOCKED INDICATOR LAMP WAS BURNED OUT. REPLACING THE LAMP RESOLVED THE ISSUE. NO FURTHER ACTION WAS REQUIRED.
WHEEL BEARING FAILED. TIRE BLEW. REPLACED THE BEARING IAW MM 32-45-23.

LANDING GEAR HANDLE DOWNLOCK DID NOT RELEASE ON TAKEOFF. RELEASED AFTER APPROXIMATELY 5 MINUTES OF LEVEL FLIGHT. CORRECTED BY REPLACEMENT OF SAFETY SWITCH ASSEMBLY.
MASTER WARNING LIGHT ILLUMINATED WITH AN ASSOCIATED, AUTOPILOT DISCONNECT, RED SEGMENT LIGHT. THERE WAS NO ACTUAL DISCONNECTING OF THE AUTOPILOT & THERE WAS NO AUDIBLE WARNING ASSOCIATED WITH A DISCONNECTING AUTOPILOT. R & R NR 1 AUTOPILOT MONITOR.
PILOT MODE SELECTOR PANEL & AUTO PILOT CONTROL PANEL STARTED FLASHING. PF FELT WHAT WAS BELIEVED TO BE AN UNCOMMANDED ENGAGING & DISENGAGING, BUMP OF THE YAW DAMPER IN THE RUDDER PEDALS. REPLACED MODE SELECTOR & AP MODE CONTROLLER.
AILERON AUTOPILOT SERVO CABLE FRAYED FROM CONTACT WITH GUARD POST. REPOSITIONED CAPSTAN GUARD POSTS TO ALLOW FOR CLEAR MOVEMENT OF CABLE.
OIL PRESS FLUCTUATING BETWEEN 70 & 800 PSI ON THE NR 1 ENG. FOUND NO OIL ON DIPSTICK WHEN CHECKED. ENG WAS SERVICED TO BRING OIL QUANTITY UP. RAN ENG & FOUND ALL OIL PRESSURES, TEMPS & ENG TORQUE NORMAL.
RIGHT WING LE HINGE SEGMENTS BROKEN. SEGMENTS HAD SEVERE CORROSION, UNABLE TO REMOVE HINGE PIN THEREFORE, RT OTBD FRONT SPAR IS BEING REPLACED.
DURING PREFLIGHT, CREW NOTICED A STRONG SMELL OF WHAT THEY THOUGHT WAS A SANITIZER. APPROX 2 HOURS INTO THE FLT, THE SMELL BECAME IRRITATING, CAUSING THROAT AND CHEST IRRITATION, NAUSEA, HEADACHE AND SLIGHT DIZZINESS. DURING FLIGHT AT LOW ALTITUDES, CABIN DUMP SELECTION APPEARED TO LESSEN THE INTENSITY OF THE ODOR FOR THE SHORT PERIODS THAT IT WAS USED. AFTER LANDING. LINEMAN COMMENTED ABOUT THE STRONG ODOR COMING FROM THE AIRCRAFT CABIN WHILE HE WAS STANDING AT THE AIRSTAIR. MX WAS CHANGING THE AC (R134) FLUID. DURING THE REFRIGERANT FLUSH, APPROXIMATELY 6 OZ OF FLUSHING SOLVENT, TERPENE HYDROCARBON, WAS SPILLED UNDER THE CABIN FLOOR ON THE MAIN AIR CONDITIONER AFT EVAPORATOR BLOWER. BOTH CABIN FILTERS WERE CONTAMINATED BY A YELLOW SUBSTANCE WITH A STRONG CHEMICAL ODOR.
ABORT PRIOR TO V1 DUE TO TCAS PROCESSOR FAILURE.
LEFT MAIN UNSAFE GEAR INDICATION. SUSPECT BAD LIGHT BULB. MANUALLY LOWERED GEAR IAW ABNORMAL PROCEDURES CHECKLIST. DECLARED EMERGENCY. UNEVENTFUL LANDING. MAINTENANCE REPLACED LIGHT BULB. OPS CHECK GOOD. ALL FUNCTIONAL AND OPERATIONAL CHECKS GOOD.
