LT & RT AILERON INBD AND OUTBOARD PCU FITTINGS CRACKED. LT AND RIGHT AILERONS WERE REPLACED.
ENGINE FUEL FILTERS ON LT & RT DIFFERENTIAL PRESSURE ACROSS LT FUEL FILTER APPROACHES BYPASS CONDITION. STATUS ENGINE FUEL FILTER DISPLAYED AFTER TAKE-OFF. REMAINED ON FOR APPROXIMATELY 30 SECONDS. FUEL CONTAMINATION CHECKS WERE MADE AT FUELING STATION, NO CONTAMINATION. BOTH FUEL FILTERS WERE REPLACED, SOME DEBRIS FOUND, BUT BELIEVED INSUFFICIENT TO CAUSE BYPASS. ONE FILTER WAS FLOW CHECKED. MAX DELTA-P ACROSS THE FILTER WAS 3 PSID; INSUFFICIENT TO TRIP THE DELTA-P SWITCH. BOTH DELTA-P SWITCHES WERE CHECKED. CLOSED AND OPENED WITHIN SPEC. SWITCHES WERE DISASSEMBLED - NO ANAMOLIES FOUND.
1ST OFFICER'S FWD OUTER WINDSHIELD FAILED. PRESSURIZATION MAINTAINED AT ALL TIMES. THE OUTER LAYER OF GLASS SHATTERED DUE TO ARCING AT THE UPPER BUS BAR FOR THE WINDOW HEAT SYSTEM. THE CAUSE OF THE ARCING WAS DUE TO MOISTURE INGRESS INTO THE ELECTRICAL HEAT SYSTEM BUS BAR AT THE AREA WHERE THE HEAT FILM ATTACHES TO THE BUS BAR. CONTRIBUTING FACTORS OF POOR ADHESION OF THE MOISTURE SEAL AND DELAMINATION IN AND AROUND THE UPPER BUS BAR AREA ALLOWED THE WATER TO PENETRATE INTO THE AREA. REMOVED AND REPLACED THE WINDOW ASSY PER AMM 56-11-01-401.
HAIL DAMAGE DENTED. SUPPLEMENTAL DATA UPDATE MAJOR REPAIR ACCOMPLISHED.

CREW REPORTS LOUD NOISE AND VIBRATION ON RIGHT SIDE OF AIRCRAFT BY ROW 15. *S/D* DUCTS IN AFT OF FORWARD CARGO COMPARTMENT (MIX MANIFOLD AREA) WERE CHECKED, BUT NO DISCREPANCIES WERE FOUND. AIR ON TEST WAS PERFORMED ON RIGHT HAND PACK, AND THERE WAS NO EICAS MESSAGE INDICTING THE RIGHT PACK. AIRCRAFT WENT BACK TO REVENUE SERVICE WITH NO FURTHER INCIDENT.
POOR AIR QUALITY IN THE AIRCRAFT. *S/D* PROBLEM COULD NOT BE DUPLICATED ON GROUND. HOWEVER, WHEN CHECKING LT CONDENSER/REHEATER, IT WAS FOUND CLOGGED. A CLOGGED CONDENSER/REHEATER CAN CAUSE PACK LOW FLOW PROBLEM WHICH MAY CONTRIBUTE TO POOR AIR QUALITY. THE LT CONDENSER/REHEATER WAS REPLACED. AIRCRAFT WENT BACK TO REVENUE SERVICE WITHOUT FURTHER POOR AIR QUALITY PROBLEM.
MAJOR REPAIR REPORTING: DURING ROUTINE MAINTENANCE A CRACK WAS FOUND IN ONE OF THE MID CHORD ANGLES (P/N:141W8519-1) ON THE MAIN DECK FLOORBEAM AT STATION 246, LBL 38.5. ACCOMPLISHED REPAIR PER BOEING STRUCTURAL REPAIR MANUAL 53-10-51, FIGURE 201, REPAIR TYPE 1.
LEFT PACK COMPARTMENT DOOR DEPARTED AIRCRAFT IN FLIGHT. AIRCRAFT RETURNED TO IAD. *S/D* LEFTHAND PACK DOOR WAS FOUND MISSING. IT APPEARED THAT AS PORTIONS OF THE DOOR BROKE LOOSE, THEY HIT THE PARTS INSIDE LEFTHAND PACK BAY LEAVING PUNCTURES AND DENTS ON SEVERAL COMPONENTS (CONDENSER/REHEATER, FAN AIR DIFFUSER HOUSING, AND OTHER DUCTING). PARTS REPLACED INCLUDE: ALL DAMAGED PARTS IN LT PACK BAY, LT PACK BAY DOOR (P/N 149W2300-1), AND FITTINGS (P/N 145W2401-3 & 145W2403-3). INCIDENT IS UNDER INVESTIGATION.

