FOUND S32020 NLG DOOR CLOSE SWITCH WOULD CLOSE NOSE DOORS WITHOUT S32018 NLG ARM SWITCH BEING ACTIVATED TO ARM. UPON INSPECTION OF SWITCH S32020 AND S32018 WE FOUND THAT THE WIRE 1007-18 AND 1006-18 ARE IN THE WRONG POSITION REF WDM 32-35-12-1. WIRE 1007-18 WAS FOUND AT THE #3 ON S32020 CAUSING THE NOSE LANDING GEAR DOOR UNSAFE LIGHT TO REMAIN ILLUMINATED WITH THE SWITCH IN THE OFF POSITION FOUND WIRE 1006-18 IN THE #1 POSITION ON S32020. BECAUSE THIS WIRE WAS IN THE WRONG LOCATION, THIS ALLOWED POWER TO FLOW IN REVERSE CLOSING THE MAIN LANDING GEAR DOOR WHEN S32020 IS MOVED TO THE CLOSED POSITION. WIRES HAVE BEEN RE-ROUTED TO CORRECT POSITIONS REF WDM 32-35-12-1. OPS CHECK NEEDED REVIEW LP UPDATES. ESSENTIALLY, SWITCHES WERE WIRED INCORRECTLY CAUSING THE WRONG LANDING GEAR DOORS TO RETRACT UNCONTROLLABLY. THIS COULD HAVE CAUSED SERIOUS INJURY TO PERSONNEL AND DAMAGE TO EQUIPMENT AND AIRCRAFT. OPS CHECKED GOOD.

[08-14-07] TEARDOWN REVEALED THAT 4 LP TURBINE TRANSITION DUCTS (PW IPC 72-30-00-002-250, P/N 50M919) HAD EXPERIENCED EXTREME INNER FOOT WEAR. THIS CAUSED A LOSS OF RADIAL RETENTION AT THE FRONT OF THE DUCT, ALLOWING DUCTS TO DEFLECT INWARDS INTO PRIMARY GASPATH, AND DISTORTING GASPATH FLOW INTO THE LP TURBINE, RESULTING IN SURGE. UA HAS SEEN FOOT WEAR AT TEARDOWN, BUT NEVER TO THIS EXTENT. PARTS HAVE BEEN RETURNED TO MANUFACTURER & MTU FOR FURTHER ANALYSIS.

THIS SB WAS INTRODUCED TO PREVENT SPINNING OF THE SPUR GEAR ON THE OUTER RACE. FAILURE OF THE INNER RACE ALLOWED THE REMAINDER OF THE BEARING AND SPUR GEAR TO SPIN ON THE IDLER SHAFT. THEREFORE, DRIVE TORQUE TO THE DE-OILER WAS LOST. THIS RESULTED IN OIL EXITING THE ENGINE THROUGH THE OVERBOARD BREATHER PORT.

WIRE WAS DAMAGED ON THE PLUG SIDE. EVIDENCE OF WATER ON THE CONNECTOR AND OTHER COMPONENTS MOUNTED ON THE OVERDOOR PANEL. POSSIBLE WATER DRIPPING ON THE CONNECTOR CAUSED THE SHORT. DOES NOT APPEAR TO HAVE BEEN ANY FAILURE OF THE CONNECTOR THAT WOULD HAVE ALLOWED THE 2 PINS TO MAKE CONTACT. THE CONNECTOR WAS REPLACED WITH AN ENVIRONMENTAL PLUG THAT RESISTS MOISTURE.

READING LIGHTS AT 28AB WOULD NOT TURN OFF, SMOKE EMITTED FROM PASSENGER SERVICE UNIT PANEL. POWER TURNED OFF FROM CACP. *S/D* BOTH LAMPS WERE REMOVED AND REPLACED. AREA WAS INSPECTED FOR SIGNS OF OVERHEAT OR BURNING, NONE FOUND. ALL OPERATIONS RETURNED TO NORMAL AFTER LAMP REPLACEMENT. THERE HAVE BEEN NO REOCCURRENCES OF THIS ANOMALY.

AIRCRAFT RETURN TO ORD (ORD-LHR). *S/D* SUSPECT NO FLOW INDICATION ON SYNOPTIC PAGE RESULT OF STICKING FLOW CONTROL VALVE (PN 810204-2) PROXIMITY SWITCH (PN 808577-2). ALSO, REPLACED CABIN TEMPERATURE CONTROLLER PRECAUTIONARY.
LEFT PACK TURNED OFF DUE TO VIBRATION AT DOOR 2L/2R. *S/D* FOUND LEFT PACK RAM AIR DOOR TORQUE TUBE SHAFT (PN 213W3130-3) SHEARED AND INLET SIDEWALL DAMAGED AROUND INBOARD BEARING ATTACH OPENING. REPLACED TORQUE TUBE AND INBOARD AND OUTBOARD CAP ASSEMBLIES. FLEET SURVEY REVEALED ADDITIONAL CASES OF WEAR. BOEING DEVELOPING AN IMPROVED DESIGN.
DOOR 2 LEFT SLIDE UNRAVELED, DOOR ASSIST BOTTLE BLOWN. *S/D* THE SLIDE GIRT MATERIAL WAS DISLODGED FROM ITS RETAINING TRAY FORCING THE MODE SELECT HANDLE TO THE DOOR ARM POSITION UPON DOOR OPENING. INSTALLED NEW SLIDE AND CHECK FOR PROPER STOWAGE OF GIRT MATERIAL.
REPORT OF SMOKE IN LAVATORY AT DOOR 4 RIGHT SIDE. ADDITIONAL, REPORT OF FIRE IN LAVATORY TRASH BIN. ALARM DID NOT SOUND. WASTE COMPARTMENT EXTINGUISHER DID NOT DISCHARGE.
