DURING 100 HOUR INSPECTION FOUND STAR FLEX ARM CRACKED, APPROX 60MM OTBD FROM THICK PORTION OF ARM. CRACK APPROX 40MM LONG ON TOP SURFACE.
DURING STARTER GENERATOR 300HR INSP FOUND BRUSH RIVET MISSING AND TENSION SPRING BROKEN. SECOND OCCURANCE OF BROKEN BRUSH, RIVET AND SPRING IN 5 MONTHS 142+12HRS, SN 10938XL2 TIME SINCE O/H 676+16 TIME SINCE REPAIR 142+12 PREVIOUS OCCURANCE SN 10260 8-24-2010. ACTT4754+42
ENROUTE TO A LOCAL PUBLIC RELATIONS EVENT THE GENERATOR CAUTION LIGHT ILLUMINATED. INITIATED GENERATOR FAILURE PROCEDURES AND GENERATOR RESET PROPERLY. SHORTLY THEREAFTER AN ACRID, BURNING ODOR FILLED COCKPIT. IMMEDIATELY SECURED GENERATOR AND SET UP FOR LANDING AT AN AIRPORT APPROX (1) MILE AWAY AND LANDED WITHOUT INCIDENT. MECHANIC ARRIVED AND REPLACED GENERATOR AND BATTERY, AND ACFT WAS RETURNED TO SERVICE. MECHANIC NOTES: GENERATOR FAILED IN FLIGHT. CREW SMELLED SMOKE IN THE CABIN. PILOT LANDED AT CLOSEST AIRPORT. MECHANIC REPLACED THE STARTER/GENERATOR AND THE ACFT BATTERY AS THE BATTERY WOULD NOT CHARGE.
DURING CRUISE FLIGHT, THE ENGINE FIRE LIGHT ILLUMINATED. DURING THE DESCENT FOR A PRECAUTIONARY LANDING, THERE WAS NO SMOKE TRAILING THE AC AND THE ENGINE FIRE LIGHT EXTINGUISHED AND ILLUMINATED SEVERAL TIMES. AFTER SECURING THE AC UPON LANDING, THE PILOT INSPECTED THE ENGINE AREA AND NO FIRE DAMAGE WAS NOTED. THE MAINTENANCE STAFF CAME OUT AND FOUND MOISTURE IN THE CONNECTOR PLUG ON THE FIRE DETECTION SYSTEM. AFTER CLEANING AND INSPECTING THE SYSTEM, THE PILOT PERFORMED A CHECK FLIGHT AND THE AC WAS RETURNED TO SERVICE. (K)
DURING PATIENT TRANSFER FLIGHT FROM OUTLYING HOSPITAL TO MAIN HOSPITAL, FUEL FUMES NOTED BY ALL CABIN OCCUPANTS. VENTS AND HEATER DUCTS MANIPULATED WITH NO DISCERNABLE EFFECT. SMELL OF FUEL FUMES INCREASED IN INTENSITY TO UNPLEASANT LEVELS UPON REDUCTION OF COLLECTIVE DURING APPROACH TO LANDING PHASE. SUBSEQUENT INVESTIGATION BY DUTY MECHANIC REVEALED FAULTY HMU. MECHANIC FOUND FUEL LEAKING INTO ENGINE FROM THE P2 SENSE LINE.
AIRCRAFT WAS IN STRAIGHT AND LEVEL FLIGHT 10 MINUTES FROM T/O AT 7000 FT AGL. CYCLIC STICK JUMPED TO THE STOP AT THE 3 OâCLOCK POSITION. A/C ENTERED A RT BANK (APPROX. 10 DEGREES). IMMEDIATE RECOVERY WAS MADE. FELT CONTINUOUS FEEDBACK IN THE RT QUADRANT. SLOWED THE A/C TO 65 KTS TO REDUCE FEEDBACK ON THE CYCLIC. ABORTED THE MISSION AND TURNED THE A/C BACK TOWARDS THE LOCAL AIRPORT AND CONTACTED TOWER. ELECTED TO DECLARE A (PAN) AND SET UP FOR A NORMAL APPROACH. MADE MINIMUM CONTROL INPUTS AND LANDED WITHOUT DELAY AND NO HOVERING. DID NOT CONDUCT A "RUN ON" BECAUSE HAD NO PRESSURE ON THE CONTROLS AT THE REDUCED SPEED. MAINT CALLED AND A HYDRAULIC TEST CONDUCTED WITH NO FURTHER PROBLEMS. MECHANIC PLANNED TO SWITCH SIDES WITH THE SERVOS AND SEE IF PROBLEM FOLLOWED. DISCOVERED WHAT IS BELIEVED TO BE AN ISSUE IN THE PILOT VALVE. REPLACED THE SERVO, A/C RETURNED TO SERVICE AFTER FLIGHT CHECK.