EN ROUTE CDG-IAD, LOST RIGHT HIGH FREQUENCY RADIO. DIVERTED TO LHR. *S/D* FAULT, INTERNAL TO THE HIGH FREQUENCY ANTENNA COUPLER (P/N: 792-6140-001) CAUSED THE COUPLER TO RECEIVE BUT NOT TRANSMIT. UNIT WAS REPLACED AND FULL SYSTEM FUNCTIONALITY WAS RESTORED. TESTING VERIFIED REPORT OF RECEIVE BUT NOT TRANSMIT FUNCTION AND ADDITIONAL UNIT ANALYSIS IS SCHEDULED.
PACK MODE L LEFT PACK AIR CYCLE MACHINE HAD FAILED MESSAGE RECEIVED AND FLIGHT CREW NOTICED SMOKE IN COCKPIT AT TOP OF DESCENT. *S/D* PACK MODEL MESSAGE WAS DUE TO AIR CYCLE MACHINE (P/N 810209-3) SEIZING. HAMILTON STANDARD AND BOEING ARE WORKING TO IMPROVE THE ACM RELIABILITY. ACM TEARDOWN SHOWED THAT ACM FAILURE WAS NOT RELATED TO THE SMOKE INCIDENT.
APU GENERATOR DROPPED OFF LINE AT DEPARTURE. FOUND GENERATOR FEEDER CABLE DAMAGED FROM TERMINAL LUG. *S/D* THE CAUSE OF THE APU GENERATOR DROPPING OFF LINE WAS DAMAGE TO THE PHASE C WIRE (W7860-0003-3/0). THE WIRE WAS REPLACED AND THE APU GENERATOR SYSTEM OPERATIONAL CHECKED. THE CAUSE OF THE OVERHEAT ON THE TERMINAL COULD NOT BE PRECISELY DETERMINED. CAUSES COULD INCLUDE TERMINATION METHOD FOR THE WIRE ASSEMBLY OR METHOD OF HARDWARE INSTALLATION AT THE TERMINAL.
APU WILL NOT START IN EITHER ELECTRICAL OR PNEUMATIC START MODE. *S/D* REMOVED AND REPLACED APU BATTERY CHARGER. PROBLEM PERSISTED. FOUND APU STARTER DAMAGED. REPLACED ELECTRIC STARTER (P/N 2704442-4) AND CLUTCH ASSEMBLY. OPERATIONALLY CHECKED NORMAL. SUSPECT CAUSE RELATED TO BOEING SERVICE RELATED PROBLEM (SRP) 777-SRP-49-0028 AND SERVICE BULLETIN 777-24-0024.
DURING FLAT FIRING OF SLIDE RAFT (P/N 62772-202) (S/N 0021) IN CONJUNCTION WITH SHOP OVERHAUL VISIT, A ONE INCH TEAR DEVELOPED IN THE LOWER TUBE UNDER THE GIRT. UNIT SENT TO VENDOR FOR REPAIR AND EVALUATION. *S/D* AFTER REVIEW BY VENDOR, CAUSE OF FAILURE COULD NOT BE PRECISELY DETERMINED.

WITH A/P ENGAGED AND SPEED BRAKE EXTENDED TO FULL UP POS. ON DESCENT PLANE TRIED TO NOSE DIVE UP TO 10 DEG NOSE DOWN. DESCENT RATE WENT TO 5880 FPM. DISENGAGED A/P WORKED FINE. TRIED A 2ND TIME WITH A/P ENGAGED SAME PROBLEM. WITH A/P DISENGAGED USED SPEED BRAKE TWICE WITH NO NOSE DOWN EXCESS.
CENTER HYDRAULIC SYSTEM FLUID PRESSURE LOSS AFTER TAKEOFF. AIRCRAFT CONTINUED TO IAD, LANDED UNDER AMBER ALERT. *S/D* CAUSE ATTRIBUTED TO LEAK AT THE LEFTHAND MAIN LANDING GEAR ACTUATOR PRESSURE HOSE CAUSED BY CHAFING AGAINST HOSE GUIDE. INSTALLED NEW HOSE WITH A CHAFE GUARD FOR ADDITIONAL PROTECTION. FLEET WIDE CHECK ACCOMPLISHED.

BACKUP ELECTRICAL SYSTEM EICAS MESSAGE DISPLAYED, INDICATING FAILURE OF BACKUP ELECTRICAL SYSTEM. BACKUP CONVERTER AND LEFTHAND BACKUP GENERATOR WERE REPLACED. VENDOR CONFIRMED FAILURE OF THE PMG STATOR WINDINGS IN THE BACKUP GENERATOR. ALSO, THE BACKUP CONVERTER WAS FOUND TO BE DEFECTIVE.
