JUMP SEAT 1L FWD FLASHLIGHT RED LIGHT NOT FLASHING. REPLACED BATTERY.
SHORTLY AFTER T/O THERE WAS A HOT SMELL IN THE COCKPIT FOLOWED BY A CALL FROM THE AFT F/A ATATING THAT THE CAPT. SIDE AFT LAV SMOKE ALARM WAS GOING EXPERIENCED ODOR, A SHORT TIME LATER THE RIGHT AFT LAV SMOKE ALARM WENT OFF. BEFORE THE APPROACH STARTED, BOTH ALARMS STOPPED AFTER BEING ON FOR A LONG TIME. THEN BOTH ALARMS CAME BACK ON. F/A REPORTED A BURNING SMELL, BUT NO VISIBAL SMOKE. AFTER LANDING, THE MID F/A SMELLED AN ELECTRICAL ODOR. REPLACED COALESCER BAGS. PERFORMED DUCT BURNOUT. CHECKED ACM FOR LEAKS, NONE FOUND. RAN ENGINES.
SMOKEY SMELL OCCURRED AT 3500 FT WHILE ON APPROACH. SMOKE DETECTOR IN LT AFT LAV ACTIVATED, PWR SETTINGS WERE VARING GREATLY DUE TO STRONG GUSTY WINDS. SMELL DISSIPATED ON LAND. RAN ACFT TO T/O PWR AND VARIOUS PWR SETTINGS, NO PROBLEMS NOTED. ACFT OK FOR SVC.
DURING FLT, BURNING ODOR WAS NOTICED IN CABIN AND AFT LAV SMOKE DETECTOR WENT OFF. A/C RETURNED TO DEN. SUSPECT LEAK IN APU SYSTEM. DEACTIVATED APU. PERFORMED DUCT BURN-OUT PER MM. REPLACED COALESCER BAGS.
THE UPPER, AFT SKIN PANEL OF THE HORIZONTAL STABILIZER WAS FOUND CRACKED DURING AN INSPECTION BY EO 9-61905-3AD, (LT AND RT) THE DAMAGE WAS FOUND AT HMV AND REPAIRED PER ERA 333575-14AD.
THE FUSELAGE SKIN WAS FOUND DENTED AT FS 218 AND STRINGER 19L. THE DAMAGE WAS REPAIRED PER ERA 333534-14 AT HMV.
GROUND DAMAGE WAS DISCOVERED TO THE FUSELAGE SKIN AT FS 228 BETWEEN STRINGERS 17L AND 18L. THE DAMAGE WAS REPAIRED AT HMV PER ERA 333535-14.
ON FINAL APPROACH WITH AIRSPEED AT 150 KTS THE F/A AND PILOTS SMELLED A HOT ODOR WITH BOTH ENG AND WING A/I ON. SMELL DISSIPATED WHEN WING A/I TURNED OFF. FOUND OIL ON APU INLET. PERFORMED DUCT BURNOUT.
THE RT VERTICAL STABILIZER-TO-FUSELAGE ATTACH ANGLE WAS FOUND WITH A 1.5 INCH CRACK AT FS 1463. THE CRACK WAS ROUTED OUT AND A STEEL REPAIR ANGLE INSTALLED PER ER/A 366856-14, REV A.
JUMPSEAT 1L FLASHLIGHT RED LIGHT NOT FLASHING. REPLACED FLAHSLIGHT BATTERY AND BULB.
AFT LAV SMOKE DETECTOR ACTIVATED ON SHORT FINAL. ENG AND AIRFOIL A/I HAD BEEN TURNED ON PASSING 15,000 FT AND TURNED OFF PASSING 2,000 FT. BURNING ODOR WAS NOTICED BUT NO SMOKE. REPLACED WATER BAGS, OPERATED PACKS NO ODORS NOTICED.
THE FUSELAGE SKIN WAS BADLY DENTED AT FS 237 BETWEEN STR 17L AND 18L. STRINGER 17L WAS CRACKED AND THE FS 237 FORMER WAS CRACKED AND BUCKLED. THE STRINGER AND FRAME WERE REPAIRED PER THE SRM. THE SKIN DAMAGE WAS CUTOUT AND REPAIRED WITH AN EXTERNAL REPAIR DOUBLER AND INTERNAL FATIGUE DOUBLER PER ER/A 364128-14. DAMAGE WAS CAUSED BY GROUND EQUIPMENT IMPACT.
EMERGENCY FLOOR TRACK BULB FWD OF 31D INOP. REPLACED LIGHT FIXTURE, CKS OK.
AFTER TAKEOFF, NOSE GEAR UNSAFE LIGHT ILLUMINATED, AIR NOISE FROM NOSE GEAR AREA. DISASSEMBLED NOSE STRUT, FOUND THREE PINTS OF WATER AND ICE INSIDE STRUT. CLEANED STRUT, INSTALLED NEW SEALS, SERVICED PER MM AND OPERATION NORMAL.
THE LT LOWER FORWARD FUSELAGE WAS PUNCTURED BY GROUND EQUIPMENT RESULTING IN DAMAGE TO THE SKIN, FRAMES, AND STRINGERS AT FS 229-294. ACCOMPLISHED REPAIRS PER DL ERA 271965-14.
DURING INSPECTION, AFT AIRSTAIR EMERGENCY LIGHT FOUND INOP. REPLACED LIGHT ASSEMBLY, OPERATION NORMAL.
DURING CRUISE, HAD SMOKE AND ODOR IN CABIN AFTER ANTI ICE WAS TURNED ON. DIVERTED TO TYS SMOKE DISSIPATED WHEN ANTI ICE WAS TURNED OFF. CHECKED APU AND ENGINE OIL QTY. FOUND NORMAL. PERFORMED DUCT BURN-OUT PROCEDURES, RETURNED TO SERVICE.
FOUND ON B-1 LETTER CHECK, CRACK AT OUTBD FWD WHEEL WELL FRAME. REPAIRED PER SRM 53-05 FIG 78A.
ON TAKEOFF AND APP HAD SMOKE/FUMES IN CABIN. RAN PACKS, ENGINES AND APU COULD NOT DUPLICATE, PERFORMED DUCT BURNOUT, RETURNED TO SERVICE.
