DISCREPANCY: WHILE AT C-CHECK IN KBQK, STAMBAUGH AVIATION FOUND CORROSION ON THE L/H WING LOWER SURFACE AT STR. 17 (SKIN SPLICE) AND AT XCW 85. BLENDED OUT THE DAMAGED AREA, PERFORMED HFEC OPEN HOLE INSPECTION AND FABRICATED A REPAIR DOUBLER USING 2024T3 .125â AND FILLER USING 7075T6 .063â. THEY PICKED UP THE HOLES, CLEANED, TREATED, PRIMED AND INSTALLED THE FILLER AND REPAIR DOUBLER IAW MD80 SRM 57-03 AND FLIGHT VEHICLES CONSULTING, INC. ENGINEERING ORDER 25095-1. PICKED UP HOLES, CLEANED, TREATED, PRIMED, AND INSTALLED THE SPLICE REPAIR SECTION OF THE LT HORIZONTAL UPPER TRAILING EDGE BEAM IAW SRM 55-10-6 AND 51-30-2, NO OTHER DEFECTS NOTED AT THIS TIME

MX R & R RECIRCULATION FAN IAW AMM 21-20-12. MX PERFORMED BORESCOPE INSPECTION OF #1 & #2 ENGINES. NO ENGINE OIL LEAKS WERE FOUND. MX INSPECTED LT & RT ACMS & ASSOCIATED DUCTS & FOUND OIL IN LT DUCTS. THE LT ACM R & R IAW AMM 21-51-01 & DUCTS CLEANED. OPS & LEAK CHECKED GOOD. BOTH LT & RT WATER SEPARATOR COALESCER BAGS R & R IAW AMM 21-20-02.
DISCREPANCY: THE FLIGHT CREW REPORTED THAT DURING CRUISE AT FL370 AFTER SWITCHING THE ENGINE ANTI-ICE ON THEY COULD SMELL SMOKE, FOLLOWED BY SIGNIFICANT SMOKE. THE CREW RAN THE SMOKE/FIRE FUMES CHECKLIST. THE CREW DONNED OXYGEN MASKS AND DECLARED EMERGENCY. THE CREW REPORTS THAT THE SMOKE CLEARED BY FL090. LANDED SAFELY IN KLBB WITH NO OTHER ISSUES. CORRECTIVE ACTION: USA JET MAINTENANCE IN KLBB INSPECTED THE RECIRCULATION FAN AND ASSOCIATED DUCTING. MAINTENANCE FOUND THE RECIRCULATION FAN TO BE MAKING GRINDING NOISE. MAINTENANCE REMOVED AND REPLACED THE RECIRCULATION FAN IAW MD88 AMM 21-20-12, OPERATIONAL AND LEAK CHECK GOOD. PERFORMED ENGINE GROUND RUN SYSTEM CHECK WITH AIR CONDITIONING SYSTEM CHECK GOOD IAW MD88 AMM 72-00-00.
