Part: SKIN — BIRD STRIKE (FWD FUSELAGE)

BIRD STRIKE REPORTED DURING TAKEOFF, AND CREW RETURNED TO FIELD. ACCOMPLISHED BIRD STRIKE INSPECTION FOR THREE STRIKES ON THE LEFT SIDE OF THE NOSE AND ONE STRIKE ON THE RIGHT SIDE. THERE WAS NO DAMAGE TO THE AIRCRAFT OR EITHER ENGINE.

CREW REPORTED A BANG TYPE NOISE HEARD WHEN POWER WAS APPLIED FOR TAKEOFF. REPLACED THE PRESSURIZING AND DUMP VALVE. OPS CHECK GOOD. SHOP TEST SHOWED NO INTERNAL FAILURE TO THE P AND D VALVE.

Part: OUT FLOW VALVE — FAILED (FUSELAGE)

CREW REPORTS, UNABLE TO PRESSURIZE AIRPLANE AFTER TAKEOFF. CAN CONTROL OUTFLOW VALVE OPEN AND CLOSE. THE RIGHT FLOW CONTROL VALVE WOULD NOT TURN ON. REPLACED FLOW CONTROL VALVE AND CONNECTOR PINS. THE SHOP FOUND THE FLOW CONTROL VALVE HAD LOW AIR FLOW CAUSED BY A FAILED BUTTERFLY SEAL, A SENSE LINE WITH A HOLE IN IT, A BAD FILTER CIRCUIT AND FROZEN MOTOR BEARINGS.
