CREW STATED THAT WHILE ON FINAL FOR RUNWAY 19L, THE GEAR DID NOT COME DOWN. THEY REQUESTED DELAY VECTORS TO RUN PROCEDURE. THEY PERFORMED PROCEDURE A SUCCESSFULLY, GEAR EXTENDED. THEY DECLARED AN EMERGENCY AND LANDED WITHOUT FURTHER INCIDENT. FOUND THE RH MAIN LANDING GEAR BRACING CYLINDER LEAKING. THEY REMOVED AND REPLACED THE RH MLG BRACING CYLINDER WITH A SERVICEABLE UNIT IAW MM32-30-11. LEAK CHECK WAS NORMAL, HOWEVER THEY WERE UNABLE TO PERFORM THE REQUIRED OPERATIONAL CHECK. LANDING GEAR LOCKS WERE INSTALLED AND THE AIRCRAFT WAS INSPECTED AND FERRIED TO KYIP IN ORDER TO PERFORM THE OPS CHECK. UPON ARRIVAL IN KYIP THE LANDING GEAR LOCKS WERE REMOVED AND SEVERAL LANDING GEAR EXTENSION AND RETRACTION TESTS WERE PERFORMED IAW MM 32-30-00. OPS AND LEAK CHECKS WERE BOTH NORMAL AT THIS TIME

THEY REMOVED AND REPLACED THE AIR CONDITIONING SHUT-OFF VALVE IAW DA20 MM21-20-1, NO HELP. THEY FOUND THE RH P3 VALVE BINDING, CAUSING THE CABIN CONDITIONING CIRCUIT BREAKER TO POP THEY REMOVED AND REPLACED THE RH P3 VALVE, OPS AND LEAK CHECKS BOTH NORMAL IAW USA JET WORK CARD N36-01. THEY PERFORMED AN OPS. CHECK OF THE AIRCRAFT PRESSURIZATION SYSTEM IAW DA20 MM 21-30-5. OPS CHECK NORMAL WITH NO OTHER DEFECTS NOTED AT THIS TIME

PULLED LEFT T-HANDLE AND SHUT ENGINE DOWN. USA JET MAINTENANCE SENT TO KONT FOUND METAL DEBRIS IN ENGINE EXHAUST WITH FAN SEIZED. ENGINE CORE TURNS BUT THERE IS CLANKING NOISE IN THE TURBINE AREA. SUSPECT FAILED TURBINE BLADE. ENGINE WILL BE REPLACED AND SENT TO ENGINE SHOP FOR TEARDOWN AND EVALUATION. SU0PPLEMENTAL INFORMATION ADDED 12/06/18 - TEARDOWN OF THE ENGINE REVEALED THAT A FIRST STAGE TURBINE BLADE HAD FAILED CAUSING ADDITIONAL DAMAGE TO BOTH FIRST STAGE & 2ND STAGE TURBINE BLADES.

INDICATION WAS THREE GREEN, NO RED. DECLARED EMERGENCY WITH TOWER AND REQUESTED AN IMMEDIATE RETURN. MAINTAINED 2000 AGL. MADE A TURN TO THE LEFT AND MADE A NORMAL VISUAL APPROACH TO RWY 13. LANDING WITHOUT INCIDENT. CORRECTIVE ACTION: CONTRACT MAINTENANCE AT MMLO INSPECTED THE EXTERIOR OF THE AIRFRAME SURFACES, TAIL, FLAPS, DLE, AND WHEEL WELLS. NO DAMAGE NOTED EXCEPT FOR BLOOD, ETC. IN NUMEROUS LOCATIONS. LT OUTBOARD HYDRAULIC LINE LEAKS DUE TO A HOLE IN THE LINE. THE LT ENGINE INLET/EXHAUST/ROTATION/FOD INSPECTION REVEALED NO DEFECTS. OPENED COWLING AND NO DEFECTS WERE NOTED. ANTI-SKID TEST WAS GOOD. HYDRAULIC QUANTITY IS GOOD. MAINTENANCE TECH DISPATCHED TO MMLO REMOVED AND REPLACED DAMAGED BRAKE LINES. OPS AND LEAK CHECK WAS GOOD AND BLED IAW DA-20 MM 32-40-22. INSTALLED AND SAFETIED LANDING GEAR ACTUATOR LOCKS AS REQUIRED. CLEANED DEBRIS FROM STRUT AND INSPECTED AREA. NO DAMAGE NOTED. INSPECTED AIRCRAFT AND BOTH ENGINES PER GMM VOL. 3, CHP. 5.60.1. NO DEFECTS NOTED. PERFORMED OPERATION OF BOTH ENGINES IAW GE SEI-189 OPERATIONS MANUAL. OPS CHECK GOOD. NO DEFECTS NOTED. AIRCRAFT FLOWN UNDER MEXICAN AND US SPECIAL FLIGHT PERMIT, WITH LANDING GEAR DOWN AND LOCKED, TO KLRD FOR RESOLUTION OF GEAR NOT RETRACTING. MAINTENANCE AT KLRD REMOVED AND REPLACED THE 6G RELAY & LANDING GEAR SELECTOR SWITCH AND PERFORMED UPLOCK CHECK IAW DA-20 MM 32-30-00 & 29-20-1. OPS AND LEAK CHECK GOOD DURING GEAR EXTENSION & RETRACTION. NO DEFECTS NOTED.

