FLT NR 4127 - SHORTLY AFTER TAKEOFF, WHILE SETTING CRUISE POWER (97 PERCENT RPM) THE LEFT ENGINE FLAMED OUT. AIRCRAFT RETURNED TO FIELD AND LANDED WITHOUT FURTHER INCIDENT. PIT MAINTENANCE REMOVED AND REPLACED LEFT FUEL CONTROL IAW MM 73-00-00. GROUND CHECKED NORMAL. AIRCRAFT RETURNED TO SERVICE.
DURING A TIRE CHANGE IN LYN, A MAINTENANCE TECHNICIAN NOTICED SOME DAMAGE TO THE LEFT MAIN WHEEL ASSY. UPON REMOVAL OF THE WHEEL ASSY THEY ALSO NOTICED DAMAGE TO BRAKE ASSY AND WHEEL BEARINGS. MAINTENANCE REPLACED LEFT MAIN WHEEL, WHEEL BEARINGS, AND BRAKE ASSY IAW MM 32-40-00 AND REPLACED LEFT MLG AXLE WHICH WAS DAMAGED AS A RESULT OF BEARING FAILUE IAW MM 32-00-00. AIRCRAFT RETURNED TO SERVICE. (M)
IPT - FLT 4164 - AIRCRAFT SUFFERED DAMAGE TO LT MLG AS A RESULT OF IMPACTING A DEER ON TAKEOFF FROM IPT, AIRCRAFT DIVERTED TO MDT FOR EMERGENCY LANDING. LT MLG COLLAPSED ON LANDING CAUSING DAMAGE TO LT ENGINE, PROPELLER AND OUTBOARD WING TIP. LT MLG ENGINE AND PROPELLER REPLACED IAW BAE MM, ALL STRUCTURAL REPAIRS CONDUCTED IAW WITH BAE SRM.
