AFTER TAKEOFF, CLIMBING THROUGH 10,000 FEET WE RECEIVED A RED MASTER CAUTION LIGHT, AND A RED ILLUMINATED CABIN ALT LIGHT. WE RAN THE CHECKLIST. DISPATCH ADVISED US TO RETURN TO PBI. ATC WAS NOTIFIED AND NO EMERGENCY WAS DECLARED.
ANTI-SKID LIGHT ILLUMINATED DURING FLIGHT. MAINTENANCE HAS FOUND LANDING GEAR HARNESS DAMAGED FROM BEING MISROUTED AND RUBBING ON TIRE.
#2 ENG SLOWLY ROLLED BACK ON DESCENT UNTIL NIL, COULD NOT RESTART. T/S AND REPLACED FDU. OPS CHECK SATISFACTORY.
AS THE THROTTLES WERE DECREASED, THE RIGHT ENGINE N1 CONTINUED DECREASING AT A STEADY SLOW RATE UNTIL CUTOFF.REMOVED AND REPLACED THE RIGHT FUEL DELIVERY UNIT. OPS CK LEAK CHECK GOOD REF F544-31 MM 73-19-01
AIRCRAFT WAS FLYING TOUCH-AND-GO PATTERN WORK WHEN THE CREW NOTICED BUFFETING AND LOST THEIR AUX HYD SYSTEM. AFTER LANDING, DISCOVERED NR 6 SPOILER ACTUATOR UPPER MOUNT BOLT HAD SHEARED AND THE ACTUATOR DROPPED DOWN ONTO THE FOREFLAP. AFTER FLAPS WERE LOWERED AND RE-RAISED, THE ACTUATOR PUNCTURED THE FOREFLAP AND BOTH ACTUATOAR HYD B-NUTS WERE SNAPPED OFF CAUSING AUX HYD SYS FAILURE. DISCOVERED THE UPPER ACTUATOR ATTACH BOLT FITTING BUSHINGS WERE NOT INSTALLED AT SPOILER OVERHAUL CAUSING EXCESSIVE PLAY ON THE BOLT LEADING TO ITS FAILURE.
WHILE PERFORMING A C3 CHECK AND THE A.A.C.I.P. SB 3173, FOUND NR 2 AND NR 3 OVERWING FITTING CRACKED FORWARD OF FUSELAGE ATTACHMENT BOLT. INSTALLED NEW TYPE FITTING: NR 1, PYLON AT: ATT 51,029/ATC 20,325, FEB 91. NR 2 AND NR 3, ATT 52,324/ATC 20,629 MAY 95. NR 4, REPLACED, PREVENTIVE ONLY, MAY 95.
DURING A C-CHECK, THE RT WING FORWARD SPAR UPPER CHORD BETWEEN FSS 360 AND FSS 586 FOUND WITH CORROSION AROUND HI-LOCKS. PART REPLACED. ACFT TT 52,324.7 HOURS. ACFT TC 20,628 CYCLES.
DURING A C-CHECK, RT WING FORWARD SPAR LOWER CHORD, FSS 387-640, FOUND WITH CORROSION AROUND HI-LOCKS. PART REPLACED. ACFT TT 52,324.7 HOURS. ACFT TC 20,628 CYCLES.
PERFORMING A FLEET COMPANY INSPECTION, THE LT FILLET FLAP TRANSMISSION SUPPORT FITTING WAS FOUND CRACKED. SUPPORT FITTING IN THE PROCESS TO BE REPLACED IAW BOEING SIB 3251, REV. 2.
