PERFORMED NORMAL START, AFTER BRINGING BOTH THOTTLES TO FLY, ENG # 2 TWIST GRIP CAUTION LIGHT REMAINED ON CAD. UNABLE TO EXTINGUISH CAUTION LIGHT, PERFORMED NORMAL SHUTDOWN, A/C NEVER LEFT GROUND
PILOT REPORTED #1 ENGINE FIRE WARNING LIGHT DURING FLIGHT. THIS LIGHT INITIALLY BEGAN TO FLICKER, THEN WENT ON SOLID WITH AURAL TONE. THE PILOT CHECKED ALL ENGINE INSTRUMENTATION, AND THEY ALL WERE ALL IN NORMAL RANGE. HE THEN PERFORMED A VISUAL WITH THE ASSISTANCE OF THE CREW WITH NO VISUAL INDICATION OF A FIRE. AT THIS TIME, THE LIGHT BEGAN TO FLICKER AGAIN, AND THEN WENT OUT. THE PILOT THEN REDUCED POWER, AND CHOSE TO CONTINUE FLIGHT TO THE PLANNED AIRPORT DESTINATION. AFTER ABOUT ONE MINUTE, A BRIEF BURNING SMELL WAS FOLLOWED BY THE #1 ENGINE FIRE LIGHT AND AURAL TONE AGAIN. AT THIS TIME, THE PILOT ESTABLISHED OEI/EMERGENCY PROCEDURES. THE FIRE BUTTON WAS DEPRESSED (BOTTLE DISCHARGED) AFTER THE N1 DROPPED BELOW 50%. AFTER ONE MINUTE, AND THE #1 ENGINE FIRE LIGHT STILL ILLUMINATED, THE #2 FIRE BOTTLE WAS DISCHARGED AS WELL. AN UNEVENTFUL SINGLE ENGINE RUN ON LANDING WAS PERFORMED WITH NO EVIDENCE OF FIRE FOUND BY EITHER THE PILOT OR MYSELF FOLLOWING SHUTDOWN OF THE REMAINING ENGINE. TROUBLESHOOTING DETERMINED THAT THE #1 ENGINE AFT OUTBOARD FIRE PROBE TO BE CLOSING BEFORE THE CORRECT TEMPERATURE (PROBE ORDERED). AS FOR THE BRIEF BURNING SMELL THAT OCCURRED DURING THE SECOND LIGHT EVENT, THE SOURCE COULD NOT BE IDENTIFIED. THIS AIRCRAFT REMAINS OUT OF SERVICE AT THE TIME OF THIS SUBMISSION AWAITING THE FIRE DETECTORS AND REPLACEMENT FIRE BOTTLES.
PILOT MADE A PRECAUTIONARY LANDING AFTER CREW COMPLAINED OF FUEL ODOR IN THE CABIN AREA. ARRIVED ABOUT AN HOUR AFTER THE OCCURRENCE. THERE WAS NO ODOR OF FUEL. I INSPECTED THE AIRCRAFT BELLY, FUEL CAP, SUMP DRAINS, BELLY WEEP HOLES, DECK DRAINS, ENGINE HOT END GANG DRAINS, BEHIND THE MEDICAL WALLS IN THE AREA OF THE FUEL VENT LINES, MEDICAL FLOORING, AND FOUND NO EVIDENCE OF FUEL OR FUEL ODORS. A 10 MINUTE OPS CHECK FLIGHT WAS CONDUCTED WITH NO FUEL ODORS DETECTED IN THE CABIN AREA. POST FLIGHT INSPECTION OF THE AREAS LISTED ABOVE REVEALED NO FUEL OR FUEL ODORS. FUEL ODOR COULD NOT BE DUPLICATED. THE AIRCRAFT WAS THEN RETURNED TO SERVICE.
APPROXIMATELY 5 MINUTES FOLLOWING DEPARTURE FROM BASE, THE MEDICAL CREW IN THE CABIN REPORTED SMELLING WHAT WAS BELIEVED TO BE A FUEL ODOR COMING FROM THE REAR OF THE AIRCRAFT. THE PILOT IMMEDIATELY ABORTED THE FLIGHT, AND RETURNED TO BASE WITHOUT INCIDENT. AFTER AIRCRAFT LANDED, IN GROUND IDLE ON NR 1 ENGINE AND NR 2 ENGINE FOUND NO LEAKS OR FUEL FUME ODORS. INSPECTED ALL VISIBLE FUEL LINES AND DRAINS FOR EVIDENCE OF FUEL WITH NONE DETECTED. INSPECTED BOTTOM OF AIRCRAFT FOR VISABLE FUEL DRIPS FROM FUEL SUMP DRAINS AND FUSELAGE DRAIN HOLES WITH NO FUEL LEAKS OR FUMES DETECTED IAW MM 28. PILOT PERFORMED OPERATIONAL FLIGHT OF THE AIRCRAFT WITH MAINTENACE IN THE BACK TO SEE IF THE FUEL ODOR WAS DUPILICATED WITH NO DISCREPANCIES NOTED
PILOT REPORTED, CREW MEMBER SMELLED A FUEL ODOR IN THE CABIN AREA. FLIGHT WAS ABORTED. AIRCRAFT RETURNED TO BASE. MAINTENANCE NOTIFIED. AIRCRAFT AND ENGINES INSPECTED FOR FUEL LEAKS. NONE FOUND. A 22 MINUTE OPS CHECK FLIGHT WAS CONDUCTED WITH NO FUEL ODOR DETECTED. POST OPS CHECK FLIGHT REVEALED NO FUEL LEAKS OR ODORS ON, IN, OR AROUND THE ENGINES OR AIRFRAME. SOURCE OF FUEL ODOR UNKNOWN. ODOR WAS UNABLE TO BE DUPLICATED AT THIS TIME.
