CLIMBING OUT AFTER LOW APPROACH, BOTH THROTTLES AT 85% N1, HEARD A CLUNK AND FELT A SMALL BUMP. ALL LEFT ENGINE INSTRUMENTS HIGH. ITT SHOWED ALL RED LIGHTS. NO FIRE INDICATIONS OR WARNING, BROUGHT LEFT ENGINE TO IDLE, N1 CAME BACK TO IDLE AND ITT STAYED A LITTLE HIGH (827DEGC). FUEL FLOW ALSO AT IDLE (66), BUT N2 STAYED AT 94.4. SHUT DOWN LEFT ENGINE WITHOUT FURTHER PROBLEM.
INTERMITTENT FLASHING BLEED AIR WING DUCT FAIL AND BLEED AIR DETECT. MX TROUBLESHOOTING.
PILOTS PFD MOMENTARILY BLANKED OUT, ADI RETURNED WITH AIRSPEED & ALTITUDE FLAGGED AND MACH TRIM DISENGAGED. AFTER TRANSFERRING AUTOPILOT TO COPILOT SID AND RUNNING CHECKLIST FOR MACH TRIM FAILURE. MACH TRIM RE-ENGAGED. MAINTENANCE PERFORMED TESTS FOR GENERATOR SYSTEM AND AUTOPILOT WITH NO DEFECTS FOUND. WILL CONTINUE TO MONITOR.
THE NOSE GEAR DID NOT RETRACT FOR SEVERAL MINUTES WITH GEAR HANDLE PLACED UP.
AFTER RAISING THE LANDING GEAR HANDLE, IT WAS DISCOVERED THAT THE NOSE GEAR DOORS DID NOT OPEN AND THE NOSE GEAR DID NOT MOVE. LANDING GEAR SELECTOR VALVE WAS FOUND TO HAVE FAILED. REPLACEMENT OF SELECTOR VALVE CORRECTED THE DISCREPANCY.
SERVICE DOOR OPEN LIGHT ILLUMINATES IN FLIGHT. (ON AND OFF RANDOMLY. CORRECTED BY REPAIRING BROKEN WIRE ON DOOR SWITCH.
RH MLG INDICATION ILLUMINATED AT CRUISE FOR 5-10 MINUTES, INDICATION ONLY. GEAR (RH) WAS RETRACTED , NO GEAR "DISAGREE" FLASHING. REMOVAL OF #1 AND #2 RIGHT HAND DOWNLOCK SWITCHES CORRECTED THE INDICATION ISSUE.
MAIN LANDING GEAR FAILED TO EXTEND HYDRAULICALLY(3X ATTEMPTS.) MANUAL EXTENSION WAS SUCCESSFUL. CORRECTED THE FAULT BY REPLACING THE MLG SELECTOR VALVE. THE AIRCRAFT IS RETURNED TO SERVICE.
IN FLT, LEFT ENG FUEL LOW PRE LT ILLUM. MOMENTARILY AND WENT OUT. A SLIGHT FUEL SMELL WAS DETECTED. ON DESCENT WHEN APU STARTED, A STRONG FUEL SMELL FILLED CABIN-SHUT APU DOWN. OPERATIONAL CHECK OF #1 ENGINE WITH NO DEFECTS NOTED.APU ECU NOTED NO START EVENT PRIOR TO SUCCESSFUL START IN FLT. APU GROUND START AND OPERATION CHECK GOOD WITH NO FUEL SMELL.
HYDRAULIC SYSTEM NR 3 FAILURE. FAILED IN FLIGHT, 0 PRESSURE, 0 QUANTITY. FOUND SYSTEM 3A HYD PRESSURE LINE CHAFED THROUGH HOLE FROM SUPPORT CLAMP. IT WAS DETERMINED THE HYDARULIC PRESSURE AND RETURN LINES HAD INCORRECT SUPPORT CLAMPS INSTALLED FOR HYDRAULIC LINE USE CAUSING THE CLAMP RUBBER TO DETERIORATE AND CHAFE THE HYDRAULIC LINES. CORRECT CLAMPS HAVE BEEN INSTALLED.

ACRID ELEC ODOR EMANATING FROM PILOT'S ALTITUDE AWARENESS PANEL. REPLACEMENT OF THE PILOTS ALTITUDE AWARENESS PANEL RESOLVED THE ISSUE.
FLAP FAIL IN FLIGHT STUCK AT 20'.
WING ANTI-ICE DUCT FAIL LIGHT ILLUMINATED. INDICATION GONE WHEN WING ANTI-ICE TURNED OFF. PERFORMED WING ANTI-ICE OPERATIONAL TEST, COULD NOT DUPLICATE FAILURE ON GND. BLEED AIR FAILURE BIT BALLS WERE RESET BEFORE MX CHECKED THEM.
RNA-34BF "LOCKS UP" WHEN IN VICINITY OF FM RADIO STATIONS BROADCASTING ON 107.9 MHZ. INDICATIONS TO THE USER ARE THAT THE RADIO CANNOT BE TUNED TO ANOTHER FREQUENCY AND ALL OUTPUTS BECOME FROZEN OR ABSENT. MFG HAS BEEN MADE AWARE OF THE PROBLEM BY CANADA, SWEDEN, AND NOW US FLIGHT INSPECTION ORGANIZATIONS. INITIAL PROBLEMS REPORTED AT LEAST BY 2008, BUT NO RESOLUTION YET.
NUMBER TWO ENGINE 10TH STAGE BLEED AIR VALVE WOULD NOT OPEN FULLY. REPLACED BLEED AIR VALVE, OPS CK GOOD.
AT CRUISE FL350, THE BLEED AIR LEAK DETECT LIGHT (CENTER PANEL) INTERMITTENTLY BEGAN FLASHING. NOTICED THAT THE LEFT 10TH STAGE "DOLLS EYE" WAS TRIPPED. INITIALLY, DID NOT GET THE LEFT 10TH STAGE DUCT FAIL LIGHT. BEGAN A DESCENT TO A LOWER ALTITUDE (FL200 THEN FL110) TO RUN THE CHECKLIST. ACCOMPLISHED THE 10TH STAGE DUCT FAIL CHECKLIST. PER THE CHECKLIST THE LEFT 10TH STAGE WAS SELECTED OFF, ISOLATION V CLOSED. WHILE RETURNING TO AIRPORT, WE CONFIGURED TO BURN OFF FUEL TO GET DOWN TO LANDING WT. NOTICED THAT THE BLEED AIR LEAK DETECT LIGHT (CENTER PANEL) ILLUMINATED AND THE LEFT 10TH STAGE DUCT FAIL LIGHT WAS ILLUMINATED. PER THE CHECKLIST, THE LEFT ENGINE WAS RETARDED TO IDLE. ACCOMPLISHED A 20 FLAP SINGLE ENGINE (LEFT THROTTLE IN IDLE) APPROACH AND LANDING. DISCOVERED 10TH STAGE BLEED AIR VALVE CLAMP LOOSE, TIGHTENED CLAMP AND LEAK CHECKED OK.
LOST ALL HYDRAULIC PRESSURE ON NR 3 HYDRAULIC SYSTEM, DECLARED EMERGENCY AND LANDED WITHOUT INCIDENT. MAINTENANCE DISCOVERED HYDRAULIC PRESSURE HOSE, P/N 600-75123-107 RUPTURED. RU3A (M)
