DURING FLIGHT INSPECTION AT KSAV IN N77 ALL CREW MEMBERS ACKNOWLEDGED SIGNIFICANT EYE IRRITANT PRESENT. MYSELF AND MS HAD WATERING PRESENT IN EYES, SIC ACKNOWLEDGE EYES BURNING. I DECLARED AN EMERGENCY WITH SAV APPROACH AND WAS GIVEN AN IMMEDIATE CLEARANCE TO LAND RWY 28. ALL CREWMEMBER IN AIRCRAFT DONNED O2 MASKS AND GOGGLES. LANDING WAS UNEVENTFUL AND EXIT IF RUNWAY FOR EMERGENCY SHUTDOWN AND EVACUATION. CRASH RESCUE RESPONDED AND DISCONNECTED BATTERY AND CHECKED FOR HOT SPOTS. ALL CREW MEMBERS FLUSHED EYES WITH WATER PROVIDED BY CRASH RESCUE. COMPANY WAS CONTACTED BY AIRPORT MANAGEMENT TO TUG AIRCRAFT TO SHELTAIR FBO. NOTIFIED THE MX TUG OPERATOR BRAKES WERE ON AND ENTERED THE AIRPLANE TO RELEASE. REPORTED A BURNING ODOR.
CREW NOTICED FUEL STREAMING FROM THE RIGHT MAIN FUEL CAP. MAINTENANCE FOUND RIGHT HAND FUEL CAP LANYARD ON TOP OF FLANGE. LANYARD WAS REMOVED FROM FLANGE, CAP WAS SECURED DOWN INTO FILLER NECK. NO TREND.
HEARD POPPING NOISE IN HEADSETS, FOLLOWED BY TCAS FAILURE, FOLLOWED BY ELECTRICAL FUMES IN CABIN.
DURING FLIGHT, FUEL OBSERVED STREAMING FROM WING BEHIND LEFT GEAR.
RIGHT GENERATOR FAILED IN FLIGHT. RESET GENERATOR, FAILED AGAIN WITHIN 10 SECONDS.
RED CABIN DOOR MASTER WARNING ILLUMINATED IN FLIGHT. STAYED ON FOR DURATION OF FLIGHT. AFTER REPLACING A BROKEN SPRING TAB FOR THE CABIN DOOR SWITCH THE ISSUE WAS SOLVED.
RECEIVED A GEAR UNSAFE CONDITION, AROUND 300 AGL THE LANDING GEAR AUDIBLE WARNING WENT OFF, AND ATC VERIFIED THAT OUR GEAR WAS NOT DOWN.PERFORMED THE LANDING GEAR UNSAFE INDICATION ABNORMAL CHECKLIST. AT THAT POINT WE NOTICED THE LANDING GEAR RELAY WAS POPPED.COMPLETED THE LANDING GEAR MANUAL EXTENSION CHECKLIST, EXTENDED THE LANDING GEAR SUCCESSFULLY.DECLARED AN EMERGENCY.AND LANDED UNEVENTFULLY
DURING CRUISE FLIGHT, RT GENERATOR DROPPED OFF LINE, RESET NORMALLY, DROPPED OFF AGAIN ABOUT 10 MIN LATER.
ELECTRICAL SMELL & ENVIRO TEMP CONTROL CB POPPED. FOUND & REPAIRED THE POWER LEAD AT A/C RESET SWITCH IN THE NOSE GEAR WW, WITH NO FURTHER DISCREPANCIES NOTED.
DURING CLIMB NR 2 ENG REAR LATCH ALLOWING APPROX 1/2 INCH GAP TO APPEAR ON INSIDE OF COWLING. REAR LATCH CAM HOOK BROKEN ALLOWING LATCH TO BE ROTATED TO CLOSED POSITION WITHOUT PROPER ENGAGEMENT WITH LATCH PIN.
CABIN DOOR LIGHT CAME ON DURING FLIGHT. THIS IS A REPEAT. HOOK MICROSWITCH WAS ADJUSTED AFTER FIRST OCCURRENCE, THEN CHANGED AFTER SECOND OCCURRENCE.
STRONG ELECTRICAL FUMES NOTED IN COCKPIT ON ENGINE SHUTDOWN. COULD NOT DUPLICATE. SUSPECT BACK LIGHTING ON A/P CONTROLLER WENT OUT THAT COULD CREATE ELECTRICAL BURNED ODOR.