ON TAKEOFF AND INITIAL CLIMB OUT ALL CREWMEMBERS NOTICED ABNORMAL SMELL; MISSION SPECIALISTS (2) NOTED HAZE THROUGHOUT CABIN WHICH REDUCED VISIBILITY, NON-FLYING PILOT CONFIRMED; IFR CANCELLED, FLYING. PILOT EXECUTED REVERSAL AND PRECAUTIONARY LANDING BACK AT DEPARTURE POINT.
STRONG HEAT SMELL WHEN WINDSHIELD HEAT SELECTED ON. MAINTENANCE FOUND EFIS COOLING TUBE CHAFING ON CONTACT (A2) OF PILOTS WINDOW HEAT RELAY (LOW) K118. REPOSTIONED TUBE AND PROTECTIVE BOOTS ON RELAY CONTACTS. ALSO FOUND HIGH HEAT RELAY K119 MAKING HUMMING SOUND. REPLACED K119 WITH NEW RELAY P/N MS24166-D1. OP CHECKS OK.
DURING C CHECK INSPECTION DISCOVERED LOWER SKIN CRACKED ON LEFT OUTBOARD WING FLAP.
ELECTRICAL BURNING SMELL IN AIRCRAFT INFLIGHT. INITIATED APPROPRIATE CHECKLIST. ATC NOTIFIED. PRECAUTIONARY LANDING MADE. ELECTRICAL SMELL DISSIPATED PRIOR TO LANDING. LOCAL MAINTENANCE DUPLICATED THE SMELL ON THE GROUND BUT UNABLE TO DETERMINE SOURCE.
HYDRAULIC FLUID LOW LIGHT ILLUMINATED APPROX 10 MIN AFTER INITIAL TAKEOFF, WHILE LEVEL AT 4000 FT MSL. WHILE REVIEWING HYD FLUID LOW ANNUNCIATOR ILLUMINATED CHECKLIST, THE HYDRAULIC PUMP TURNED ON AND RAN FOR 6 SECONDS. THE LDG GEAR RELAY CB POPPED AND THE PUMP SHUT OFF. CREW MANUALLY EXTENDED THE LANDING GEAR IAW THE MANUAL GEAR EXTENSION CHECKLIST SUCCESSFULLY. AIRCRAFT LANDED UNEVENTFULLY.
UPON COMPLETION OF LOW APPROACH ATTEMPTED TO RAISE GEAR. 3 GREEN DOWN AND LOCKED LIGHTS EXTINGUISHED, GEAR HANDLE WOULD NOT RAISE, AND LANDING GEAR RELAY CB (2AMP) POPPED. ACCOMPLISHED UNSAFE LANDING GEAR INDICATION CHECKLIST. SUCCESSFULLY EXTENDED THE GEAR MANUALLY IAW CHECKLIST. LANDING WAS UNEVENTFUL. SECURED HYD SERVICE VALVE, INSPECTED AND OPS CHECKED LANDING GEAR.
ON FINAL APPROACH KLN-88 RECEIVER FLICKERED AND THE SCREEN WENT BLANK. A STRONG ELECTRICAL ODOR PREVAILED THROUGH OUT THE CABIN. KLN-88 WAS TURNED OFF AND AIRCRAFT LANDED WITHOUT INCIDENT. APPARENT CAUSE WAS KLN-88 POEWR SUPPLY FAILURE. (M)
EN ROUTE AT FL200 RIGHT BLEED AIR FAIL ANNUNCIATOR LIGHT ILLUMINATED. EXECUTED EMERGENCY ROCEDUES AND LANDED AT NEAREST AIRPORT. FBO MAINTENANCE TROUBLESHOT SYSTEM AND FOUND HOLE IN RIGHT BLEED AIR SENSING LINE. SPLICE LINE AND OPS CHECK GOOD. (M)
CLIMBING THROUGH 17,000 FEET,SUDDEN ODOR AND SMOKE WERE OBSERVED IN CABIN/COCKPIT. IMMEDIATE DESCENT AND DIVERSION TO NEAREST SUITABLE AIRFIELD WAS BEGUN. SMOKE OBSERVED COMING FROM THE RIGHT ENGINE. SUSPECT OIL LEAK PAST CARBON SEAL. (M)