R & R AILERON TRIM SAFETY BOX RELAYS WITH NO HELP. R & R TRIM/SPEED/BRK CONTROL PANEL. SYS NO LONGER HAS DELAY. RANG AILERON CONTROL WIRES TO AILERON TRIM SAFETY BOX. OPS CHECK WAS GOOD IAW WIRING DIAGRAM 27-10-5, PG. 103. SYS OPS CHECKED GOOD IAW MM 27-00-5. ACFT WAS RETURNED TO SERVICE.

ON SUBSEQUENT TEST FLIGHT THE ENGINE FLAMEOUT OCCURRED AT FL310 WHILE SETTING UP THE MAX ALTITUDE ACCELERATION TEST. TEST FLIGHT CONDUCTED WHILE ENROUTE AND ENGINE WAS RELIT AT FL280. ACFT CONTINUED AND LANDED WITHOUT INCIDENT. SUBSEQUENT INSPECTION OF ENGINE USING A BORESCOPE, DETERMINED NUMEROUS (20+) FIRST STAGE TURBINE BLADES HAD FD. ENGINE WILL BE REPLACED AND SENT FOR SHOP VISIT.

ENGINES NR 1 AND NR 2 THEN BOTH STABILIZED WITH THE CROSSFEED VALVE OPEN. AIRCRAFT LANDED BACK WITHOUT FURTHER INCIDENT. DURING GROUND RUN, FOUND NR 1 BOOST PUMP PRESSURE DROP, OPENED CROSSFEED VALVE AND NR 2 BOOST PUMP, PRESSURIZED BOTH SIDES. REMOVED AND REPLACED NR 1 ENG BOOST PUMP IAW MM. RAN BOTH ENGINES AT TAKE OFF POWER. BOTH ENG OPS WERE GOOD DURING RUN AND AT NO TIME ROLLED BACK. AC WAS TEST FLOWN SATISFACTORY AND RETURNED TO SERVICE.(K)

ON TAKEOFF, CREW DETECTED VIBRATION ON RT MAIN GEAR. CREW HAD TOWER INSPECT RUNWAY TO FIND TIRE CAP AS SUSPECTED. AC DIVERTED, TO BURN FUEL AND OBTAIN EMERGENCY FACILITIES. CREW DECLARED AN EMERGENCY AND LANDED WITHOUT INCIDENT. EMERGENCY WAS CANCELLED BY CREW UPON EXITING RUNWAY. MECHANIC WAS DISPATCHED TO EVALUATE AC. UPON INSPECTION, MECHANIC FOUND RT IB FLAP, RT OB GEAR DOOR, RT WING IB T/E, AND NR 2 ENGINE AFT COWL TO HAVE MINOR SHEET METAL DAMAGE (DENTS). ACCORDING TO CREW THE NR 4 TIRE STILL HAD PRESSURE ON LANDING AND ONLY DEFLATED DURING TAXI TO RAMP. THE AIRCRAFTS NR 3 AND NR 4 WHEEL AND TIRE ASSEMBLIES WERE REPLACED WITH SERVICEABLE UNITS. AIRCRAFT UNDERGOING REPAIRS FOR SHEET METAL DAMAGED.

ELP - WHEN FUEL WAS NOT ABLE TO TRANSFER PROPERLY FROM RIGHT TO LEFT WING, THE CREW RETURNED TO POINT OF DEPARTURE AT ELP. NO EMERGENCY PROCEDURES WERE REQUIRED. MAINTENANCE FOUND THAT THE LEFT WING INTERCONNECT JET PUMP WAS CLOGGED. THE OBSTRUCTION WAS REMOVED AND THE PUMP REINSTALLED. PERFORMED A LEAK AND OPERATIONS CHECK IAW FALCON MM CHAP 28-10-3. NO DEFECTS WERE NOTED. (M)
