ACTUATOR CONTROL TUBE END FITTING WORKING & RIVETS ATTACHING END FITTING WORKING, FOUND DURING 12 MO INSPECTION.
THIS WAS A HOSPITAL TRANSFER FROM CRESTON, IA TO DES MOINES, IA. ON THE FIRST LEG BETWEEN KOXV AND CRESTON APPROXIMATELY HALF WAY (15MIN FLIGHT TIME) THE T/R CD INOP CAUTION LIGHT CAME ON AND WENT OFF AGAIN AFTER A FEW SECONDS. AROUND 5MIN LATER IT CAME ON AGAIN AND STAYED ON. THE EMG PROCEDURE STATES "LAND AS SOON AS PRACTICAL". I THEREFORE DECIDED TO ABORT THE FLIGHT AND RETURN BACK TO KOXV AS THIS IS WHERE THE AIRCRAFT'S MAINTENANCE FACILITY IS LOCATED. I REPORTED THE ISSUE TO MERCY COMM CENTER AND RLANDED BACK AT KOXV WITHOUT FURTHER ISSUES.
PILOT MADE PRECAUTIONARY LANDING IN FIELD DUE TO THE ODOR OF SMOKE IN THE COCKPIT. FOUND ANTI COLLISION LIGHT SWITCH FAILED.
TR FLAPPING BEARING ELASTOMER FAILURE & DELAMINATION AT 1054.25 HOURS. SLL IS 15,000 HOURS.
DURING CRUISE, ENGINE CHIP CAUTION WARNING LIGHT ILLUMINATED, NO OTHER INDICATORS WERE EVIDENT OF ENGINE DAMAGE, ACFT LANDED. MECHANIC FOUND CHIPS ON BOTH CHIP DETECTORS AND IS EVALUATING CORRECTIVE ACTIONS.
REMOVED LOWER & MAST TRANSMISSION AND FREEWHEEL CHIP DETECTORS. ONLY THE TRANSMISSION LOWER DETECTOR HAD A SMALL AMOUNT OF FUZZ. CLEANED DETECTORS AND REINSTALLED, RAN UNIT AND REINSPECTED AFTER RUN AND RETURNED TO SERVICE IAW MM 63-14 & FIG 63-2.
WHILE ON THE FIRST LEG OF A MEDICAL FLIGHT, THE TRANSMISSION CHIP CAUTION LIGHT ILLUMINATED. ONCE THE CHIP LIGHT CAME ON, DIVERTED ACFT. TRANSMISSION CHIP REMAINED ILLUMINATED UNTIL THE ACFT WAS SHUTDOWN, WHEN THE BATTERY WAS TURNED BACK ON, TRANSMISSION CHIP LIGHT DID NOT COME BACK ON. INSPECTED AND ACFT RETURNED TO SERVICE. REMOVED M/R GB AND FREEWHEELING UNIT CHIP PLUGS. FOUND FUZZ ON LOWER CHIP PLUG. CLEANED ALL PLUGS AND REINSTALLED. GROUND RUN ACFT FOR 10 MINUTES. REINSPECTED LOWER CHIP PLUG-FOUND CLEAN. REINSTALLED LOWER CHIP PLUG. ALL WORK IAW MM 63-14 AND TABLE 63-2.
REMOVED CHIP DETECTORS. LOWER TRANSMISSION MAGNETIC CHIP DETECTOR HAD SOME FUZZ AND A SINGLE SPIKE LIKE PARTICLE JUST LONG ENOUGH TO BRIDGE THE DETECTOR. CLEANED DETECTOR, GROUND RAN AND REINSPECTED DETECTOR. RETURNED ACFT TO SERVICE CLASSIFYING CHIP AS INSIGNIFICANT DEBRIS IAW BHT 407 MM FIGURE 63-13.
THE MECHANIC FOUND A CRACK WHILE PERFORMING AN AIRWORTHINESS INSP ON THE LT UPPER TAILBOOM TO HORIZ STABILIZER SUPPORT IN THE BEND RADIUS INTENDING TO OTBD EDGE FWD OF THE FIRST SCREW. NO HIDDEN DAMAGE WAS FOUND ON THE TAILBOOM UPON REMOVAL OF THE SUPPORT. THE CRACK MAY HAVE BEEN CAUSED BY PRELOADING IN THE BEND RADIUS DUE TO MISSING PFTE TAPE (C-460) ON FRAYED SURFACE OF THE LIQUID SHIM.
PILOT STATED TORQUE WENT TO 110.6 FOR (1) SECOND DURING START. CLEARED FAULT FROM GAUGE IAW MM CH 76, PROPULSION INSTRUMENT GUIDE AND EMC-35A MX GUIDE. NO FADEC FAULTS NOTED. COMPLIED WITH MM 71-14, 71-7, 71-8, TABLE 95-4 AND MM 72-00-00 PARA 1.B, PAGE 622. RETURNED ACFT TO SERVICE,
