WHILE ON BASE FINAL FOR TAXIWAY, THE NR 1 ENG OIL PRESS STARTED FLUCTUATING WITH OIL TEMPERATURE NORMAL & TORQUES MATCHED. AT SHORT FINAL THE ENG CHIP CAUTION LIGHT ILLUMINATED, SIDE STEPPED THE RAMP & LANDED 100 YARDS SHORT OF NORMAL LANDING AREA. SHUTDOWN NR 1 ENG FOLLOWED BY NR 2. INSPECTED & FOUND OIL ALL OVER THE COMPRESSOR. SPUN COMPRESSOR BY HAND, RUBBING & BEARING NOISE COULD BE HEARD. PULLED CHIP DETECTORS & FOUND METAL SHAVINGS ON THE UPPER PLUG.
ON STARTUP NR 1 ENG CHIP LIGHT ILLUMINATED. UPON INSPECTION OF CHIP DETECTORS FOUND UPPER DETECTOR TO HAVE EXCESSIVE AMOUNTS OF METAL. R & R TURBINE.
DURING FLIGHT WITH 2 MEDICAL PERSONNEL AND PILOT ABOARD HEADING BACK TO AIRPORT NR 1. ENGINE MADE A LOUD NOISE AND FLAMED OUT, PILOT NOTED ENGINE INDICATIONS AND SECURED THAT ENGINE, DECLARED AN EMERGENCY WITH TOWER AND LANDED AC WITHOUT INCIDENT. AFTER INVESTIGATION FOUND NR 1 ENGINE HAD INGESTED A CLOTH RAG, NR 1 ENGINE COMPRESSOR RECEIVED DAMAGE TO COMPRESSOR BLADES AND STATORS. (K)
AFTER STORMSCOPE UNIT USED FOR ONE FLIGHT AND WARMED UP, NEXT FLIGHT, UNIT WILL NOT DISPLAY. REMOVED AND REPLACED.
SHAFT BROKEN. REMOVED AND REPLACED.
NR 1 BLADE PATH BREAKING OUT NOZZLE NR 2. REMOVED AND REPLACED.
CRACKED DIAPHRAGM. REMOVED AND REPLACED.
FOLLOWING TAKEOFF, RPM WARNING LIGHT, ATTEMPTED TO BEEP ENGINES DOWN - ONLY NR 1 RESPONDED. RETURNED AND EMERGENCY LANDING MADE. FOUND NR 2 ACTUATOR INOPERABLE. PARTS ON ORDER.
TRANSCEIVER WILL NOT TRANSMIT WHEN UNIT GETS HOT.

UPPER ANTI-COLLISION LIGHT INOPERATIVE. REMOVED AND REPLACED.
GYRO IS TUMBLED AND SPINS OUT OF CONTROL WHEN POWER IS APPLIED. TIME SINCE OVERHAUL 57.3 HRS.
