WITH NO DEFECT NOTED. NO SIGNS OF OIL CONTAMINATION NOTED. DURING OUR FLT DECK CHECKS, WITH APU BLEED ON, TRIED MULTIPLE COMBINATIONS OF PACKS, VALVES, FANS ON & OFF. MANUFACTURED SOLID SHIM AND REINSTALL. SHIM MIGRATION TYPICALLY CAUSED BY IMPROPER CONTACT SURFACE MATCH OR INADEQUATE TENSION ON FASTENERS.

NR 1 ENGINE UPPER IDG TERMINAL BLOCK HAS MELTED AREAS AND MISSING SHRINK WRAP ON WIRES. THE IDG STRUT DISCONNECT TERMINAL BLOCK INSPECTION/TORQUE CHECK ACCOMPLISHED AT THE SEGMENTED 'A' DISCOVERED THE OVERHEATED TERMINAL BLOCK. REVIEW OF IDG WRITE-UPS, LOGBOOK, AND MAINTENANCE MESSAGES PRIOR TO DISCREPANCY DID NOT INDICATE ABNORMAL IDG OPERATING CONDITIONS OR IN-SERVICE DIFFICULTIES. PHASE 'B' INITIALLY OVERHEATED POSSIBLY DUE TO IMPROPER TORQUE CAUSING THE CABLE LUGS TO LOOSEN AND OVERHEAT WITH HIGH RESISTANCE.
DOOR 2L WILL NOT DISARM. ONE OF THE CASTINGS THAT LIFT THE SLIDE GIRT BAR WHEN ACTUATED BY THE DOOR ARM/DISARM LEVER, WAS FOUND BROKEN, PREVENTING THE SLIDE GIRT BAR FROM BEING LIFTED FREE OF THE FLOOR GIRT FITTING. THE FITTING WAS REPLACED AND THE GIRT BAR OPERATION WAS CHECKED. UAL HAS AN AMM MAINTENANCE TIP TO ADVISE TO CHECK FOR THIS CONDITION SHOULD THE DOOR BE HARD TO DISARM.

AIRCRAFT HAD FLAP DRIVE FAILURE INVOLVING PRIMARY AND SECONDARY SYSTEMS. CENTER ACE WAS REPLACED AS PRECAUTIONARY. DETAILED TESTS OF FLAP PRI CONTROL VALVE AND FLAP SECODARY MODES PASSED WITH NO FAULTS.
UPON ARRIVAL AT DEN, HOT BRAKES, SMALL FLAMES AND SOME SMOKE REPORTED AT OUTBOARD WHEELS ON RIGHT MAIN LANDING GEAR. FLAMES SELF-EXTINGUISHED WITH NO DAMAGE TO AIRCRAFT. *S/D* UNABLE TO DETERMINE ANY DEFINITIVE CAUSE. LT AND RT MAIN LANDING GEAR WHEEL AND BRAKES WERE INSPECTED PER 777 AMM 05-51-14, WITH NO EVIDENCE OF DAMAGE FOUND.
BURNING SMELL IN GALLEY AREA. CIRCUIT BREAKERS PULLED AND SMELL SUBSIDED. *S/D* PLASTIC FOOD WRAP STUCK IN BACK OF OVEN CAUSING SMELL. REPLACED OVEN.
SHORTLY AFTER DEPARTING ORD EN ROUTE LHR, CREW RECEIVED STATUS MESSAGE ON BLEED LOOP NR 1. SUBSEQUENTLY MESSAGE RECEIVED FOR NR 2 BLEED LOOP. AIRCRAFT DIVERTED TO IAD. *S/D* DUCT CLAMP ON RIGHTHAND AC PACK LOWER FLOW CONTROL VALVE (FCV) BROKE, TRIGGERING A BLEED LEAK DETECTION BY THE DUCT LEAK OVERHEAT DETECTION (DLOD) SYSTEM AND ISOLATION OF THE RT BLEED SYSTEM. REPLACED BROKEN CLAMP ON FLOW CONTROL VALVE.
EN ROUTE LHR-IAD, ELECTRICAL ODOR REPORTED IN FLIGHT DECK AND MID GALLEY AREA. AIRCRAFT RETURNED TO LHR. *S/D* INSULATION, (HEAT SHRINK), AT BASE OF LAMPS OVERHEATED. THE OVERHEAT CONDITION IS A KNOWN PROBLEM AND BOEING IS PREPARING A SERVICE BULLETIN AND SOFTWARE TO CORRECT IT. THE CAUSE OF THE OVERHEAT IS THE RAMPING UP OF FILAMENT HEATER CURRENT WHEN BOTH LAMPS HAVE FAILED AND THE UNIT IS IN THE DIM MODE. THE SOFTWARE WILL PREVENT THE CACP/CSCP (P/N'S 285W0012-3/285W0129-3) FROM GOING INTO DIM MODE. IT WILL TURN OFF THE HEATER CURRENT TO THE CACP AND WILL SIMULATE DIM MODE ON THE CSCP.
KEROSENE SMELL REPORTED ON GROUND AND ONCE IN CRUISE AROUND DOOR 2L. FLIGHT EN ROUTE EWR-LHR AND DIVERTED TO YQX. *S/D* ACCOMPLISHED IDLE AND HIGH POWER ENGINE RUNS. INSPECTED AND SEARCHED CARGO PITS WITH AIRCRAFT PRESSURIZED AND UNPRESSURIZED, NOTHING FOUND. CHECKED FOR BOTH EXTERNAL AND INTERNAL FUEL LEAKS OF THE CENTER FUEL TANK AND APU FUEL LINE. NO EVIDENCE OF FUEL LEAK WAS FOUND. REPLACED DOOR 2L AND 2R HEATERS (P/N 116817-2) PRECAUTIONARY.
BOTH HIGH FREQUENCY RADIOS FAILED SHORTLY AFTER TAKEOFF. AIRCRAFT RETURNED TO JFK DUE TO INSUFFICIENT ALTERNATIVE ROUTE FUEL.
AFTER TAKEOFF CLIMBING THROUGH 15000 FT, RIGHT ENGINE INDICATED ONE QUART OF OIL. ENGINE TEMPERATURE AND PRESSURE CONTINUED TO FLUCTUATE. AIRCRAFT RETURNED TO LAX.
