MEASURED DENT: 3.0" X 3.5", Y=.120". ACCOMPLISHED REPAIR PER EN777-53-0020-02 AND UA EVA-60625. REPLACE STR 27L TO 28L SHEAR TIE ANGLE. THE REPAIR IS A CATEGORY B REPAIR REQUIRES VISUAL REINSPECTION 094972 BEGINING AT 30000 CYCLE FROM CURRENT CYCLE COUNT. THE REPETITIVE INSPECTION WILL BE AT INTERVAL 0F 8000 CYCLE THEREAFTER. NOTIFIED SMAC-LINE MAINTENANCE TO REMOVE THE CF ITEM ONCE THE EC IS IMPL EMENTED. THE REPAIR IS FAA APPROVED.
RT ENGINE SEIZED AT 14000 FT. AMBER ALERT DECLARED. ENGINE REPLACED AT IAD. SHOP TEARDOWN FOUND ONE HPT STAGE-2 BLADE FAILED UNDER THE THE PLATFORM DUE TO STRESS CORROSION CRACKING. SIX (6) ADJACENT BLADES WERE FRACTURED, AS A RESULT, APPROXIMATELY 1/4" ABOVE THE PLATFORM, THEREBY CAUSING HEAVY VIBRATION. SEVENTEEN (17) OTHER STAGE-2 BLADES WERE SIMILARILY FRACTURED 1/4" ABOVE THE PLATFORM. ALL MAINLINE BEARINGS SUFFERED HEAVY RESULTANT DAMAGE, ANF THE N1 & N2 ROTORS WERE FROZEN.THIS WAS A FIRST RUN ENGINE.

BROKEN PRV FOUND IN SLIDE WHEN OPENED FOR OVERHAUL. SLIDE WOULD NOT HOLD AIR PRESSURE. DAMAGED PRESSURE RELIEF VALVE WAS OBSERVED DURING SLIDE/RAFT COMPONENT SHOP MAINTENANCE. THE EXACT CAUSE OF THE DAMAGE TO THE PRESSURE RELIEF VALVE COULD NOT BE DETERMINED, BUT IT IS SPECULATED IT WAS DAMAGED DURING THE LAST PACKING PROCESS. PRESSURE RELIEF VALVE ASSEMBLY WAS REPLACED.

LT ENG FIRE LOOP AND LT ENG OVERHT DETECT EICAS MESSAGES. OIL QTY AND OIL PRESSURE DROPPED. ENGINE SHUTDOWN. EMERGENCY DECLARED. ENGINE REPLACED. FOUND ONE HPT STG-2 BLADE FAILED AT AIRFOIL MID-SPAN AS A RESULT OF INTERNAL SULFIDATION. ANOTHER BLADE FAILED NEAR THE AIRFOIL PLATFORM, AS A RESULT OF CONTACT WITH DEBRIS FROM 1ST BLADE. RESULTED IN VIBRATION. OVER TIME, CAUSED FAILURE OF THE FIRE/OVERHEAT LOOPS, & RESULTED IN CRACKING OF OIL PRESSURE LINE BETWEEN SERVO FUEL HEATER & AIR OIL HEAT EXCHANGER. CAUSE OF BLADE SULFIDATION CONTAMINATION OF COOLING PASSAGES WITH TURBO-TIP COATING. PWA HAS CHANGED COATING PROCESS TO PRECLUDE FUTURE CONTAMINATION.
WIRES DETACHED FROM EMERGENCY EVAC LIGHTS ON SEAT SHROUD. REPLACED AFT EMERGENCY LIGHT ASSY.
INTERMITTENT PROBLEM WITH LEFT ENGINE FIRE DETECTION SYSTEM. THE NR 1 ENGINE OVERHEAT MESSAGE WAS A FALSE INDICATION. BOTH OF THE NR 1 ENGINE FIRE LOOPS WERE INSPECTED AND OPS CK'D WITH NO DISCREPANCIES FOUND. DUE TO LEFT CARD FAULT INDICATION FROM CMC, THE NR 1 ENGINE FIRE DETECTION CARD WAS REPLACED.
EMERGENCY LIGHTS AT SEAT 3F (FLOOR PROX) ARE INOP. *S/D* EMERGENCY PROX LIGHTS AT SEAT 3F WERE INOP DUE TO CHAFED OR DAMAGED WIRES NEAR THE THE SEAT DISCONNECT ALONG THE WIRE RUN WHICH CONTAINS THE INVERTERS. INSTALLED TEMPORARY REPAIR BY SPLICING WIRE HARNESS. PERMANENT REPAIR IS SCHEDULED FOR THE NEXT MAINTENANCE OPPORTUNITY.

VACUUM TEST OF NR 1, 1.5 & 2 BEARING COMPARTMENTS INDICATED LEAKAGE ONE PPH OVER LIMIT. DISASSEMBLY REVEALED LIGHT OIL COATING ON INNER WALL OF LPC DRUM. COMPONENT TEST OF NR 1.5 & 2 BEARING CARBON SEAL ASSEMBLY REVEALED HIGH AIR LEAKAGE AT THE 1.5 CARBON SEAL OF 7.3 CU. FT. / MINUTE (LIMIT = 3.0 CU FT / MIN). POSSIBLE CAUSE OF COCKPIT AND CABIN ODOR IS OIL LEAKAGE PAST THE NR 1.5 CARBON SEAL.
