AIRCRAFT SMOKY WHEN IN FULL HOT. DRAINED 0IL OUT OF COOLING TURBINE REF SA227 MM CH 21-50-00
CREW MEMBER NOTICED THE LEFT BRAKE HAD LOST PRESSURE ON TAXI AND WAS WORSENING. AFTER NOTICING THE BRAKE WAS LOSING PRESSURE ANOTHER AIRCRAFT NOTIFIED THE PILOT ON TOWER FREQUENCY THAT THE WHEEL WAS SMOKING. TOWER CONFIRMED THE CONDITION AND PILOT LET THE TOWER KNOW THAT AIRCRAFT WAS GOING TO EXIT RUNWAY. FIRE CREW HOSED DOWN LEFT MAIN GEAR AND ENGINES WERE SHUT DOWN. MX REMOVED AND REPLACED #1 & #2 MAIN BRAKE AND WHEEL ASSEMBLY WITH SERVICEABLE BRAKE AND ASSEMBLY. OPS CHECK GOOD.
PILOT REPORTED LOSS OF HYDRAULIC PRESSURE. MX FOUND FAILED FLAP UPLINE ASSEMBLY AND REPLACED.
PILOT REPORTED SMOKE POURING OUT FROM AROUND LEFT AUDIO PANEL. MX REMOVED AND REPLACED PILOTS INSTRUMENT PANEL POTENTIOMETER WITH NEW. OPS CHECK GOOD.
FLIGHT CREW REPORTED THAT SMOKE FILLED THE CABIN APPROXIMATELY 30 MINUTES AFTER TAKEOFF. PILOT PERFORMED CHECKLIST PROCEDURES AND WAS ABLE TO STOP THE SMOKE. AFTER LANDING, THE SOURCE OF THE SMOKE WAS INVESTIGATED AND FOUND TO HAVE ACTUALLY BEEN A FINE MIST OF HYDRAULIC FLUID COMING FROM A CRACKED LANDING GEAR DOWN LINE RUNNING THROUGH THE COCKPIT. THE CREW ALSO NOTED AN ACCUMULATION OF HYDRAULIC FLUID UNDER THE LT RUDDER PEDALS. MX REPLACED THE FAULTY LINE AND OPERATIONALLY CHECKED THE SYSTEM, NOTING NO FURTHER ANOMALIES.
FLIGHT CREW REPORTED UNCOMMANDED FLAP MOVEMENT ON SHORT FINAL. LANDING WAS UNEVENTFUL. MX FOUND FAULTY FLAP COMMAND ASSEMBLY AND REPLACED.
PILOT REPORTED THAT THE STABILIZER TRIM FAILED TO FUNCTION 10-15 MINUTES AFTER TAKEOFF. PILOT RETURNED TO DEPARTURE AIRPORT AND LANDED WITHOUT INCIDENT. PILOT REPORTED THAT THE ACTUATOR FUNCTIONED NORMALLY ON THE GROUND AFTER LANDING. MAINTENANCE INVESTIGATED AND FOUND NO SYSTEM ANOMOLIES AND THAT THE PITCH TRIM ACTUATOR WAS WORKING NORMALLY ON THE GROUND. THE ACTUATOR WAS REPLACED AS A PRECAUTIONARY MEASURE.
CREW REPORTED LT MLG, SLOW TO LOCK DOWN, RECYCLED LG, LOCKED DOWN AFTER RECYCLE. MX FOUND INBD DOWNLOCK SWITCH OUT OF ADJUSTMENT. CLEANED, LUBED & ADJUSTED. GND TESTED GOOD.
RAMP PERSONNEL OBSERVED THAT THE NR 3 AND 4 BRAKES WERE SMOKING/ON FIRE AS ACFT TAXIED INTO RAMP. GROUND PERSONNEL EXTINGUISHED FIRE IAW ESTABLISHED PROCEDURES. MX BELIEVES BRAKES OVERHEATED DUE TO PILOT "RIDING" BRAKES DURING TAXI.
LOCATED AND MARKED END OF CRACKS AT WS 81.174 ON RIGHT AND LEFT WINGS IN THE LOWER RADIOUS OF THE WING RIBS. BOTH CRACKS WERE APPROX. 6 INCHES LONG AND FOLLOWED THE RADIUS FORWARD AND AFT.
PILOT OBSERVED OIL AND EGT FLUCTUATIONS AND A LOW OIL PRESSURE LIGHT. DID A PRECAUTIONARY ENGINE SHUTDOWN WITHOUT FURTHER PROBLEMS. MECHANIC FOUND NO CAUSE, DID SOME TROUBLESHOOTING AND AN INSPECTION. FOUND SMALL OIL LEAK FROM PROP SEAL BUT LEAK WAS WITHIN NORMAL LIMITS. GROUND RUN ENGINE WITHOUT DUPLICATING THE INDICATION PROBLEM, SO THEY RELEASED IT TO SERVICE.
DURING TAKEOFF AT SJU, THE AMBER NOSE WHEEL STEERING FAIL LIGHT ILLUMINATED AND THE PILOT ABORTED THE TAKEOFF. MAINTENANCE REMOVED AND REPLACED THE NOSE WHEEL STEERING PRESSURE SWITCH WITH A NEW UNIT. THE OPERATIONS CHECK WAS GOOD AND THE AIRCRAFT WAS RETURNED TO SERVICE. (M)
DURING APPROACH TO SJU, SMOKE WAS NOTICED IN THE COCKPIT ABOUT 15 MINUTES AFTER THE HEATER WAS TURNED ON. THERE WAS ALSO A GREATER DEAL OF SMOKE COLLECTION IN THE AFT CABIN. AN EMERGENCY WAS DECLARED AN A FEW MINUTES LATER, THE LEFT WING OVERHEAT LIGHT ILLUMINATED. DURING APPROACH TO THE RUNWAY, THE WING OVERHEAT LIGHT WENT OUT AND THE AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AND DISCOVERED THAT THE COOLING TURBINE HAD SEIZED. THE COOLING TURBINE WAS DEFERRED AND THE AIRCRAFT WAS RETURNED TO SERVICE. (M)
SVMI-TJSF - FLT 9800 - ABOUT ONE HOUR OUT IN CRUISE THE RIGHT ENGINE FIRE LIGHT CAME ON. PERFORMED SHUTDOWN AND DISCHARGED FIRE BOTTLE. AIRCRAFT RETURNED TO SVMI AND LANDED WITHOUT FURTHER INCIDENT. MAINTENANCE INSPECTED AIRCRAFT AND FOUND NO EVIDENCE OF FIRE. MAINTENANCE FOUND A DEFECTIVE 600 DEGREE DETECTOR. REMOVED AND REPLACED DETECTOR, INSTALLED NEW FIRE BOTTLE. PERFORMED FUNCTIONAL CHECK OF THE FIRE DETECTOR SYSTEM, NO DEFECT NOTED. AIRCRAFT RETURNED TO SERVICE.
NOTED CORROSION AT AIRCRAFT WINDOW OPENING AREAS AND SKIN SEAMS. SUSPECT AREAS, AND ALL X-RAY(S) SHOT TO ENSURE CORROSION DID NOT PENETRATE STRINGER AND SKIN MATING SURFACES. DETERMINED TO BE ONLY SURFACE CORROSION AND TREATED AREAS. WILL MONITOR THIS CONDITION. SUSPECT CAUSE IS FROM PAINT STRIPPING OPERATIONS.
