WHILE PERFORMING A VISUAL INSPECTION OF THE CANT BULKHEAD FOR CRACKS PER AD 2024-20-02 FOUND CANTED BULKHEAD STA 942,225 LOWER CAP CORROSION. LOG PAGE 75814 NON-ROUTINE# 42 COMPLIED WITH FLIGHT VEHICLES CONSULTING INC (FVCI) EO 24201-1 REVSION B DATED 12/9/2024 APPROVED ON 8110-3. THE CORROSION WAS CUT OUT AND FILLERS AND DOUBLERS INSTALLED.
ON CLIMB OUT KDRT, AIRCRAFT BEGAN ALTERNATING LEFT/RIGHT YAW EVENTS WHICH CONTINUED THROUGHOUT FLIGHT WITH AUTOPILOT ON OR OFF. UNSCHEDULED LANDING KSAT. CORRECTIVE ACTION REMOVED AND REPLACED STAB AUG. COMPUTER AND PERFORMED OPS CHECK IAW DC9 AMM 22-11-1. OPS CHECK GOOD.
ON CLIMB OUT KDRT, AIRCRAFT BEGAN ALTERNATING LEFT/RIGHT YAW EVENTS WHICH CONTINUED THROUGHOUT FLIGHT WITH AUTOPILOT ON OR OFF. UNSCHEDULED LANDING KSAT. CORRECTIVE ACTION REMOVED AND REPLACED STAB AUG COMPUTER AND PERFORMED OPS CHECKS IAW DC9 AMM 22-11-1. OPS CHECKS GOOD.
AFTER TAKE OFF, THE PILOT SELECTED GEAR UP WITH NO RESULTS. GEAR DOORS OPENED BUT THE GEAR DID NOT RETRACT. AFTER CYCLING THE GEAR HANDLE SEVERAL TIMES, IT WAS DISCOVERED THAT THE GEAR WOULD STILL NOT RETRACT AND THAT THE GEAR DOORS WOULD NOT CLOSE. THE PILOT DECIDED TO DIVERT. FLEW BY THE TOWER TO CONFIRM THAT THE GEAR WAS DOWN AND THAT THE GEAR DOORS WERE STILL OPEN WHICH THE TOWER CONFIRMED. DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. NOTED AT AIRCRAFT TOUCHDOWN THAT THE MAIN GEAR DOORS CLOSED. DISCOVERED THAT THE LANDING GEAR CONTROL VALVE WAS SLIGHTLY OUT OF ADJUSTMENT.
NOSE LANDING GEAR GREEN LIGHT ON AFTER GEAR RETRACTION AND UPLOCK CHECK UNSATISFACTORY.
LEFT MAIN LANDING GEAR GREEN INDICATION (DOWN AND LOCKED) DID NOT ILLUMINATE, BULBS TESTED OK.
DURING CLIMB AT 6000FT MSL PITCH TRIM AURAL WARNING SOUNDED, STABILIZER OBSERVED UNCOMMANDED NOSE DOWN TRIM RUNAWAY. PRIMARY TRIM REVERSED RUNAWAY PROPERLY. COMPLETED RUNAWAY STABILIZER EMERGENCY PROCEDURE. PROBLEM STABLE AFTER CIRCUIT BREAKERS D-6, D-7, D-8 PULLED.
RIGHT WING TRAILING FUEL AFTER TAKEOFF, LEFT WING FUEL LEVEL DROPPED 1000 POUNDS.
CRACK OF UNKNOWN LENGTH AND DIRECTION IN DOUBLER UPPER LEFT AFT PRESSURE BULKHEAD DOOR CUTOUT CORNER. STOP DRILLED CRACK IN -13 DOUBLER, INSTALLED REPAIR IAW DC9 SRM J060088-543 AND DC9 SB 53-137.
AFT PRESSURE BULKHEAD SKIN CRACKED AT UPPER LEFT DOOR CUTOUT. REMOVED AND REPLACED SKIN IAW DC9 SRM 51-30-2 AND 51-30-5.
X-RAY INSPECTION ON LEFT ELEVATOR REVEALED CRACKS AT STATIONS: XE 74(.750), XE 191(.600), XE 195(.750), XE 199 (.750), AND XE 219 (.400). REPAIRED CRACKS AT STATIONS: XE 191, XE 195, XE 199 AND XE 219. NO CRACKS DETECTED AT XE 74. REPAIRED IAW DC9 SRM 55-02.
AFT PRESSURE BULKHEAD DOOR CUTOUT UPPER RIGHT CORNER DOUBLER HAD APPROXIMATELY 1.5" RADIUS CRACK. REPAIRED UPPER RIGHT CORNER OF AFT PRESSURE BULKHEAD IAW DC9 SRM J060088-543 REPAIR AND DC9 SB 53-137.
RIGHT AILERON OUTBOARD TAB DELAMINATED 3 PLACES, 1 UPPER AND 2 LOWER APPROXIMATELY 3 BY 3, 3 BY1 1/2, OUTBOARD TIP REPAIR DISBOND. REMOVED AND REPLACED RIGHT AILERON TRIM TAB IAW DC9 MM 27-10-5.
FOUND LEFT MAIN LANDING GEAR ACTUATOR ROD END PULLED LOOSE FROM THE ACTUATOR.
LEFT LANDING GEAR LIGHT REMAINED ILLUMINATED IN THE BRIGHT MODE AFTER TO WITH THE LANDING GEAR IN THE UP POSITION AFTER THE UP-LATCH CK. GEAR WARNING HORN DID NOT ACTIVATE OR SOUND WITH THROTTLES AT IDLE AND GEAR INDICATING UP.
FORWARD CARGO COMPARTMENT, DOOR JAMB, STUB BEAM SHORT FLANGE IS CRACKED.

SUBJECT HORIZONTAL STABILIZER ACTUATOR: RECEIVED WITH A CRACKED NUT WITH METAL PARTICLES ON SCREW SECTION AD 200-15-15.
FOUND DURING 4C CHECK, AFT CARGO COMPARTMENT BARRIER DOOR, DAMAGED INNER AND OUTER SKIN AND HONEYCOMB CORE. DAMAGED PARTS WERE REPLACED BY NEW ONE. (M)
