CREW STATES HEAVY FUEL ODOR IN AFT CABIN GOT WORSE IN FLIGHT. AIRCRAFT CURRENTLY OOS FOR REPAIRS.
FLIGHT LEG RETURNING TO BASE FROM PATIENT DROP OFF THE CREW MEMBER IN THE BACK SAID THEY COULD SMELL FUEL. I COULD NOT AND HE STATED THAT HE HAD THE WINDOW OPEN. WHEN I ASKED HIM TO CLOSE THE WINDOW I COULD THEN SMELL THE FUEL UP IN FRONT OF THE CABIN.
SLIGHT ODOR OF FUEL SMELL REPORTED IN FLIGHT
WHILE ENROUTE TO A SCENE CALL I AND THE NURSE, BOTH SITTING IN FRONT OF THE AIRCRAFT, NOTICED A PLASTIC/BURNING SMELL. WHEN WE ASKED MEDIC SITTING IN REAR OF AIRCRAFT IF HE SMELLED OR SAW ANYTHING OUT OF THE ORDINARY HE SAID THAT ONE OF THE THREE MEDICAL A/C ELECTRICAL OUTLETS WAS PRODUCING A DARK GRAY SMOKE. I CONFIRMED SMOKE AND ORIGIN AND TURNED AIRCRAFT TOWARD BOULDER CITY AIRPORT WHICH WAS APPROXIMATELY 6 NM AWAY AND PREPARED TO LAND IMMEDIATELY IF NEEDED. AIRCOM WAS ALSO ADVISED OF OUR DIVERSION. AT THIS POINT THE GFI FOR THE OUTLET POPPED AND THE SMOKE AND SMELL CEASED. I THEN HAD THE INVERTER CUT OFF SWITCHED TO THE MEDICAL OUTLET PANEL ACTIVATED. NO MORE SMOKE WAS OBSERVED, AND WE LANDED WITHOUT INCIDENT AT BOULDER CITY AIRPORT. AFTER EXITING AIRCRAFT AND VERIFYING THERE WAS NO NEED TO ACTIVATE THE FIRE DEPARTMENT, ALL APPROPRIATE PERSONS WERE NOTIFIED.
ON 9-17-2021 VICCA-5002 11088 N7UC CENTRAL FUSELAGE CRACK L/H SIDE ACFT TT 6202+04 WHILE PERFORMING THE 06-5300-EO-3328 A109-INSP OF CENTRAL FUSELAGE FRAME ASSY AT STA 1815, I DISCOVERED A CRACK ON THE INBOARD L/H SIDE, PICS HAVE BEEN SENT OUT. THIS CRACK GOES IN BOTH DIRECTIONS 180 DEGREES FROM A RIVET. THERE IS NOT ENOUGH MATERIAL REMAINING TO STOP DRILL IT ON ONE SIDE OF THE RIVET AND THE CRACK GOES ALL THE WAY TO THE EDGE OF THIS INNER FWD ANGLE.
LOOSE CONNECTIONS ON BACK LEFT CABIN MEDICAL LIGHT CAUSED FAILURE TO LIGHT. ELECTRICAL SMELL WAS REPORTED IN CABIN, SOURCE FOUND AND FIXED BY REPLACING POSITIVE AND NEGATIVE TERMINALS ON BACK LEFT CABIN MEDICAL LIGHT.
DURING HEAVY MAINTENANCE, CRACKS WERE FOUND ON 2 BULKHEADS. LOCATION IS APPROXIMATELY F.S. 3525, ROUGHLY LOCATED ON THE BULKHEADS BENEATH THE AFT TRANSMISSION SUSPENSION BAR ATTACHMENTS. THE TWO CRACKED BULKHEADS ARE ORIGINAL VERSIONS OF THE FOLLOWING P/N'S; 109-0390-24-133 REV.B1, 109-0330-51-103M01, 109-0390-24-109 REV.B1. THERE IS A REPAIR THAT HAS BEEN CREATED SPECIFICALLY FOR THIS KNOWN CRACKING LOCATION;ASRP-3C-A-53-36-02-01A-664A-D UPPER BULKHEAD AT STA 3525- SPECIAL REPAIR PROCEDURE.
UNCOMMANDED YAW INPUTS DURING FLIGHT.
ON APPROACH, THE LEFT MAIN LANDING GEAR FAILED TO FULLY EXTEND AND LOCK DOWN. USED EMERGENCY DOWN AND AT THAT TIME THE LEFT DOWN AND LOCKED INDICATION ILLUMINATED.
THE PILOTS AFT DOOR POST IS CRACKED AT JUNCTION WHERE THE POST ATTACHES TO THE FLOOR. THE CRACK STARTS ON THE INSIDE AND REACHES AROUND TO THE FRONT SIDE OF THE POST.
DURING AN AIRFRAME INSPECTION, FOUND 4 BONDING STRAPS THAT GROUND THE HELIPILOT COMPUTERS TO THE AIRFRAME BROKEN. THESE BONDING STRAPS ARE INSTALLED UNDER BT 109EP-51 DATED 2-9-2005 THESE STRAPS ARE THE GROUNDING POINTS FOR THE HELIPILOT SYSTEM. IT IS AN ON GOING PROBLEM WITH THESE BREAKING. THEY SHOULD BE A BONDING BRAID INSTEAD OF A SHEETMETAL STRAP. WE HAVE ADDRESSED THIS ISSUE WITH MFG AND NOTHING HAS CHANGED.
NR 1 ENGINE LT OB FORWARD ENGINE SUPPORT BRACKET FOUND CANCELLED DURING ROUTINE MAINTENANCE. PART IS VISABLE AFTER INSPECTION PANEL IS REMOVED INSIDE ENGINE COMPARTMENT. PANEL NR 109-0322-10-527.
