LEFT WING LOWER SURFACE WAS FOUND REWORKED ON OUTER WING INSPAR SKIN PANEL NR 3 AT WSTA, L-18 STRINGER,SURROUNDING STRINGER FASTNER. DAMAGED AREA IS APPROX 1.75 IN DIA AND HAS BEEN BLENDED PRIMED AND PAINTED. DAMAGE WAS REPAIRED BY PATCHING PER AIR CRUISERS CMM 25-61-32.

AIRCRAFT EXPERIENCED A LOSS IN OIL QUANTITY IN THE RT ENGINE. DIVERTED TO YUL. OIL QUANTITY AT TOP OF DESCENT WAS ZERO WITH NORMAL TEMP/PRESSURE. ARRIVED WITH NO PROBLEMS. "ENG OIL FILTER R" BYPASS MESSAGE DISPLAYED AT TOP OF DESCENT; ENGINE CONTINUED TO OPERATE NORMALLY UNTIL DELIBERATELY SHUT DOWN ON THE GROUND. METAL WAS FOUND ON NR 3 BEARING MAGNETIC CHIP DETECTOR, ENGINE REMOVED. EXTENSIVE FRETTING WEAR BETWEEN NR 3 BEARING AND CARBON SEAL SEAT COMPONENTS, ESPECIALLY INNER RACE AND REAR SEAL SEAT. OIL WETTING & COKING WAS FOUND IN THE HIGH PRESSURE TURBINE T1 DISK, INDICATING OIL LOSS THROUGH THIS AREA.
ELECTRICAL ODOR IN THE AREA OF ROW 13 AND 14 ON FLOOR AREA DVS SYSTEM, TURNED OFF MASTER POWER SW. CONTACTED SQAMC, RECOMMENDED THAT SYSTEM BE LEFT OFF FOR REMAINDER OF FLIGHT. *S/D* OPERATED THE VIDEO SYSTEM FOR 6:30 HOURS, VIDEO SYSTEM OPERATED NORMAL AND NO ELECTRICAL SMELL WAS NOTICED. REMOVED ACCESS COVERS BENEATH SEATS AND CHECKED THE SEAT POWER SUPPLY, VIDEO DISTRIBUTION BOXES, SEAT ELECTRONIC UNITS AND EMERGENCY LIGHT CABLES, ALL CHECKED OK. AS A PRECAUTION PACKS OPERATED FOR TWO HOURS, AND NO ELECTRICAL SMELL WAS APPARENT.
LOST LT ENGINE GENERATOR AFTER DEPARTURE FROM FRA. FLIGHT CONTINUED WITH APU RUNNING UNTIL APU SHUTDOWN. DIVERTED TO LHR WHERE OVERWEIGHT LANDING WAS MADE. THE APU GEN WAS SELECTED IN PLACE OF LEFT GEN. HOWEVER, THE APU HAD AN AUTO SHUTDOWN RESULTED IN A DIVERTION TO LHR. THE REMOVED IDG FINDING REPORT IS NOT AVAILABLE AT THIS TIME. HOWEVER, IT WAS SUSPECTED THAT THE IDG EXCITER FAILURE COULD RENDER THE IDG INOPERATIVE. THE LEFT IDG WAS REPLACED.
LOST LEFT ENGINE GENERATOR AFTER DEPARTURE FROM FRA. AFTER DISCUSSION WITH MAINTENANCE, FLIGHT CONTINUED WITH APU RUNNING UNTIL APU SHUTDOWN. DIVERTED TO LHR WHERE OVERWEIGHT LANDING WAS MADE. AN OVER SPEED FAULT WAS DETECTED CAUSING AN AUTOMATIC SHUTDOWN OF THE APU. LINE MAINTENANCE ACCOMPLISHED FIM TASK 49-60-812. THE SPEED SENSOR CONNECTOR WAS CLEANED AND REINSTALLED. APU OPERATED NORMAL. HOWEVER, ACARS STILL DETECTED A FAULT COMING FROM THE SENSOR. ENGINEERING REQUESTED THE SENSOR TO BE REPLACED. AFTER THE REPLACEMENT OF THE PART, FAULTS WERE STILL BEING DETECTED. THEREFORE, ANOTHER SENSOR REPLACEMENT WAS REQUESTED. THE SECOND SENSOR WAS ABLE TO FIX THE FAULTS.

AFTER TAKEOFF FROM CDG UNABLE TO GET WATER PRESSURE TO GALLEYS OR LAVS. DUMPED FUEL AND DIVERTED TO LHR. *S/D* CAUSE WAS DETERMINED TO BE THE QUANTITY/PRESELECT SWITCH (P/N: 42HY331448-3) NOT BEING IN THE OFF/RESET POSITION. THIS CAUSED THE PRESELECT OVERFLOW FILL VALVE TO REMAIN OPEN CAUSING PRESSURE TO BE VENTED OVERBOARD. THE CONDITION WAS CORRECTED BY RESETTING THE QUANTITY PRESELECT SWITCH.
FOUND SHORTED AND BURNED WIRING ABOVE CEILING AT THE P300 PANEL. *S/D* THE CAUSE OF THE PROBLEM WAS DAMAGE TO WIRES IN WIRE BUNDLE W2833. THE PROBABLE CAUSE WAS METHOD OF SECURING THE WIRE BUNDLE TO A STANDOFF OR DAMAGE TO THE WIRE DURING THE INITIAL INSTALLATION. THE DAMAGED WIRES WERE REPLACED. THE ELECTRICAL POWER SYSTEM WAS CHECKED AND FOUND TO OPERATE PROPERLY. BOEING WAS NOTIFIED OF THE PROBLEM. UNITED WILL INSPECT THIS AREA ON THE REST OF THE FLEET TO ASSURE THIS WIRE BUNDLE IS ROUTED CORRECTLY AND PROPERLY SECURED.

THE LOSS OF ELECTRICAL SIGNAL DUE TO WIRE DAMAGE GAVE NON-VALID SIGNALS TO THE AIRCRAFT ELECTRICAL POWER SYSTEM LOGIC ASSOCIATED WITH THE APU (AUXILIARY POWER GENERATOR) POWER AVAILABILITY.
FLIGHT RETURNED TO DEN, UNABLE TO MAINTAIN CABIN ALTITUDE AT CRUISE. *S/D* THE BOOT, REF: (P/N S212W301-9), IPC 36-12-04-005, ITEM 155) AT THE PENETRATION OF THE APU DUCT INTO THE AFT DOME OF FUSELAGE, WAS FOUND TORN IN THE SEAM. FURTHER EXAMINATION OF THE BOOT REVEALED THAT THE STEEL RING (INSIDE THE BOOT) SNAPPED AT THE WELD. THE BOOT WAS REPLACED ACCORDING AND THE A/C WENT TO REVENUE SERVICE.
EN ROUTE, LOST HIGH FREQUENCY RADIO NR 2. *S/D* THE DRIVERS IN THE TRANSCEIVER FAILED. FAILURE PREVENTED THE TRANSCEIVER FROM TUNING PROPERLY. THE FAILURE IS ATTRIBUTED TO NORMAL COMPONENT FAILURE. NEED FOR ADDITIONAL CORRECTIVE ACTION IS NOT INDICATED AT THIS TIME.
