DAMAGE WAS DISCOVERED ON THE LH BEAM UNDER THE ENGINE DECK (PN: 355A21-2110.25) IN THE FORM OF AN ELONGATED HOLE IN THE LOWER FLANGE NEAR THE AFT END OF THE PART
THE TRANSMISSION SUPPORT FRAME (PN 350A21-1041-143) HAS CHAFING DAMAGE ON THE LOWER FLANGE FROM AN UNSECURED CABLE. THE DAMAGE IS UNDER THE TRANSMISSION DECK AT THE AFT RH CORNER OF THE TRANSMISSION CUTOUT
DAMAGE TO THE FWD BULKHEAD CROSSBEAM L/H AND R/H CORNERS. CORNERS SEEM TO HAVE BEEN DAMAGED WHILE LOCATING NUTPLATES ON 15 DEGREE BULKHEAD. DAMAGE DIMENSIONS FOR BOTH LH AND RH CORNERS ARE APPROXIMATELY ( .100â L X .020â W X .015â D)
DAMAGE TO THE ENGINE DECK (PN: 350A21-1239.21) WAS NOTED IN THE FORM OF A CHAFE. THE DAMAGE AREA WAS DISCOVERED TO BE JUST AFT OF THE SCUPPER DRAIN HOLE AND IS APPROXIMATELY 0.40â LONG, 0.30â WIDE, AND 0.010â DEEP AT THE DEEPEST POINT.
MGB CHIP LIGHT ILLUMINATED AFTER DEPARTURE FROM BASE WITH PAT ON BOARD. MAINTENANCE FOUND FOUR SMALL METALLIC PARTICALS (.2MM OR LESS IN SIZE), DETERMINED TO BE CATEGORY F (SWARF) MATERIAL, NOTE TSO IS 15 HOURS AT TIME OF EVENT. PARTICALS WERE RECOVERED AND SENT TO LAB FOR ANALYSIS. CORRECTIVE ACTION INCLUDED CLEANING THE CHIP DETECTOR, DRAINING, AND FILTERING THROUGH A FINE MESH SCREEN, THE MGB OIL, REPLACING THE MGB FILTER, REFILLING THE GEARBOX, AND COMPLETING A 10 MINUTE HOVER CHECK, WITH SATISFACTORY RESULTS.
DURING REMOVAL OF THE FUEL TANK SADDLES FOR THE CRASH RESISTANT FUEL TANK INSTALL, INSPECTED THE SADDLES AND FOUND A BRACKET ON THE FWD RIGHT SADDLE WITH A 25MM CRACK. THE CRACK WAS ON A 2 INCH BY 3 INCH BRACKET OF WHICH THE CRACK STARTED AT A RIVET HOLE EXTENDING TO THE EDGE OF THE BRACKET.
AIRCRAFT AND CREW WERE IN ROUTE TO THE DESTINATION . ABOUT 10 MINUTES OUT FROM THE DESTINATION, ALL CREW MEMBERS SMELLED A BURNING ODOR AND WITHIN 10 SECONDS OF NOTING THE BURNING ODOR THE ENGINE DISPLAY SCREEN DISPLAYED AN ENGINE FLI (FLIGHT LIMITATION INDICATOR) FAILURE ADVISORY MESSAGE. PILOT ELECTED TO PERFORM A PRECAUTIONARY LANDING AT A SOCCER FIELD. A SECOND AIRCRAFT WAS QUICKLY DISPATCHED TO THE LANDING SITE FOR PATIENT TRANSFER AND COMPLETION OF PATIENT TRANSPORT. MAINTENANCE WAS NOTIFIED AND THE ON-DUTY MECHANIC WAS DISPATCHED TO INVESTIGATE. ON 8-28-17 IT WAS DETERMINED THAT AN AFT SECTION OF THE INLET BARRIER FILTER SEAL BECAME DE-BONDED FROM THE FRAME THAT THE FILTER SLIDES INTO THE ENGINE INTAKE HOUSING. THE AFT PORTION OF THE SEAL WAS INGESTED INTO THE ENGINE, THUS CAUSING THE SMELL TO BE FORCED THROUGH THE P3 SYSTEM AND INTO THE CABIN. ENGINE WILL BE REPLACED TO DETERMINE THE EXTENT OF INTERNAL FAILURE.
UPON POWERING UP FOR TAKEOFF, THE PILOT AND MEDICAL CREW SMELLED NOXIOUS FUMES IN THE CABIN. AFTER INSPECTION IT WAS DETERMINED THAT THE EHSI INDICATOR HAD INTERNAL FAILURE. THE ITEM WAS PLACED ON THE DDL, MEL LIST, THE ACFT WAS FLOWN TO AUROR AND A REPLACEMENT EHSI WAS INSTALLED.
FLOOR NUTPLATE CAME LOOSE WHILE INSTALLING A SCREW AND DAMAGED THE SUPPORT RIB. DURING THE INSTALLATION OF A LITTER LOADING ATTACHMENT SCREW INTO A NUTPLATE IN FLOOR IN CABIN AREA, LT SIDE OF ACFT, RIVETS RETAINING THE NUTPLATE SHEARED. THE NUTPLATE SUBSEQUENTLY SPUN ABOUT THE ATTACHING SCREW, DAMAGING THE OEM SUPPORT RIB.
