ON TAKEOFF AT ABOUT 40 KIAS, THE MASTER CAUTION ILLUMINATED ALONG WITH THE FUELFLTR BP LIGHT. RECEIVED FUEL SAMPLE 07/30/2025. SHOWED ALUMINUM FLAKES. TANKS EMPTIED AND CLEAN. REPLACING EJECTOR PUMP RH. THE FUEL CREW HAS FOUND ALL THE METAL SHAVINGS TO BE IN THE RIGHT TANK ONLY. THEY HAVE INFORMED US THAT IT APPEARS TO BE SWARF FROM DRILLING..BOTH TANKS INSPECTED AND CLEANED.
ON TAKEOFF RUNWAY 14L, REACHED TAKEOFF POWER DETENT, FELT A SMALL LURCH OF IN RUDDER PEDALS. MASTER CAUTION ILLUMINATED. REMOVED AND REPLACED RUDDER BIAS SHUT OFF VALVE
AFTER THE AUTOPILOT WAS ENGAGED THE PITCH TRIM COULD NOT BE CONTROLLED. IT KEPT PITCHING UP, EVEN WHEN THE TRIM WAS ALL THE WAY FORWARD. BOTH PILOTS HAD PUSH THE YOKE AT THE SAME TIME IN ORDER TO BE ABLE TO STOP THE PITCH UP AND CLIMB.
BLOWN NOSE WHEEL TIRE DURING TAKE-OFF ROLL. REMOVED TIRE, CLEANED AND INSPECTED MLG WHEEL ASSY AND INSTALLED NEW TIRE.
FLIGHT CREW REPORTED THE EMERGENCY LIGHTS WERE INOPERATIVE. MAINTENANCE FOUND THE BATTERIES HAD BEEN LEFT ON. THE BATTERIES WERE REPLACED, SYSTEM OPS CHECKED NORMAL.
FLT 4141 - SHORTLY AFTER TAKEOFF, LT ENGINE LOST POWER, NO TORQUE OR OIL PRESSURE. CREW DECLARED AN EMERGENCY, RETURNED TO FIELD, AIRCRAFT LANDED WITHOUT FURTHER INCIDENT. INSPECTION OF LT ENGINE REVEALED A PROBLEM WITH ENGINE FUEL PUMP DRIVE. MAINTENANCE REMOVED AND REPLACED LT ENGINE IAW MM 72-00-00. CONDUCTED ENGINE GROUND RUNS IAW 72-00-00. OPS CHECKED GOOD AND AIRCRAFT RETURNED TO SERVICE.
JST - FLT 4088 - ON APPROACH, SELECTED GEAR DOWN, NO SAFE GEAR INDICATION - NLG - CYCLED GEAR HANDLE 4 TIMES, STILL NO INDICATION. RETURNED TO PIT, DECLARED AN EMERGENCY. AIRCRAFT LANDED WITHOUT INCIDENT. MAINTENANCE REPLACED NLG DOWNLOCK MICROSWITCH. OPS CHECKED GOOD. AIRCRAFT RETURNED TO SERVICE.
FLT 4110 - WHEN SELECTING GEAR DOWN ON APPROACH TO A00, CREW UNABLE TO GET GEAR SAFE LIGHT NLG. CYCLED GEAR SEVERAL TIMES, STILL UNABLE TO GET SAFE LIGHT NLG. CREW ELECTED TO RETURN TO PIT, UNABLE TO VERIFY NLG DOWN, CREW DECLARED AN EMERGENCY. AFTER DECLARING AN EMERGENCY, NLG GREEN LIGHT ILLUMINATED. AIRCRAFT LANDED WITHOUT DIFFICULTY. EMERGENCY EVACUATION NORMAL. MAINTENANCE INSPECTED AND CLEANED NLG DOWNLOCK MICROSWITCH, CONDUCTED LANDING GEAR SERVICING. OPS CHECKED GOOD. ACFT RETURNED TO SERVICE.
FLT 4134, DURING TAKEOFF ROLL, CREW REPORTED LT AIRSPEED INDICATOR FAILED TO RESPOND NORMALLY. TAKEOFF ABORTED AND AIRCRAFT RETURNED TO THE GATE WITHOUT FURTHER INCIDENT. INSPECTED LEFT PITOT SYSTEM, TURNED ON PITOT HEAT AND DRAINED SYSTEM. PERFORMED OPERATIONAL CHECK IAW BAE MM 34-11-00. AIRCRAFT RETURNED TO SERVICE.
