TANDEM HYDRAULIC PN 117-45020V002, SN 05-0198, TT 9205 4 FAILED AMC BK117 AAIP A-1641 TEST. THE LATERAL 1 SERVO VLV PN 105-46682 WAS FOUND MISSING ROLL PIN & SPRING.
PILOT REPORTED FUEL SEEPING FROM ENGINE NR 2 COWLING AND MIGRATING BACK TO RT CLAMSHELL DOOR: UPON TROUBLESHOOTING, FOUND DAMAGED O-RING AT ENGINE 2 DRAIN VALVE. THE MECHANIC R & R THE O-RING AND PERFORMED LEAK CHECK WITH NO DISCREPANCIES NOTED. THE ACFT WAS RETURNED TO SERVICE AT THAT TIME.
THE PILOT DETECTED THE ODOR OF FUEL IN THE CABIN AND COCKPIT, APPROXIMATELY FOUR MINUTES INTO FLIGHT. RETURNED TO BASE. MX INSPECTED ACFT IAW THE MM 60. NO DEFECTS WERE NOTED. A MX CHECK FLIGHT WAS PERFORMED WITH NO ODOR DUPLICATED. THE ACFT WAS RETURNED TO SERVICE.
PILOT DID NOT PUT FUEL CAP BACK ON PROPERLY AFTER REFUELING, AFTER TAKEOFF A FUEL ODOR WAS NOTICED PILOT MADE AN UNSCHEDULED LANDING, NOTICED FUEL ON THE CABIN DOOR. INSTALLED FUEL CAP PROPERLY WITH OUT FURTHER INCIDENT.