CREW STATED THAT WHILE ON FINAL FOR RUNWAY 19L, THE GEAR DID NOT COME DOWN. THEY REQUESTED DELAY VECTORS TO RUN PROCEDURE. THEY PERFORMED PROCEDURE A SUCCESSFULLY, GEAR EXTENDED. THEY DECLARED AN EMERGENCY AND LANDED WITHOUT FURTHER INCIDENT. FOUND THE RH MAIN LANDING GEAR BRACING CYLINDER LEAKING. THEY REMOVED AND REPLACED THE RH MLG BRACING CYLINDER WITH A SERVICEABLE UNIT IAW MM32-30-11. LEAK CHECK WAS NORMAL, HOWEVER THEY WERE UNABLE TO PERFORM THE REQUIRED OPERATIONAL CHECK. LANDING GEAR LOCKS WERE INSTALLED AND THE AIRCRAFT WAS INSPECTED AND FERRIED TO KYIP IN ORDER TO PERFORM THE OPS CHECK. UPON ARRIVAL IN KYIP THE LANDING GEAR LOCKS WERE REMOVED AND SEVERAL LANDING GEAR EXTENSION AND RETRACTION TESTS WERE PERFORMED IAW MM 32-30-00. OPS AND LEAK CHECKS WERE BOTH NORMAL AT THIS TIME
CREW REPORTED THAT PREFLIGHT-TAXI-TAKEOFF-CLIMB-THROUGH CRUISE CHECK AT FL320 ALL FUEL INDICATIONS WERE NORMAL. 30 MINUTES LATER FO NOTICED THAT THERE WERE LESS THAN 500 LBS. INDICATING ON THE R/H FUEL GAUGE AND THE L/H FUEL GAUGE SHOWED 3500 LBS. THE CAPTAIN DIRECTED THE FO TO RUN THE âFUEL ASYMMETRY PROCEDURE. THE CAPTAIN LOOKED FOR A POSSIBLE DIVERSION AIRPORT FIRST, THEN ASKED ATC FOR THE CLOSEST AIRPORT WITH AT LEAST A 6000 FT RUNWAY. ATC INFORMED THEM THAT KARG WAS 70 MILES OUT. THE FO INITIATED CROSS FEED TO SEE IF THEY COULD KEEP THE RH FUEL TANK FROM DEPLETING TO 0. THE CAPTAIN DECLARED AN EMERGENCY DESCENT INTO KARG. THE AIRCRAFT LANDED SAFELY WITH 0 LBS. FUEL IN THE RH WING, 300 LBS. IN THE RH FEEDER TANK. AND 3200 LBS.IN THE LH WING. MAINTENANCE IN KARG FOUND THE WING PRE INTERCONNECT OUT OF ADJUSTMENT. THEY ADJUSTED THE WING PRE INTERCONNECT IAW MM 28-10-33. LEAK AND OPS. CHECKS NORMAL. THEY ALSO REMOVED AND REPLACED THE RH TRANSFER PUMP IAW MM 28-10-32. LEAK AND OPS. CHECKS BOTH NORMAL. NO OTHER DEFECTS NOTED AT THIS TIME
DISCREPANCY: CREW REPORTED THAT THE CABIN ALTITUDE LIGHT AND HORN WENT OFF UPON REACHING CRUISE ALTITUDE. THEY DONNED MASKS AND RAN POH PROCEDURE. THEY COULD NOT REDUCE CABIN ALTITUDE WITH AUTO OR MANUAL SYSTEMS. THEY EXECUTED EMERGENCY DESCENT. THEY EXECUTED AN EMERGENCY DESCENT, COORDINATED A DIVERSION TO KDAL WITH ATC AND OPS. UPON REACHING 10000 FEET, THEY STOWED MASKS. THE REST OF THE TRIP WAS UNEVENTFUL. CORRECTIVE ACTION: USA JET MAINTENANCE IN KDAL FOUND THE CABIN CONDITIONING CIRCUIT BREAKER POPPED. THEY REMOVED AND REPLACED THE AIR CONDITIONING SHUT-OFF VALVE IAW DA20 MM21-20-1, NO HELP. THEY FOUND THE RH P3 VALVE BINDING, CAUSING THE CABIN CONDITIONING CIRCUIT BREAKER TO POP THEY REMOVED AND REPLACED THE RH P3 VALVE, OPS AND LEAK CHECKS BOTH NORMAL IAW USA JET WORK CARD N36-01. THEY PERFORMED AN OPS. CHECK OF THE AIRCRAFT PRESSURIZATION SYSTEM IAW DA20 MM 21-30-5. OPS CHECK NORMAL WITH NO OTHER DEFECTS NOTED AT THIS TIME
DISCREPANCY: THE CREW REPORTED ON DEPARTURE, CLIMBING THROUGH 8,000 FEET THE ENGINE #2 FIRE WARNING WENT OFF. THE CREW LEVELED THE AIRPLANE AT 9,000 FEET TO RUN THE RED BOARDER CHECKLIST PROCEDURE. DURING STEP 1 OF THE CHECKLIST PROCEDURE (THRUST LEVER-VERIFY IDLE) THE ENGINE FIRE LIGHT WENT OUT. FLIGHT WAS CONTINUED WITH #2 ENGINE AT IDLE. DISCREPANCY: DURING C-CHECK AT KDHN A CRACK WAS FOUND ON L1 DOOR AFT BEAM NR 3. CORRECTIVE ACTION: COMMERCIAL JET IN KDHN STOP DRILLED CRACK AND VERIFIED BY NDT. THEY INSTALLED NEW REPAIR DOUBLER AND GUSSET FITTING ON L1 DOOR NR 3 BEAM AFT AREA, IAW DC9 SRM 52-05 FIGURE 85 SHEET 4.