BURNED RUBBER SMELL IN COCKPIT. AMMETER SHOWS FULL DISCHARGE FOR A SECOND. AFIS WENT OFF LINE. BUS VOLTAGES CAME BACK UP. NO BREAKERS, DID NOT POP. MAINTENANCE APPLIED EXTERNAL POWER TO AIRCRAFT AND OPERATED AUX A/C, FWD AND AFT BLOWERS, AFIS POWER, AVIONICS POWER AND INVERTERS. RAN SYSTEMS FOR 1 HOUR WITH NO ABNORMAL INDICATIONS OR BURNING SMELL. RAN ENGINES WITH GEN POWER WITH AUX A/C, FWD AND AFT BLOWERS, A/C, AFIS POWER, AVIONICS POWER, AND INVERTERS FOR 45 MINUTES. NO ABNORMAL INDICATIONS OR BURNING SMELL NOTED. REMOVED LT & RT INBOARD LEADING EDGES, LT & RT INVERTER COVERS, INBOARD LT & RT NACELLE PANELS, NOSE AVIONICS ACCESS PANELS, CENTER CABIN FLOORBOARDS. INSPECTED ALL ACCESSIBLE WIRING, BUSBARS, CONTACTORS, RELAYS AND CURRENT LIMITERS AND CIRCUIT BREAKERS FOR LOOSE CHAFING OR ARCING. NO DISCREPANCIES FOUND. INSPECTED ALL ACCESSIBLE SYSTEMS AND FOUND NO ABNORMALITIES OR EVIDENCE OF BURNING OR SMOKE. ALL INDICATIONS ON AMMETER AND VOLTMETER WERE NORMAL. NO NEGATIVE DEFLECTIONS ON AMMETER NOTED. DEPOWER AIRCRAFT INSTALLED ALL REMOVED COVERS AND PANELS AND RESTORED AIRCRAFT TO NORMAL. AIRCRAFT FLEW 1.1 HOURS WITH NO BURNING SMELL. AIRCRAFT WAS AGAIN OPERATED WITH ENGINES RUNNING AND ALL SYSTEMS ON AND OPERATING FOR 2 HOURS WITH NO PROBLEMS. AFTERWARDS, AVIONICS REMOVED, INSPECTED AND RE-INSTALLED BOTH EFIS TUBES, AFIS CDU, PILOTS KAV-485, TCAS CONTROL, TRIMBLE CDU, TECH'S IC AND AUDIO MONITOR PANELS, LOWER AND INSPECTED OVERHEAD LIGHTING CONTROL PANEL. INSPECTED TECHS MOLEX CONNECTOR FOR FLOOR HEAT. FOUND NO EVIDENCE OF OVERHEATING OF COMPONENTS OR SMOKE SMELL. AIRCRAFT RETURNED TO SERVICE. NO DEFECT FOUND.
LEVELED AT FL260, EXECUTED CRUISE CHECKLIST AND RIGHT ENGINE CHIP DETECT LIGHT ILLUMINATED. RAN THE APPROPRIATE CHECKLIST AND RETURNED TO BASE. LANDING WAS UNEVENTFUL. CHECKED WITH MAINTENANCE FOUND A VERY SMALL, ALMOST MICROSCOPIC PIECE OF FOD ON THE CHIP DETECT SENSOR.
LOWER AFT WING ATTACH BOLT CRACKED AT DRIVE END. WING ATTACH BOLTS REMOVED FOR NDT AS PART OF ROUTINE INSPECTION OF WING ATTACH FITTINGS AND ATTACH BOLTS. LOWER AFT ATTACH BOLT WAS FOUND TO BE CRACKED AT THE HEAD END (SOCKET HEAD BOLT). BOLTS WERE REPLACED 09/25/03. BOLTS REPLACED EVERY 15 YEARS AND INSPECTED EVERY 5 YEARS. DATE OF LAST INSPECTION WAS 03/10/05.
RT OUTBOARD FLAP (INBOARD HINGE FITTING ON OUTBOARD SIDE) HAS BEARING WEAR. FLAP BEARING HAS CHAFED FLAP HINGE FITTING ALMOST THROUGH THE THICKNESS OF THE METAL. FLAP ROLLER BEARING ATTACH FITTING FAILURE WAS IMMINENT.
LT OUTBOARD FLAP (INBOARD HINGE FITTING ON OUTBOARD SIDE) HAS SEVERE CHAFING DAMAGE (CAUSED BY AFT ROLLER BEARING) NEARLY TO THE POINT OF FAILURE.
DEEP GOUGE FOUND ON UPPER BONDED FLOOR RIB OF RUDDER TRIM TAB PUSH/PULL ROD TUNNEL.
FLAP TRACK EXFOLIATION CORROSION: LT WING, INBOARD FLAP, INBOARD TRACK (P/N 101-120056-1; OUTBOARD FLAP, INBOARD (P/N 101-110017-1) AND OUTBOARD (P/N 101-110016-3). RT WING, INBOARD FLAP, INBOARD TRACK (P/N 101-120056-1); OUTBOARD FLAP, OUTBOARD TRACK (P/N 101-110016-3).
DENT IN SKIN (APPROX 1.5 INCH DIAMETER) AT A RIVET, DISCOVERED IN RT FUSELAGE SKIN BELOW CO-PILOT D WINDOW. CLOSER EXAMINATION REVEALED AN APPROXIMATE 0.2 INCH CRACK ON STRINGER 10 AT FS 134R, WL 100R THAT WILL CONSTITUTE ENGINEERING ORDER 09281 FOR A MAJOR REPAIR.
