AWAITING BOMBARDIER ENGINEERING DISPOSITION.

NOSE GEAR DID NOT RETRACT ON TAKEOFF. NOSE DOOR OPEN LIGHT REMAINED ILLUMINATED. ACCOMPLISHED GEAR DISAGREE CHECKLIST. MA ADJUSTED LH LANDING GEAR DOOR (NOSE) OPEN SWITCH S1GK TO CORRECT THIS ISSUE
NOSE GEAR DID NOT RETRACT. DOOR LIGHT REMAINED ON. COMPLETED GEAR DISAGREE CHECKLIST. FOUND LOOSE BACKSHELL AND CANNON PLUG CONNECTION GOING TO NOSE LANDING GEAR SELECTOR VALVE HARNESS. TIGHTENED BACKSHELL AND CANNON PLUG. NO FURTHER ISSUES.
INBOARD ANTI-SKID FAIL ANNUNCIATOR ON. LT WHEEL SPEED SENSOR WAS REPLACED. OPERATIONAL CHECK SATISFACTORY.
OUTBOARD ANTI-SKID LIGHT ILLUMINATED WHEN GEAR EXTENDED WOULD NOT RESET. FOUND ANTI-SKID CONTROL PANEL LT LIGHT MODULE INTERMITTENT. THE ISSUE WAS RESOLVED BY REPLACING THE ANTI-SKID CONTROL PANEL.

BLEED AIR FAIL LIGHT ILLUMINATED WHEN BLEED AIR IN CLOSED POSITION. REPLACEMENT OF THE PNEUMATIC SOV SOLVED THE ISSUE. NO FURTHER ACTION REQUIRED.
FLAPS FAIL ON INITIAL TAKE OFF. FAILED AT 18^. AIRSPEED REACHED 240 KIAS WITH NO OVER SPEED WARNING. COMPLIED WITH OVER SPEED INSPECTION. LUBED FLAPS.
AFTER TAKEOFF COULDNâT RAISE GEAR, THEN WOW INPUT LIGHT CAME ON. LIGHT WENT OUT AFTER 2 MIN. GEAR HANDLE REMAINED LOCKED DUE TO MOMENTARY WOW INPUT FAILURE, TROUBLESHOOTING CONTINUING.
UPON COMPLETION OF A LOW APPROACH, GEAR HANDLE SELECTED UP FOR CLIMBOUT. AFTER MAINS RETRACTED, NOSE GEAR REMAINED EXTENDED AND RED LIGHT IN HANDLE CONTINUED FLASHING. INITIALLY NO OTHER INDICATIONS. AFTER HOLDING AND APPROXIMATELY 40 MINUTES AFTER INITIAL MALFUNCTION, NOSE GEAR DOOR LIGHT ILLUMINATED. LIGHT IN THE HANDLE WAS FLASHING ENTIRE TIME.
RIGHT SIDE WINDSHIELD ANTI-ICE FAILURE IN CRUISE FLIGHT. PERFORMED WINDSHIELD ANTI-ICE FAILURE AS DIRECTED IN ABNORMAL PROCEDURE. CONTINUED ON ORIGINAL ROUTE TO PLANNED DESTINATION. APPROX 20 MIN LATER NOTICED CRACK OF OUTER PLY OF RIGHT SIDE WINDSHIELD. CRACK RAN FROM TOP TO BOTTOM ABOUT CENTER SCREEN. PERFORMED ABNORMAL PROCEDURE FOR WINDSHIELD OUTER FACE PLY FAILURE. CHANGED DESTINATIONS TO A CLOSER AIRPORT. ON DESCENT, OUTER PLY OF WINDSHIELD SEPARATED FROM AIRCRAFT. ONCE ON THE GROUND, MECH VISUALLY INSPECTED AIRCRAFT AND FOUND NO FURTHER DAMAGE.
REMOVED RUDDER TO NDI ATTACH FITTINGS. DURING INSPECTION A CRACK IN UPPER FITTING WAS DETECTED. DURING STRIPPING OF PAINT, DISCOVERED PITTING AROUND BUSHING INSERT. CORROSION WAS DEEP INTO BRACKET AT EDGE OF BUSHING. THIS UPPER HINGE FITTING AND BUSHING SITS AT BOTTOM WHERE MOISTURE WOULD SIT IN CAVITY.
WHILE MANEUVERING FOR APPROACH, CREW LOWERED LANDING GEAR. BOTH MAIN GEAR INDICATIONS WERE "DOWN AND LOCKED". NOSE INDICATOR DID NOT ILLUMINATE. NOSE DOOR OPEN LIGHT REMAINED ILLUMINATED AND AIRCRAFT BUFFETED INDICATING NOSE DOORS WERE OPEN. THE LIGHT IN THE GEAR HANDLE WAS NOT ILLUMINATED. THE CREW ABORTED THE APPROACH, CLIMBED AND HELD IN VFR CONDITIONS. CREW PERFORMED A FLY-BY AND TOWER PERSONNEL VERIFIED NOSE GEAR APPEARED DOWN. RETURNED AIRCRAFT TO HOLDING PATTERN TO REDUCE FUEL LOAD. DECLARED AN EMERGENCY WITH ATC AND PERFORMED AN UNEVENTFUL VISUAL LANDING. MAINTENANCE FOUND NOSE LOCK MICRO SWITCH BAD. REMOVED AND REPLACED. CYCLED GEAR SEVERAL TIMES. OPS CHECKED GOOD.
ON DESCENT THRU 160 BOTH THROTTLES WERE BROUGHT TO IDLE. NR 2 LOW OIL PRESSURE LIGHT CAME ON, AND LOW OIL PRESSURE TAPE INDICATED LOW. N1 LT 65 PERCENT, RT 34 PERCENT, POWER RE-APPLIED AND ENGINE RECOVERED. MX CHECKED ENGINE ON GROUND AND NO FAULT FOUND.
IN CRUISE FLIGHT, LEFT PILOT WINDSHIELD CRACKED THE ENTIRE VERTICAL LENGTH OF THE WINDOW. PRESSURIZATION WAS CHECKED AND ABNORMAL CHECKLIST FOR WINDSHIELD OUTER FACE PLY FAILURE WAS ACCOMPLISHED. ATC AND FICO WERE ADVISED AND TURN AROUND CONDUCTED. UNEVENTFUL LANDING WAS CONDUCTED.
ANC - AIRPORT RAMP. AIRCRAFT OBSERVED TO BE LEAKING FUEL FROM THE LOWER TAIL SECTION WITHIN A FEW MINUTES FOLLOWING APU START. THE AIRCRAFT ENGINES WERE NOT STARTED. THE APU WAS SHUT DOWN WITHOUT FURTHER INCIDENT AND THE FLIGHT CANCELLED. INVESTIGATION REVEALED THE APU LOW-PRESSURE FUEL FILTER COVER MOUNTING EAR FAILED AT THE SCREW MOUNTING HOLE AND SEPARATED FROM THE FUEL CONTROL UNIT. (X)