DISCREPANCY: DURING C-CHECK IN KDHN CORROSION WAS FOUND AT FS 724 BETWEEN L27R AND L28R ON FUSELAGE SKIN. CORRECTIVE ACTION: COMMERCIAL JET IN KDHN REMOVED DAMAGED AREA AT FS 724 BETWEEN L27R AND L28R. PERFORMED A HFEC INSPECTION OF THE AREA. THEY INSTALLED REPAIR DOUBLERS -69 AND -71 AND FILLER -75, IAW EO 12254 AND 8110-3.
DISCREPANCY: DURING C-CHECK IN KDHN THE FORWARD BAGGAGE COMPARTMENT FS 322 CARGO DOOR LOWER JAMB WAS FOUND TO BE CHAFED AT LONGERON 28. CORRECTIVE ACTION: COMMERCIAL JET IN KDHN REMOVED DAMAGED AREA AND PERFORMED A HFEC INSPECTION OF THE AREA. THEY FABRICATED A NEW REPAIR DOUBLER USING .050 THICK CLAD 2024-T3 ALUMINUM. DRILLED, TREATED, PRIMED AND INSTALLED -003 DOUBLER, IAW EO 2020-1118 AND 8110-3.
DISCREPANCY: CREW REPORTED THAT AFTER DEPARTURE FROM KLRD AND CLIMBING THROUGH 10000 FEET, THE NR 3 ENGINE FAILED WITH ASSOCIATED VIBRATIONS. THEY DECLARED AN EMERGENCY AND PERFORMED THE ENGINE FIRE, SEVERE DAMAGE OR SEPARATION CHECKLIST AND LANDED AT THE DEPARTURE AIRPORT WITHOUT FURTHER INCIDENT. CORRECTIVE ACTION: USA JET MAINTENANCE IN KLRD INSPECTED THE ENGINE AND FOUND ALL FOUR STAGES OF THE TURBINE SECTION TO HAVE FOD. THEY REMOVED AND REPLACED THE NR 3 ENGINE, IAW USA JET GMM VOLUME 4 CHAPTER 72.01.1 TO 72.01.26. ALL OPERATIONS AND LEAKS NORMAL DURING ENGINE TRIM RUN.
DISCREPANCY: DURING C-CHECK AT KDHN, CORROSION WAS FOUND ON THE FORWARD CARGO LOWER FUSELAGE BEAM AT STATION 579R â 579L ON THE FORWARD AND AFT ENDS. CORRECTIVE ACTION: COMMERCIAL JET CUT OUT THE DAMAGED SECTION OF THE FLOOR BEAM AT STATION 579 RBL 38.0 TO RBL 41.2, FABRICATED, LOCATED, DRILLED, DEBURRED, TREATED, PRIMED AND INSTALLED -1 REPAIR ANGLE, -2 REPAIR ANGLE, -3 FILLER AND -4 FILLER IAW ED20161 REV A AND DC9 SRM 51-10-4, 51-10-3 AND 51-60-0. FAA APPROVED PER 8110-3 ROBERT MANASHI DERT-710167-SW STRUCTURES DATED 5/22/2020.
DURING C-CHECK AT KDHN 3 NDT CONFIRMED CRACKS FOUND ON L/H WING XRS 267 UPPER SPAR CAP DURING ACCOMPLISHMENT OF AD05-07-03, S/B 57-179. REMOVED AND REPLACED DISCREPANT AREA OF THE LT WING UPPER REAR SPAR CAP FROM XRS 167.15 TO XRS 319.5 IAW DC9 S/B 57-179 AND DRAWING J060214.
