DECLARED EMERGENCY ON CURRENT ATC FREQUENCY AND ASKED FOR VECTORS AND DESCENT TO A SUITABLE AIRPORT. THEY PERFORMED OIL PRESSURE LIGHT ON CHECKLIST WHICH LEADS TO ENGINE FAILURE SHUTDOWN IN FLIGHT THEN LANDING WITH LEFT ENGINE SHUTDOWN WINDMILLING N1 AND HYDRAULIC PRESSURE THEN GEAR DOWN PROCEDURE A. LANDED WITHOUT INCIDENT AT KLEX. AFTER PARKING NOTED OIL LEAKING FROM DRAIN AND OPENED THE AFFECTED ENGINE COWL AND NOTED THE ENGINE WAS COVERED IN OIL. RELAYED INFORMATION TO FLIGHT OPS. AND MAINTENANCE DEPARTMENTS.CORRECTIVE ACTION: USA JET MAINTENANCE IN KLEX REMOVED AND REPLACED THE #1 ENGINE IAW USA JET AIRLINES GMM VOL # CHAPTER 72 ENGINE CHANGE CHECKLIST. LEAK AND OPS. CHECKS NORMAL, NO DEFECTS NOTED AT THIS TIME. REMOVED ENGINE TO BE SENT TO ITI FOR EVALUATION.

LANDING GEAR CYCLED AND GEAR DOWN AND LOCKED LIGHTS ILLUMINATED. LANDING GEAR EXTENSION PROCEDURE A USED TO ENSURE LANDING GEAR DOWN AND LOCKED. EMERGENCY DECLARED WITH APPROACH CONTROL. ABNORMAL LANDING GEAR LIGHT SITUATION ADVISORY GIVEN TO CENTERS. TOWER NOTIFIED AND CRASH/ FIRE/RESCUE BY RUNWAY. FLY BY SHOWED ALL THREE GEAR DOWN. UNEVENTFUL LANDING. CORRECTIVE ACTION: MAINTENANCE AT KYIP FOUND THE BEARING SUPPORT ARM ASSEMBLY HAD FAILED ON THE RIGHT LATERAL DOOR. MAINTENANCE REMOVED AND REPLACED THE DAMAGER LATERAL AND CENTER DOORS WITH SERVICEABLE UNITS AFTER DOING NDT ON THE RELATED MOUNTING PARTS. THE BEARING SUPPORT ARM ASSEMBLY WHICH FILED ON THE RIGHT LATERAL DOOR WAS A P/N MY20294-10810. THIS IS AN ORIGINAL DESIGN PART. SERVICE BULLETIN 492 ADDRESSES THIS PART AND REPLACES IT WITH P/N MY20294-10810-5. THE STATED REASON IS TO INCREASE THE FATIGUE BEHAVIOR OF THE LATERAL MAIN GEAR DOOR FORWARD BEARING ARMS. THE RIGHT LATERAL DOOR ACTUATOR BELLCRANK WAS REMOVED FOR INSPECTION AND ALSO ITS TWO STOPS. THE STOPS AND “SCHNOOR” WASHERS WILL BE REPLACED WITH NEW PARTS. THE BELLCRANK WILL BE INSPECTED IAW MM 32-10-21. THE GEAR AND DOOR CONTROL LINKAGES WILL BE INSPECTED, RIGGED AND TESTED. THE GEAR WILL BE CYCLED TO TEST ALL FUNCTIONS IN NORMAL AND EMERGENCY MODES. MINOR DAMAGE WAS FOUND TO A DOOR SEAL ON THE AFT SIDE OF THE WHEEL WELL. THIS SEAL HOLDER WILL BE REPLACED WITH A SERVICEABLE PART. THE AIRCRAFT REMAINS OUT OF SERVICE WHILE AN “A” CHECK IS BEING PERFORMED. FURTHER INFORMATION WILL FOLLOW ON COMPLETION OF THE INSPECTION AND AIRCRAFT RETURN TO SERVICE. 01/10/19 SUPPLEMENTAL INFORMATION – AFTER COMPLETION OF THE “A” CHECK, INCLUDING CYCLING OF THE LANDING GEAR, THE AIRCRAFT WAS RETURNED TO SERVICE AND OPERATED FOR WITH NO MALFUNCTION OF THE GEAR INDICATION.

FOUND NOSE GEAR DOOR HYDRAULIC ACTUATOR BLOWN OUT ON POST FLIGHT INSPECTION. MAINTENANCE TRAVELED TO EVALUATE AIRCRAFT. MAINTENANCE REPLACED THE NNLG DOOR ACTUATOR IAW MM 32-30-23. LEAK AND OPS CHECK WAS GOOD. REPLACED THE NR 1 HYDRAULIC PUMP FILTER IAW MM 29-00-2. NO PARTICLES NOTED. THE NR 1 ENGINE HYDRAULIC PUMP WAS REPLACED IAW MM 71-00-1. LEAK AND OPS CHECK WAS GOOD AND NO CONTAMINATION OF REMOVED PUMP NOTED. THE LT AND RT HYDRAULIC SYSTEM WAS SERVICED IAW MM 12-10-31. NR 1 HYDRAULIC SYSTEM WAS NOT COMPLETELY EMPTY AT TIME OF SERVICE. NR 1 AND NR 2 HYDRAULIC SYSTEMS, OPERATIONALLY CHECK AND TAXIED ACFT. ALL SYSTEMS FUNCTIONING NORMAL. ACFT WAS THEN FLOWN, WITH THE LANDING GEAR DOWN AND LOCKED, UNDER A SPECIAL FLIGHT PERMIT FOR FINAL RIG CHECK OF NLG AND RETURN-TO-SERVICE. UPON ARRIVAL MAINTENANCE COMPLIED WITH A NLG DOOR INSPECTION/TEST IAW MM 32-20-20. NO LEAKS NOTED ON NOSE GEAR DOOR ACTUATOR. INSPECTION FOR PROPER INSTALLATION AND GEAR SWING WAS COMPLETED WITH NOT DEFECTS NOTED. INSPECTION OF THE FAILED ACTUATOR SHOWED THAT THE CASTING CRACKED AND SUBSEQUENTLY FAILED ALLOWING THE END CAP TO BLOW OUT OF THE END OF THE CYLINDER WHICH RESULTED IN THE LOSS OF HYDRAULIC FLUID. TOTAL TIME AND CYCLES OF THE ACTUATOR ARE UNKNOWN. ACTUATOR ACCUMULATED 7168 HOURS AND 5384 CYCLES SINCE RESEAL, SINCE INSTALLATION ON ACFT.

SECURED B NUT AND PERFORMED ALL REQUIRED ENGINE RUNS IAW GMM VOLUME 3 "STALL/FLAME OUT PROCEDURE CHECKS" ALL ENGINE PARAMETERS ARE WITH IN LIMITS, RESTRICTED ALTITUDE TO FL330. AIRCRAFT RETURNED TO MAIN BASE AT YIP ON 1/12/08 AND MAINTENANCE PERFORMED A DETAILED INSPECTION ON NR 1 ENGINE IAW GMMK VOLUME 3 CHAPTER 72. PERFORMED GROUND RUNS IAW SEI 189 AND ALL PARAMETERS WITH LIMITS. AIRCRAFT RETURNED TO SERVICE.
