Maintenance disclosures
SMOKE WAS DETECTED IN THE CABIN FROM SMELL AND VISUALLY. AIRCRAFT DIVERTED TO THE NEAREST ACCEPTABLE AIRFIELD (KSHD). TROUBLESHOOTING LED TO THE FLAP DRIVE MOTOR ASEMBLY AS THE PART CAUSING THE SMOKE.
DURING ENGINE PRE-TAKEOFF PERFORMANCE RUNS, THE #2 PROP WOULD NOT COME OUT OF FEATHER. MAINTENANCE TEAM INSPECTED THE ENGINE &PERFORMED ENGINE RUNS AND COULD NOT DUPLICATE THE DISCREPANCY. OUT CAUTION, IT WAS DECIDED TO REPLACE THE RH ENGINE PROP GOVERNOR.
DURING LANDING GEAR EXTENSION, RIGHT GENERATOR INOP LIGHT ILLUMINATED.
CREW REPORTED FUEL ODOR/FUMES. FOUND LEAK COMING FROM FUEL WING FLOAT VALVE ASSEMBLY. IAW COMMUNIQUE 2012-04, REPLACEMENT KIT 101-9081-5, INSTALLED ON ACFT.
DURING TAKEOFF ROLL AT APPROXIMATELY 1335Z ON RUNWAY 12 AT KLBF, PF NOTED THAT THE LOWER EFIS TUBE FAILED AT APPROXIMATELY 50-60KTS, WELL BELOW V1. BOTH PILOTS ANNOUNCED ABORT & THE TAKEOFF ROLL WAS SAFELY ABORTED. REPLACED LOWER EFIS TUBE, OPERATIONS CHECKED GOOD.
ON TAKEOFF ROLL WITH FULL THROTTLE APPLIED A VERY STRONG BURNING RUBBER SMELL FILLED THE COCKPIT. THE RT ENG OTBD HOT LIP FLEX HOSE SHOWED SIGNS OF EXHAUST LEAKAGE AROUND THE CLAMP WHICH CONNECTS TO THE INLET HEAT TUBE ON THE EXHAUST STACK. IT APPEARED THE CONNECTOR FLANGE THREADED ONTO THE HOSE HAD BACKED OFF SLIGHTLY.
ON TAKEOFF ROLL WITH MAXIMUM TAKEOFF POWER SET AT 80 KIAS, A BURNING SMELL WAS NOTICED BY ALL CREW MEMBERS. TAKEOFF WAS ABORTED, EXITED THE RUNWAY & BURNING ODOR DISSIPATED, RETURNED TO FBO.
NUMBER 2 RT PROP PITCH LIGHT ILLUMINATED ON APPROACH TO LANDING & REMAINED ON UNTIL SHUTDOWN.
FLIGHT CREW MEMBER NOTICED WHAT APPEARED TO BE A CONTRAIL COMING OFF OF LEFT WING. PIC RECOGNIZED CAUSE AS FUEL BEING SUCKED OUT OF LEFT WING. PRECAUTIONARY LANDING MADE. FOUND THAT LEFT WING FUEL CAP WAS FLAT WITH THE WING BUT UPON OPENING IT, NOTED THAT LANYARD APPEARED TO HAVE BEEN CAUGHT IN THE THREADS WHEN CAP WAS CLOSED. CAP WAS NOT COMPLETELY SECURED AND ALLOWED FUEL TO BE SIPHONED OUT OF WING TANK AT HIGH SPEED.
LEFT PROP FAILED TO COME OUT OF FEATHER WHEN TESTE IAW FLIGHT TEST PROCEDURES FOLLOWING MAINTENANCE. EMERGENCY PROCEDURES COMPLIED WITH AND AIRCRAFT LANDED SINGLE ENGINE SAFELY. RETURNED FOR SERVICE 12-03-98 FOLLOWING PROPELLER, PROP GOVERNOR CHANGE AND RIGGING PROCEDURES IAW TI 4128.2. TEAR DOWN REPORT FROM DUNCAN AVIATION ON PROPELLER P/N HCB4MP3B, S/N FWA 2215, WAS INCONCLUSIVE. A TEAR DOWN REPORT ON PROP GOVERNOR P/N 8221-212, S/N 2253651 HAS BEEN REQUESTED.
