ON A FLIGHT FROM SILVER CITY, NM TO MEMORIAL MEDICAL CENTER IN LAS CRUCES. I GOT A YELLOW AND RED GOV LIGHT. I WAS ABOUT 5 MINS FROM KSVC, SO I TURNED AND LANDED AT GRANT COUNTY AIRPORT. AIRCOM WAS ALERTED, AND THEY GOT NATIVE 30 HEADED OVER TO PICK UP OUR PATIENT. I DID A LOW POWERED APPROACH AND LANDED SAFELY. MANAGEMENT ALL NOTIFIED AND MECHANIC ARRIVED AND DETERMINED THE HELICOPTER NEEDED A NEW DECU
ABOUT 30 MINUTES AFTER TAKEOFF FROM GILA REGIONAL WITH A INTERFACILITY TRANSFER THE COCKPIT STARTED TO SMELL STRONGLY LIKE BURNING PLASTIC/RUBBER. NO SMOKE WAS PRESENT AND INSTRUMENTS AND WARNING LIGHT PANEL WAS NORMAL WITH NO INDICATIONS OF A PROBLEM BESIDES THE SMELL. I OPENED VENTS AND WINDOW TO VENT SMELL BUT IT DIDN'T GO AWAY. WE WERE OVER DESOLATE DESERT WITH NO CITY'S OR ROADS. COCHISE COUNTY P33 AIRPORT WAS THE CLOSEST DIVERSION FOR US SO WE ALL DECIDED TO MAKE THE DIVERSION AND WAS PREPARED TO MAKE AN EMERGENCY LANDING THE WHOLE WAY TO P33. AFTER 23 MINUTES WE MADE IT SAFELY TO P33 AND LANDED. WILCOX BASE CAME AND PICKED UP OUR PATIENT AND FINISHED THE TRANSFER WHILE WE HAD THE MECHANIC INSPECT OUR AIRCRAFT. UPON INSPECTION HE DECIDED IT WAS THE HALOGEN LIGHT ON THE CEILING ABOVE THE MED CREW, THE NVG FILTER AND THE SEALANT THAT HOLDS THE FILTER IN WAS ALL BURNT AND CRACKED AND HE FIGURED THAT WAS THE SOURCE OF THE BURNING SMELL AND RETURNED US TO SERVICE AND WE FLEW BACK TO BASE AND LANDED SAFELY
CORROSION WAS NOTED ON THE LH AND RH SIDE OF THE CABIN FLOOR EDGE. THE DAMAGE WAS REPAIRED VIA BLENDING AND TRIMMING OUT THE DAMAGED PORTION(S) OF THE EDGE FRAME, ALONG WITH FABRICATING AND INSTALLING SPLICE DOUBLER(S) AT LH STA 75.19 AND RH STA 73.19 TO REINFORCE THE DAMAGED AREAS
THE TRANSMISSION SUPPORT FRAME (PN 350A21-1041-143) HAS CHAFING DAMAGE ON THE LOWER FLANGE FROM AN UNSECURED CABLE. THE DAMAGE IS UNDER THE TRANSMISSION DECK AT THE AFT RH CORNER OF THE TRANSMISSION CUTOUT. THE CHAFING ON THE TRANSMISSION DECK FRAME WITH A LENGTH OF .42â AND WIDTH OF .19â WITH A MAX DEPTH OF .014â.
THE L/H LATERAL BAGGAGE COMPARTMENT WE IDENTIFIED THE LANDING GEARâS L/H VIBRATION DAMPENER FITTING WAS GOUGED. DAMAGE LOCATED AT APROX. STA 2700, WL 2550, BL 600L. THE DAMAGE MEASURES L=0.600â, W=0.300â, D=0.025â.
AIRCRAFT HAS CHAFE DAMAGE ON THE AFT SIDE OF THE 15 DEGREE BULKHEAD LOCATED IN THE L/H LATERAL BAGGAGE COMPARTMENT IN TWO DIFFERENT LOCATIONS. THE DAMAGE IS LOCATED AT APPROXIMATELY STA 2700, WL 2300, AND BL 500L. THE OB DAMAGE MEASURES.2â LONG AND .11â WIDE WITH A MAX DEPTH OF .009â. THE IB DAMAGE MEASURES .4â LONG AND .39â WIDE WITH A MAX DEPTH OF .015â.
DAMAGE TO THE 15 DEGREE BULKHEAD LH WEB (PN: 350A21-1032.60 IN THE FORM OF CHAFES. THE FIRST CHAFE DAMAGE AREA WAS FOUND NEAR THE X-FRAME AND WAS MEASURED TO BE APPROXIMATELY 0.40â X 0.39â AND 0.015â DEEP; THE SECOND CHAFE DAMAGE AREA WAS FOUND ON THE STIFFENING FEATURE AND WAS MEASURED TO BE APPROXIMATELY 0.20â X 0.11â AND 0.009â DEEP
AC 7015 HAS CHAFE DAMAGE TO THE L/H X-FRAMEâS AFT LOWER LEG LOCATED AT APPROXIMATELY STA 3650, WL 2305, AND BL 430L. THE DAMAGE MEASURES .21â LONG AND .2â WIDE WITH A MAX DEPTH OF .019â.
DAMAGE TO THE ENGINE DECK (PN: 350A21-1239.20) IN THE FORM OF CHAFES NEAR THE SCUPPER DRAIN. THE DAMAGE AT THE SCUPPER DRAIN WAS MEASURED TO BE 3.90â X 7.40â AND 0.014â DEEP
GOUGING FOUND ON RIB UNDERNEATH CABIN FLOOR AT THE RIB BEHIND THE CO PILOTS COLLECTIVE. GOUGING WAS THROUGH THE COMPLETE THICKNESS OF THE RIB. THE DAMAGE WAS APPROXIMATELY 13/32" IN LENGTH AND LOCATED AT THE BEND RADIUS OF THE RIB. ** THIS AIRCRAFT REMAINS OOS AT THE TIME OF THIS SUBMISSION. **
